パーソナライズドオンコゲノミクス(POG)または同等のプログラムの一環として全ゲノムおよびトランスクリプトーム解析(WGTA)を受けた不治の固形腫瘍の参加者。
a.参加者は、腫瘍の配列決定が成功し、POG ゲノム解析者による正式なレビューを受け、CAPTIV-8 因子が同定されていることが判明し(免疫、負担、変異(IBV)スコア ≥ 5 を含む)、分子腫瘍委員会でのレビューを受けている必要があります( MTB) に割り当てられ、最終意見が文書化された状態で特定の腫瘍定義コホート (登録受付中) に割り当てられます。
- 東部協力腫瘍学グループ (ECOG) のパフォーマンス ステータス 0 ~ 2。

除外基準:
- プログラムされたデス1/リガンド(PD-1/PD-L1)軸を標的とするモノクローナル抗体による以前の治療(抗体薬物複合体およびその他の実験薬を含む)。
- -登録前の14日または薬物の5半減期のいずれか長い方以内の、承認された薬剤または治験薬による治療、または治療目的を持った別の臨床試験への参加。
治療または未治療の中枢神経系(CNS)病変を有する無症候性の参加者は、以下の基準をすべて満たす場合に適格です。
- RECIST v1.1 に従って、測定可能な疾患は CNS の外側に存在する必要があります。
- 参加者には頭蓋内出血や脊髄出血の病歴はありません。

登録前の過去 2 年以内のいずれかの時点での活動性自己免疫疾患。以下を含みますが、これらに限定されません。
- 重症筋無力症、筋炎、自己免疫性肝炎、全身性エリテマトーデス、関節リウマチ、炎症性腸疾患、抗リン脂質症候群に伴う血管血栓症、ウェゲナー肉芽腫症、シェーグレン症候群、ギラン・バレー症候群、多発性硬化症、血管炎、糸球体腎炎。
- 安定用量の甲状腺補充ホルモンを投与されている自己免疫関連甲状腺機能低下症の病歴のある参加者は、この研究の参加資格がある可能性があります。
- 安定した用量のインスリン療法を受けており、コントロールされているI型糖尿病の参加者は、この研究の参加資格があります。
湿疹、乾癬、慢性単純苔癬、または皮膚症状のみを伴う白斑のある参加者(たとえば、乾癬性関節炎のある参加者は除外されます)は、以下の条件を満たす場合に限り許可されます。
- 発疹は体表面積 (BSA) の 10% (10%) 未満をカバーする必要があります。
- 疾患はベースラインでは十分に制御されており、低効力の局所ステロイドのみを必要とします。
- 過去 12 か月以内に、ソラレンと紫外線照射 (PUVA)、メトトレキサート、レチノイド、生物学的製剤、経口カルシニューリン阻害剤または高効力経口ステロイドによる治療を必要とする基礎疾患の急性増悪がないこと。
- -特発性肺線維症、器質化肺炎(閉塞性細気管支炎など)、薬剤性肺炎、特発性肺炎の病歴、またはスクリーニング胸部CTスキャンでの活動性肺炎の証拠。 放射線分野における放射線肺炎(線維症)の既往は認められます。
- 以前の HIV 検査陽性(過去に HIV の病歴または症状がある参加者は、血清学的検査が陰性の場合のみ参加資格があります)。
- 既知の活動性B型肝炎ウイルス(HBV、慢性または急性、スクリーニング時にB型肝炎表面抗原[HBsAg]検査が陽性と定義される)またはC型肝炎に罹患している参加者。
- 活動性結核。
- サイクル 1、1 日目の前 2 週間以内の重度の感染症。感染症の合併症、菌血症、または重度の肺炎による入院が含まれますが、これらに限定されません。
- ニューヨーク心臓協会の心臓病(クラスII以上)、過去3か月以内の心筋梗塞、不安定な不整脈、不安定狭心症などの重篤な心血管疾患。
- -サイクル1、1日目の前21日以内に大規模な外科的処置を受けた、または研究期間中に大規模な外科的処置が必要になると予想される。
- 同種幹細胞または固形臓器の移植歴がある。
- -サイクル1、1日目の前の6週間または薬物の5半減期のいずれか短い方以内の全身免疫刺激剤(インターフェロン、インターロイキン-2(IL-2)を含むがこれらに限定されない)による治療。
- -サイクル1、1日目前の2週間以内に全身コルチコステロイドまたは他の全身免疫抑制薬(プレドニゾン、デキサメタゾン、シクロホスファミド、アザチオプリン、メトトレキサート、サリドマイド、抗腫瘍壊死因子[抗TNF]薬を含むがこれらに限定されない)による治療、または治験中に全身免疫抑制剤の必要性が予想される場合。
- -治験治療開始前4週間以内の弱毒生ワクチンによる治療、またはアテゾリズマブ治療中またはアテゾリズマブの最終投与後5か月以内にそのようなワクチンの必要性が予測される。
- 治療する臨床医がこのプロトコールを続行するのは不適当であると判断した参加者。
