この研究は、意図しない妊娠のリスクがある 16 歳から 18 歳までの 2 つの救急部門 (ED) のいずれかに受診した女性を対象とした前向きコホート研究です。
私たちの主な目的は、救急部門ベースの避妊薬を服用している 15 歳から 18 歳の女性の間で、避妊を開始する意思を評価することです (「高い」= 非常に/ある程度可能性がある vs 「低い」= わからない/可能性は低い/リッカート尺度の調査では絶対にない)。カウンセリング。
私たちの二次的な目的は次のとおりです。
- 同じ母集団の中で、インデックス ED 訪問後 4 週間以内の紹介サイトへの出席として定義される避妊ケアの紹介の完了を評価します。
- 最終的に避妊を開始した人の割合を、電子医療記録文書 (すなわち、訪問メモ、手順メモ、投薬レビュー) および参加者レポート (フォローアップ コール) を通じて評価します。
- 定性的な面接方法を使用して、A) 避妊を開始する意思を表明する、B) 避妊ケアへの紹介を完了する、C) ハイリスクの思春期のこのユニークなグループの間で最終的に避妊を開始するための決定に影響を与える態度、障壁、ファシリテーターを調査します。

- 妊娠のリスクが高い 15 ~ 18 歳の女性で、過去 6 か月以内の性行為、または将来の性行為の可能性があると定義されています。
- 来年中に妊娠を希望しない女性。
- 資格のある個人は、英語で話すことと読むことに堪能でなければなりません。
- 青年の同意。
除外基準:
- 現在、ホルモン避妊薬または子宮内避妊器具を使用している女性。
- 妊娠中の女性。
- -患者は参加を制限する発達遅滞を持っています。
- 患者は性的暴行を受けて救急外来に来ています。
- 患者はスクリーニングするには病気が多すぎる。

半構造化インタビューが実施され、根拠のある理論を使用して分析され、避妊開始に関する意思決定に影響を与える要因が解明されました。 登録された青年のサブセットのみが、任意のインタビューに参加しました (28 人の参加者)。 要因は、行動を説明するために態度、主観的規範、コミュニティ、ヘルスケア、および認識された行動制御要因に依存する、計画行動の生態学的に拡張された理論である eTPB と呼ばれる理論的枠組みに基づいていました。 インタビュー分析により、避妊の副作用と安全性に関する懸念、以前の避妊経験、避妊を使用する理由、仲間、家族、パートナーの影響、ソーシャル メディア、妊娠率、避妊教育へのアクセス、医療提供者との出会い、避妊への信頼などの要因が得られました。意思決定と知識、ED セッションの特徴。上記の要因はすべて、eTPB からのコンストラクトに対応していました。 |
