Deze studie is een prospectieve cohortstudie met vrouwen die zich tussen de 16 en 18 jaar oud melden op een van de twee spoedeisende hulpafdelingen (SEH), die het risico lopen op een onbedoelde zwangerschap.
Ons primaire doel is het beoordelen van de intentie om anticonceptie te starten ("hoog"=zeer/enigszins waarschijnlijk vs "laag"=onzeker/niet waarschijnlijk/zeker niet op Likert-schaalonderzoek) bij vrouwen van 15 tot 18 jaar die op de afdeling spoedeisende hulp anticonceptie krijgen advisering.
Onze secundaire doelstellingen zijn als volgt:
- Beoordeel bij dezelfde populatie de voltooiing van een verwijzing voor anticonceptiezorg, gedefinieerd als aanwezigheid op een verwijzingssite binnen 4 weken na het bezoek aan de index-ED.
- Beoordeel het percentage dat uiteindelijk met anticonceptie begint door middel van elektronische medische dossierdocumentatie (d.w.z. bezoeknotitie, procedurenota, medicatiebeoordeling) en deelnemersrapport (vervolggesprekken).
- Gebruik kwalitatieve interviewmethodologie om attitudes, belemmeringen en facilitators te onderzoeken die van invloed zijn op beslissingen om A) de intentie te uiten om anticonceptie te starten, B) een verwijzing voor anticonceptiezorg te voltooien en C) uiteindelijk anticonceptie te starten bij deze unieke groep adolescenten met een hoog risico.

Beschrijving
Inclusiecriteria:
- Vrouwen van 15-18 jaar met een hoog risico op zwangerschap, gedefinieerd als seksuele activiteit in de afgelopen 6 maanden of waarschijnlijke toekomstige seksuele activiteit.
- Vrouwtjes die het komende jaar niet zwanger willen worden.
- In aanmerking komende personen moeten bekwaam zijn in het spreken en lezen van het Engels.
- Toestemming van de adolescent.
Uitsluitingscriteria:
- Vrouwen die momenteel hormonale anticonceptie of een spiraaltje gebruiken.
- Vrouwtjes die zwanger zijn.
- Patiënt heeft een ontwikkelingsachterstand die deelname beperkt.
- Patiënt meldt zich op de SEH na aanranding.
- Patiënt is te ziek om gescreend te worden.

Er werden semigestructureerde interviews gehouden en geanalyseerd met behulp van gefundeerde theorie om de factoren op te helderen die van invloed zijn op de besluitvorming rond het starten met anticonceptie. Slechts een subgroep van ingeschreven adolescenten nam deel aan interviews die optioneel waren (28 deelnemers). Factoren waren gebaseerd op een theoretisch raamwerk genaamd eTPB, de ecologisch uitgebreide theorie van gepland gedrag, waarbij wordt vertrouwd op attitudes, subjectieve normen, gemeenschap, gezondheidszorg en waargenomen gedragscontrolefactoren om gedrag te verklaren. Interviewanalyse leverde de volgende factoren op: zorgen over bijwerkingen en veiligheid van anticonceptie, eerdere ervaringen met anticonceptie, redenen voor het gebruik van anticonceptie, invloed van leeftijdsgenoten, familie en partners, sociale media, zwangerschapspercentages en toegang tot anticonceptie-educatie, ontmoetingen met zorgverleners, vertrouwen in anticonceptie besluitvorming en kennis, en de ED-sessiekenmerken; alle bovenstaande factoren kwamen overeen met een construct van eTPB. |
