Radiothérapie hypofractionnée suivie d'un rappel hypofractionné avec chimiothérapie concomitante hebdomadaire pour le cancer du poumon non à petites cellules de stade III non résécable : une étude prospective de phase II à un seul bras
L'objectif principal est d'évaluer l'innocuité et l'efficacité de la radiothérapie hypofractionnée suivie d'un hypo-boost combiné à une chimiothérapie hebdomadaire concomitante chez les patients LA-NSCLC non résécables.

Description détaillée
Les patients reçoivent quatre cycles hebdomadaires de docétaxel (25 mg/㎡) et de nédaplatine (25 mg/㎡), chacun d'une durée d'un jour, en même temps qu'une radiothérapie thoracique fractionnée de 40 Gy/10 fractions et de 28 Gy/7 fractions administrées dans la première et la deuxièmes cours, respectivement, avec une pause d'un mois.
Le critère de jugement principal est la survie sans progression, c'est-à-dire le temps qui s'écoule entre le premier jour de radiothérapie et la date à laquelle la maladie progresse.
La survie sans progression sera calculée à l'aide de la méthode de Kaplan-Meier. Les toxicités seront classées selon CTCAE v. 4.0.

Critère d'intégration:
- Confirmation pathologique du NSCLC.
- Les patients présentent des lésions mesurables ou évaluables selon les critères RECIST (Response Evaluation Criteria in Solid Tumours).
- Cancer du poumon de phase IIIA(N2) et IIIB non résécable confirmé par TEP/TDM, TDM ou IRM.
- Poumon entier V20>=35 % lors de l'administration de 60 Gy, qui est la dose minimale d'irradiation radicale.
- Statut de performance de l'Eastern Cooperative Oncology Group (ECOG) 0-1.
- Précédemment traité par chimiothérapie ou naïf de traitement
- Aucun antécédent de radiothérapie thoracique, d'immunothérapie ou de biothérapie
- Hémoglobine≥10 mg/dL, plaquettes≥100000/μL, nombre absolu de neutrophiles ≥1500/μL
- Créatinine sérique ≤ 1,25 fois la limite normale supérieure (UNL) ou clairance de la créatinine ≥ 60 ml/min
- Bilirubine ≤ 1,5 fois UNL, AST (SGOT) ≤ 2,5 fois UNL, ALT (SGPT) ≤ 2,5 fois UNL, phosphatase alcaline ≤ 5 fois UNL
- VEMS > 0,8 L
- CB6 dans les limites normales
- les patients et leur famille ont signé les consentements éclairés
Critère d'exclusion:
- Autre tumeur maligne antérieure ou récente, à l'exception du cancer de la peau autre que le mélanome ou du col de l'utérus in situ
- Contre-indication à la chimiothérapie
- Épanchement pleural ou péricardique malin.
- Femmes enceintes, en période d'allaitement ou sans test de grossesse 14 jours avant la première dose
- Femmes qui ont la probabilité de grossesse sans contraception
- Tendance à l'hémorragie
- Dans d'autres essais cliniques dans les 30 jours
- Addicts à la drogue ou à l'alcool, malades du SIDA
- Crise incontrôlable ou patients psychotiques sans capacité de maîtrise de soi
- Allergie sévère ou idiosyncrasie
- Ne convient pas à cette étude jugée par les chercheurs

Expérimental: radiothérapie fractionnée
La radiothérapie est administrée à l'aide d'une radiothérapie à modulation d'intensité (IMRT) simultanée intégrée (SIB).
Les patients sont irradiés à une dose palliative au cours initial : 40Gy/10f jusqu'à la tumeur macroscopique.
La maladie est réévaluée un mois après la fin du traitement initial par TDM.
Le patient qui a obtenu une rémission partielle selon les critères RECIST et qui a récupéré sa fonction pulmonaire devrait recevoir le coup de pouce supplémentaire.
Au deuxième cours, la tumeur est re-simulée.
La tumeur résiduelle a ensuite été traitée avec le deuxième cycle de radiothérapie.
Une dose de 28Gy/7f est délivrée au résidu tumoral.
La chimiothérapie concomitante consiste en une administration hebdomadaire de docétaxel (25 mg/㎡) et de nédaplatine (25 mg/㎡), chacun d'une durée d'un jour.
