このプロジェクトでは、FSHR 遺伝子の N680S 多型遺伝子型と、制御された卵巣刺激に使用される FSH の性質との間に関係があるかどうかを判断したいと考えています。
これは非介入的、観察的、横断的、レトロスペクティブな全国的および多施設研究であり、遺伝子検査を実施して、2サイクルの制御された治療を受けた患者の血液サンプル中のN680S多型の遺伝子型を決定します。過去8か月間の卵巣刺激。

研究者らは、FSRH 遺伝子の N680S 多型が、制御された卵巣刺激による IVF/ICSI の 2 つの連続したサイクルを受ける患者で使用される FSH の異なる性質 (ヒト FSH および組換え FSH) で卵巣応答に影響を与える可能性があるかどうかを調査したいと考えています。
研究への参加に同意するすべての患者は、各病院の標準的な臨床診療に従って、制御された卵巣刺激の2回目のサイクルで実行される定期的な抽出と同じ日に血液サンプルを採取します。 サンプルは中央研究所に送られ、前述の多型の遺伝子型が決定され、2 サイクルの制御された卵巣刺激で得られた臨床結果と関連付けられます。
偏りを避けるために、研究集団は、COS の 1 回目と 2 回目のサイクルで使用される FSH のタイプに関してクロスオーバー デザインに従います (つまり、募集された患者の半分は、組換え体を使用した COS の 1 回目のサイクルを受けている必要があります)。 FSH およびヒト FSH での 2 番目のサイクル; 逆に、残りの半分は、ヒト FSH での 1 番目のサイクルと組換え FSH での 2 番目のサイクルを受けたにちがいない)。 包含基準が変更される可能性を回避するために、2 つのサイクル間の経過時間は 6 か月を超えてはなりません。

- 年齢 ≤ 37 歳
- BMI < 30kg/m2
- 胞状卵胞数の第 1 サイクル COS = 5-15
- 抗ミュラー管ホルモン基礎 1 サイクル目 COS > 1.1 ng/ml かつ < 3.1 ng/ml
- COS プロトコル: COS の 2 サイクルでの開始用量は等しく (150-225 UI/日)、COS の 5 日目から用量調整を行います。直径14mm以上の卵胞が1個以上またはE2が600pg/ml以下の場合に発症するGnRHアンタゴニストを用いたCOSの2サイクルにおける下垂体抑制プロトコル; hCGrまたはGnRHアゴニストによるトリガー
- 任意経口避妊薬(OC)の使用(但し、2サイクルで同じ)
- COSの第1サイクルで4~9個の卵母細胞が回収された
- COS の 2 サイクル間の ≤ 6 か月
除外基準:
- COSサイクル中のLH活性の使用
- 重度の男性因子の存在
- グレードIII~IVの子宮内膜症
- 多嚢胞性卵巣症候群、子宮または卵巣の手術歴、超音波で卵管水腫が確認できる患者、または子宮筋腫が 30 mm を超える患者
- 下垂体、甲状腺、副腎、膵臓、肝臓、または腎臓に影響を与える主要な全身性または制御されていない内分泌代謝疾患
