Undersøgelse af virkningerne af atezolizumab hos mennesker, hvis tumor-DNA eller RNA indikerer mulig følsomhed (CAPTIV-8)
Canadisk Atezolizumab Precision Targeting for immunterapiintervention
Denne undersøgelse vil undersøge virkningerne af atezolizumab på udvalgte cancertyper hos mennesker, hvis analyse af tumor-DNA og RNA indikerer, at de kan være følsomme over for atezolizumab. Denne undersøgelse har til formål at afgøre, om informationen fra cancergenomanalysen svarer til virkningerne af atezolizumab på individer og deres cancer.
Dette er et fase 2-studie, som udføres efter at den foreløbige sikkerhedstest på et lægemiddel er afsluttet, og vil involvere cirka 200 deltagere. Deltagerne tildeles en af 8 kohorter baseret på deres primære tumortype: bryst-, lunge-, gastrointestinal (GI), primær ukendt, genitourinær (GU), sarkom, gynækologiske og 'andre' cancertyper. Deltagere i alle kohorter vil modtage den samme dosis atezolizumab (1200 mg hver 3. uge). I den første fase for hver kohorte vil 8 deltagere blive tilmeldt, og hvis ingen deltagere reagerer på behandlingen, vil tilmeldingen til den kohorte blive lukket. Hvis 1 eller flere deltagere reagerer på behandlingen, vil op til 16 yderligere deltagere blive tilmeldt denne kohorte. Deltagerne fortsætter i behandlingen, indtil de ikke længere har gavn af behandlingen, eller de beslutter sig for at stoppe behandlingen.

Inklusionskriterier:
- Alder over eller lig med 18 år på tidspunktet for underskrivelsen af informeret samtykke.
Deltagere med en uhelbredelig solid tumor, som har gennemgået hele genom- og transkriptomanalyse (WGTA) som en del af Personalized OncoGenomics (POG) eller tilsvarende program.
en. Deltagerne skal have haft succesfuld sekventering af deres tumor, været formelt gennemgået af POG-genomanalytikerne og fundet at have identificeret CAPTIV-8-faktorer (inklusive immun, byrde, variant (IBV) score ≥ 5), blevet gennemgået på Molecular Tumor Board ( MTB), og allokeres til en specifik tumordefineret kohorte (der er åben for tilmelding) med en endelig udtalelse dokumenteret.
- Eastern Cooperative Oncology Group (ECOG) præstationsstatus 0-2.
- Deltagerne skal have målbar sygdom, som defineret af RECIST 1.1.
- Forventet levetid på mindst 12 uger.
Tilstrækkelig hæmatologisk funktion og slutorganfunktion, som defineret af følgende laboratorieresultater opnået inden for 28 dage før den første undersøgelsesbehandling:
- Absolut neutrofiltal (ANC) ≥ 1500 celler/µL uden granulocytkolonistimulerende faktorstøtte.
- Antal hvide blodlegemer (WBC) > 2500/µL.
- Lymfocyttal ≥ 500/µL.
- Serumalbumin ≥ 2,5 g/dL.
- Blodpladeantal ≥ 100.000/µL uden transfusion (uden transfusion inden for 2 uger efter laboratorietest brugt til at bestemme egnethed).
- Hæmoglobin ≥ 9,0 g/dL, deltagere kan blive transfunderet eller modtage erytropoietisk behandling for at opfylde dette kriterium.
- International normaliseret ratio (INR) eller aktiveret partiel tromboplastintid (aPTT) ≤ 1,5 × øvre normalgrænse (ULN). Dette gælder kun for deltagere, der ikke får terapeutisk antikoagulering; deltagere, der modtager terapeutisk antikoagulering, skal have en INR eller aPTT inden for terapeutiske grænser i mindst 1 uge før tilmelding.
- Aspartataminotransferase (AST), alaninaminotransferase (ALT) og alkalisk fosfatase (ALP) ≤ 2,5 × ULN med følgende undtagelser: i) Deltagere med dokumenterede levermetastaser: ASAT og/eller ALT ≤ 5 × ULN. ii) Deltagere med dokumenterede lever- eller knoglemetastaser: ALP ≤ 5 × ULN.
- Serumbilirubin ≤ 1,5 × ULN. Deltagere med kendt Gilberts syndrom, som har serumbilirubinniveau ≤ 3 × ULN, kan tilmeldes.
- Serumkreatinin ≤ 1,5 × ULN.
- For kvinder i den fødedygtige alder: aftale om at forblive afholdende (afstå fra heteroseksuelt samleje) eller bruge præventionsmetoder, der resulterer i en fejlrate på mindre end 1 % (én procent) om året i behandlingsperioden og i mindst 5 måneder efter den sidste dosis atezolizumab.
- For mænd: aftale om at forblive afholdende (afstå fra heteroseksuelt samleje med en kvindelig partner i den fødedygtige alder, eller som er gravid) eller brug af præventionsmidler, der resulterer i en fejlrate på mindre end 1 % (én procent) om året, og aftale om at afstå fra fra donation af sæd, i behandlingsperioden og i mindst 5 måneder efter den sidste dosis atezolizumab.
- Evne til at give informeret samtykke til undersøgelsesprocedurerne defineret i denne protokol.
Ekskluderingskriterier:
- Enhver tidligere behandling med monoklonale antistoffer rettet mod programmeret død 1/Ligand (PD-1/PD-L1) aksen, inklusive antistof-lægemiddelkonjugater og andre eksperimentelle midler.
- Behandling med ethvert godkendt eller forsøgsmiddel eller deltagelse i et andet klinisk forsøg med terapeutisk hensigt inden for 14 dage eller fem halveringstider af lægemidlet, alt efter hvad der er længst, før tilmelding.
Asymptomatiske deltagere med behandlede eller ubehandlede læsioner i centralnervesystemet (CNS) er kvalificerede, forudsat at alle følgende kriterier er opfyldt:
- Målbar sygdom, pr. RECIST v1.1, skal være til stede uden for CNS.
- Deltageren har ingen historie med intrakraniel blødning eller rygmarvsblødning.
- Deltageren har ikke gennemgået stereotaktisk strålebehandling inden for 7 dage før påbegyndelse af undersøgelsesbehandling, helhjernestrålebehandling inden for 14 dage før påbegyndelse af undersøgelsesbehandling eller neurokirurgisk resektion inden for 28 dage før påbegyndelse af undersøgelsesbehandling.
- Deltagerne har ikke løbende behov for kortikosteroider som behandling for CNS-sygdom. Antikonvulsiv behandling i en stabil dosis er tilladt.
- Graviditet eller amning.
- Anamnese med alvorlige allergiske, anafylaktiske eller andre overfølsomhedsreaktioner over for kimære eller humaniserede antistoffer eller fusionsproteiner.
- Kendt overfølsomhed over for biofarmaceutiske præparater produceret i ovarieceller fra kinesisk hamster eller enhver komponent i atezolizumab-formuleringen.
Aktiv autoimmun sygdom på et hvilket som helst tidspunkt inden for de sidste 2 år forud for tilmelding, herunder men ikke begrænset til:
- Myasthenia gravis, myositis, autoimmun hepatitis, systemisk lupus erythematosus, rheumatoid arthritis, inflammatorisk tarmsygdom, vaskulær trombose associeret med antiphospholipid syndrom, Wegeners granulomatose, Sjögrens syndrom, Guillain-Barré syndrom, vaskulært scleritis, glomeritis, vaskulær glomeritis eller glomeritis.
- Deltagere med en historie med autoimmun-relateret hypothyroidisme på en stabil dosis af thyreoidea-erstatningshormon kan være berettiget til denne undersøgelse.
- Deltagere med kontrolleret type I diabetes mellitus på en stabil dosis insulin er kvalificerede til denne undersøgelse.
Deltagere med eksem, psoriasis, lichen simplex chronicus eller vitiligo med kun dermatologiske manifestationer (f.eks. vil deltagere med psoriasisgigt blive udelukket) er tilladt, forudsat at de opfylder følgende betingelser:
- Udslæt skal dække mindre end 10 % (ti procent) af kropsoverfladearealet (BSA).
- Sygdommen er godt kontrolleret ved baseline og kræver kun topiske steroider med lav styrke.
- Ingen akutte forværringer af den underliggende tilstand inden for de sidste 12 måneder, der kræver behandling med enten psoralen plus ultraviolet stråling (PUVA), methotrexat, retinoider, biologiske midler, orale calcineurinhæmmere eller højpotens eller orale steroider.
- Anamnese med idiopatisk lungefibrose, organiserende lungebetændelse (f.eks. bronchiolitis obliterans), lægemiddelinduceret lungebetændelse, idiopatisk lungebetændelse eller tegn på aktiv pneumonitis ved screening af CT-scanning af brystet. Anamnese med strålingspneumonitis i strålefeltet (fibrose) er tilladt.
- Tidligere positiv test for HIV (deltagere med en tidligere historie med/eller symptomer på HIV er kun berettiget, hvis serologiske test er negative).
- Deltagere med kendt aktiv hepatitis B-virus (HBV; kronisk eller akut; defineret som havende en positiv hepatitis B overfladeantigen [HBsAg] test ved screening) eller hepatitis C.
- Aktiv tuberkulose.
- Alvorlige infektioner inden for 2 uger før cyklus 1, dag 1, inklusive, men ikke begrænset til, hospitalsindlæggelse for komplikationer af infektion, bakteriæmi eller svær lungebetændelse.
- Betydelig kardiovaskulær sygdom, såsom New York Heart Association hjertesygdom (klasse II eller større), myokardieinfarkt inden for de foregående 3 måneder, ustabile arytmier eller ustabil angina.
- Større kirurgisk indgreb inden for 21 dage før cyklus 1, dag 1 eller forventning om behov for et større kirurgisk indgreb i løbet af undersøgelsen.
- Tidligere allogen stamcelle- eller fast organtransplantation.
- Behandling med systemiske immunstimulerende midler (herunder, men ikke begrænset til, interferoner, interleukin-2 (IL-2)) inden for 6 uger eller fem halveringstider af lægemidlet, alt efter hvad der er kortest, før cyklus 1, dag 1.
- Behandling med systemiske kortikosteroider eller andre systemiske immunsuppressive lægemidler (herunder, men ikke begrænset til, prednison, dexamethason, cyclophosphamid, azathioprin, methotrexat, thalidomid og antitumornekrosefaktor [anti-TNF]-midler) inden for 2 uger før dag 1 cyklus 1. , eller forventet behov for systemisk immunsuppressiv medicin under forsøget.
- Behandling med en levende, svækket vaccine inden for 4 uger før påbegyndelse af undersøgelsesbehandling eller forventning om behov for en sådan vaccine under behandling med atezolizumab eller inden for 5 måneder efter den sidste dosis atezolizumab.
- Deltagere, som den behandlende kliniker ellers mener er uegnede til at fortsætte med denne protokol.
