これは、反復測定と縦断的追跡調査を伴う前向き、無作為化、対照臨床試験です。 メキシコのアグアスカリエンテスにある施設の腎臓スクリーニングプログラムを通じて、糸球体濾過率が保たれているにもかかわらず持続的なアルブミン尿が認められる、早期段階の慢性腎臓病(CKD)を持つ14歳から35歳の参加者を募集します。 介入は、持続的な生活習慣の改善と腸内細菌叢の調節を通じて、腸-腎臓軸を標的としています。 主要な機序仮説は、腸内細菌叢の異常を減少させることで、腸内細菌由来の尿毒症毒素と全身性炎症が減少し、腎臓病の進行の早期マーカーを潜在的に軽減する可能性があるというものです。
18歳以上の参加者からは文面によるインフォームド・コンセントを取得します。 18歳未満の参加者については、親または法定後見人からのインフォームド・コンセントと併せて、文面による同意を取得します。

適格基準:
- 持続性アルブミン尿(ACR >30 および <300 mg/g)。
- 推算GFR >60 mL/min/1.73 m²。
- 特定可能な二次性腎疾患がないこと。
除外基準:
- 低アルブミン血症。
- ネフローゼ症候群。
- 持続性マクロアルブミン尿(ACR >300 mg/g)。
- 二次性または遺伝性腎疾患。

プラセボコンパレーター:コントロール
参加者は、同じ管理された食事と構造化された運動プログラムに加えて、共生体と区別がつかない経口プラセボを1日1回、180日間投与されます。

1. 生活習慣介入(食事+運動) 2. CKDで説明される腸内細菌叢異常および腸内尿毒症性代謝産物の蓄積に関連する慢性炎症反応におけるそれらの役割は、IL-1β、IL-6、TNF-αを用いて定量化されます。
