主な目的は、切除不能な LA-NSCLC 患者における、毎週の同時化学療法と併用した低分割放射線療法とそれに続く低ブースト療法の安全性と有効性を評価することです。

詳細な説明
患者は毎週、ドセタキセル(25mg/㎡)とネダプラチン(25mg/㎡)を1日ずつ4サイクル受け、同時に初回と初回で40Gy/10回と28Gy/7回の分割コース胸部放射線療法を受ける。 2番目のコースはそれぞれ1か月の休憩があります。
主要エンドポイントは無増悪生存期間であり、これは放射線療法の初日から疾患が進行する日までの経過時間です。
無増悪生存期間はカプランマイヤー法を使用して計算されます。毒性は CTCAE v. 4.0 に従って等級付けされます。

- NSCLCの病理学的確認。
- 患者は、固形腫瘍における反応評価基準(RECIST)基準に基づいて測定可能または評価可能な病変を有する。
- PET/CT、CT、またはMRIによって確認された切除不能なIIIA相(N2)およびIIIB相肺がん。
- 根治的照射の最小線量である60Gyを照射した場合、肺全体のV20≧35%。
- 東部協力腫瘍学グループ (ECOG) のパフォーマンス ステータス 0-1。
- 以前に化学療法を受けている、または治療歴がない
- 過去に胸部放射線療法、免疫療法、または生物療法を受けていない
- ヘモグロビン≧10 mg/dL、血小板≧100000/μL、好中球絶対数≧1500/μL
- 血清クレアチニンが正常上限値(UNL)の1.25倍以下、またはクレアチニンクリアランスが60 ml/分以上

- FEV1 >0.8L

- 過去または最近の別の悪性腫瘍(非黒色腫皮膚がんまたは上皮内子宮頸がんを除く)
- 化学療法の禁忌
- 悪性胸水または心嚢水。
- 妊娠中、授乳期の女性、または初回接種の14日前までに妊娠検査を受けていない女性

実験的:分割コース放射線療法
放射線療法は、同時統合ブースト (SIB) 強度変調放射線療法 (IMRT) を使用して行われます。
患者は初期コースで緩和線量、すなわち肉眼腫瘍まで40Gy/10fで放射線照射を受けます。
この病気は、最初のコースの終了1か月後にCTを使用して再評価されます。
RECIST基準に従って部分寛解を達成し、肺機能が回復した患者は、追加のブーストを受ける必要があります。
2 番目のコースでは、腫瘍が再シミュレーションされます。
その後、残存腫瘍を 2 回目の放射線療法で治療しました。
28Gy/7fの線量が残存腫瘍に送達されます。
同時化学療法は、ドセタキセル (25mg/㎡) とネダプラチン (25mg/㎡) を毎週 1 日ずつ投与します。
