Studie van het N680S-polymorfisme van het FSHR-gen en zijn relatie met het type gonadotropine dat wordt gebruikt bij gecontroleerde ovariële stimulatie
Dit project wil nagaan of er een verband bestaat tussen het N680S-polymorfismegenotype van het FSHR-gen en de aard van het FSH dat wordt gebruikt bij gecontroleerde ovariële stimulatie.
Het is een niet-interventionele, observationele, cross-sectionele en retrospectieve, nationale en multicenter studie, waarin een genetische test zal worden uitgevoerd om het genotype van het N680S-polymorfisme te bepalen in bloedmonsters van patiënten die twee cycli van gecontroleerde ovariële stimulatie in de laatste 8 maanden.

De onderzoekers willen onderzoeken of het N680S-polymorfisme van het FSRH-gen de ovariële respons kan beïnvloeden met een verschillende aard van het gebruikte FSH (humaan FSH en recombinant FSH) bij patiënten die twee opeenvolgende cycli van IVF/ICSI ondergaan met gecontroleerde ovariële stimulatie.
Bij alle patiënten die ermee instemmen om aan het onderzoek deel te nemen, zal een bloedmonster worden afgenomen op dezelfde dag als een routine-extractie die wordt uitgevoerd voor de 2e cyclus van de gecontroleerde ovariële stimulatie in overeenstemming met de standaard klinische praktijk van elk ziekenhuis. Het monster zal naar het centrale laboratorium worden gestuurd om het genotype van het bovengenoemde polymorfisme te bepalen en te correleren met de klinische resultaten verkregen met de twee cycli van gecontroleerde ovariële stimulatie.
Om enige vertekening te voorkomen, volgt de onderzoekspopulatie een cross-overontwerp met betrekking tot het type FSH dat wordt gebruikt in de eerste en tweede cyclus van de COS (d.w.z. de helft van de gerekruteerde patiënten moet een eerste cyclus van COS met recombinant FSH en een tweede cyclus met humaan FSH; en omgekeerd moet de andere helft een eerste cyclus met humaan FSH en een tweede cyclus met recombinant FSH hebben ondergaan). Om mogelijke wijzigingen van de opnamecriteria te voorkomen, mag de tijd tussen de twee cycli niet langer zijn dan 6 maanden.

Beschrijving
Inclusiecriteria:
- Handtekening van geïnformeerde toestemming
- Leeftijd ≤ 37 jaar
- BMI < 30 kg/m2
- Antrale follikels tellen 1e cyclus COS = 5-15
- Antimüller hormoon basaal 1e cyclus COS > 1,1 ng/ml en < 3,1 ng/ml
- COS-protocol: gelijke startdosis in de 2 cycli van COS (150-225 IE/dag) met dosisaanpassing vanaf de 5e dag van COS; hypofyse-onderdrukkingsprotocol in de 2 cycli van COS met GnRH-antagonist met aanvang wanneer ≥ 1 follikel met een diameter van ≥ 14 mm of E2 ≤ 600 pg/ml; en triggeren met hCGr of GnRH-agonist
- Optioneel het gebruik van orale anticonceptiva (OC) (echter hetzelfde in de 2 cycli)
- 4-9 herstelde eicellen in de 1e cyclus van COS
- ≤ 6 maanden tussen de 2 cycli van COS
Uitsluitingscriteria:
- Gebruik van de LH-activiteit tijdens de cycli van COS
- Aanwezigheid van ernstige mannelijke factor
- Graad III-IV endometriose
- Patiënten met polycysteus ovariumsyndroom, een voorgeschiedenis van baarmoeder- of eierstokoperaties, hydrosalpinx zichtbaar door echografie of baarmoederfibromen van > 30 mm
- Ernstige systemische of ongecontroleerde endocriene-metabolische ziekten die de hypofyse, de schildklier, de bijnieren, de pancreas, de lever of de nieren aantasten
