이 프로젝트는 FSHR 유전자의 N680S 다형성 유전자형과 제어된 난소 자극에 사용되는 FSH의 특성 사이에 관계가 있는지 여부를 확인하고자 합니다.
비간섭적, 관찰적, 횡단면적, 후향적, 국가 및 다기관 연구로, 2주기의 통제된 임상시험을 거친 환자의 혈액 샘플에서 N680S 다형성의 유전자형을 결정하기 위해 유전자 검사가 수행됩니다. 지난 8개월 동안 난소 자극.

상세 설명
연구자들은 FSRH 유전자의 N680S 다형성이 제어된 난소 자극과 함께 IVF/ICSI의 2주기를 연속적으로 겪는 환자에서 사용된 FSH의 다른 성질(인간 FSH 및 재조합 FSH)로 난소 반응에 영향을 미칠 수 있는지 여부를 조사하기를 원합니다.
연구 참여에 동의한 모든 환자는 각 병원의 표준 임상 관행에 따라 통제된 난소 자극의 2주기에 대해 일상적인 적출과 같은 날 혈액 샘플을 채취하게 됩니다. 샘플은 중앙 실험실로 보내져 앞서 언급한 다형성의 유전자형을 결정하고 이를 제어된 난소 자극의 2주기로 얻은 임상 결과와 연관시킬 것입니다.
편견을 피하기 위해 연구 모집단은 COS의 첫 번째 및 두 번째 주기에 사용되는 FSH 유형과 관련하여 교차 설계를 따를 것입니다(즉, 모집된 환자의 절반은 재조합 FSH와 인간 FSH로 두 번째 주기, 그리고 반대로 나머지 절반은 인간 FSH로 첫 번째 주기와 재조합 FSH로 두 번째 주기를 거쳐야 합니다. 포함 기준에 대한 잠재적인 수정을 방지하기 위해 두 주기 사이의 경과 시간은 6개월을 초과하지 않아야 합니다.

- Antral follicles count 1st cycle COS = 5-15
- 항뮬러관 호르몬 기초 1주기 COS > 1,1 ng/ml 및 < 3,1 ng/ml
- COS 프로토콜: COS 2주기에서 동일한 시작 용량(150-225 UI/일), COS 5일째부터 용량 조절; 직경 ≥ 14mm의 난포 1개 이상 또는 E2 ≤ 600pg/ml일 때 발병하는 GnRH 길항제를 사용한 COS의 2주기에서 뇌하수체 억제 프로토콜; 및 hCGr 또는 GnRH 작용제로 유발
- 선택적 경구 피임약(OC) 사용(단, 2주기에서는 동일)
- COS 1주기에서 4-9개의 회수된 난모세포
- COS의 2주기 사이에 ≤ 6개월
제외 기준:
- COS 주기 동안 LH 활동 사용
- 심한 남성 요인의 존재
- 등급 III-IV 자궁내막증
- 다낭성 난소 증후군 환자, 자궁 또는 난소 수술 병력, 초음파로 볼 수 있는 난관수종 또는 30mm 초과 자궁 근종
- 뇌하수체, 갑상선, 부신, 췌장, 간 또는 신장에 영향을 미치는 주요 전신성 또는 조절되지 않는 내분비 대사 질환
