Atetsolitsumabin vaikutusten tutkiminen ihmisillä, joiden kasvaimen DNA tai RNA viittaa mahdolliseen herkkyyteen (CAPTIV-8)
Kanadalainen atetsolitsumabin tarkkuuskohdistus immunoterapian interventioon
Tässä tutkimuksessa tutkitaan atetsolitsumabin vaikutuksia tiettyihin syöpätyyppeihin ihmisillä, joiden kasvain-DNA- ja RNA-analyysi osoittaa, että he voivat olla herkkiä atetsolitsumabille. Tämän tutkimuksen tavoitteena on selvittää, vastaavatko syövän genomianalyysin tiedot atetsolitsumabin vaikutuksia yksilöihin ja heidän syöpäänsä.
Tämä on vaiheen 2 tutkimus, joka tehdään lääkkeen alustavan turvallisuustestauksen jälkeen ja johon osallistuu noin 200 osallistujaa. Osallistujat jaetaan johonkin kahdeksasta kohortista ensisijaisen kasvaimen tyypin perusteella: rinta-, keuhko-, maha-suolikanava (GI), ensisijainen tuntematon, urogenitaalinen (GU), sarkooma, gynekologinen ja "muut" syöpätyypit. Kaikkien kohorttien osallistujat saavat saman annoksen atetsolitsumabia (1200 mg joka kolmas viikko). Ensimmäisessä vaiheessa jokaisesta kohortista otetaan mukaan 8 osallistujaa, ja jos yksikään osallistuja ei reagoi hoitoon, ilmoittautuminen kyseiseen kohorttiin suljetaan. Jos yksi tai useampi osallistuja reagoi hoitoon, enintään 16 osallistujaa otetaan mukaan kyseiseen kohorttiin. Kuvaus
Sisällyttämiskriteerit:
- Ikä on vähintään 18 vuotta tietoisen suostumuksen allekirjoitushetkellä.
Osallistujat, joilla on parantumaton kiinteä kasvain, joille on tehty koko genomi- ja transkriptianalyysi (WGTA) osana Personalised OncoGenomics (POG) tai vastaavaa ohjelmaa.
a. Osallistujilla on täytynyt olla onnistunut kasvaimen sekvensointi, POG-genomianalyytikot ovat tarkastaneet heidät virallisesti ja heillä on täytynyt tunnistaa CAPTIV-8-tekijät (mukaan lukien Immune, Burden, Variant (IBV) pisteet ≥ 5), jotka on tarkistettu Molecular Tumor Boardissa ( MTB) ja osoitetaan tietylle kasvaimen määrittämälle kohortille (joka on avoin ilmoittautumista varten), ja lopullinen lausunto on dokumentoitu.
- Eastern Cooperative Oncology Groupin (ECOG) suorituskykytila 0-2.
- Osallistujilla on oltava mitattavissa oleva sairaus RECIST 1.1:n määrittelemällä tavalla.
- Elinajanodote vähintään 12 viikkoa.
Riittävä hematologinen ja pääteelinten toiminta, joka määritellään seuraavilla laboratoriotuloksilla, jotka on saatu 28 päivän aikana ennen ensimmäistä tutkimushoitoa:

- Seerumin albumiini ≥ 2,5 g/dl.
- Verihiutalemäärä ≥ 100 000/µL ilman verensiirtoa (ilman verensiirtoa 2 viikon kuluessa kelpoisuuden määrittämiseen käytetystä laboratoriokokeesta).
- Hemoglobiini ≥ 9,0 g/dl, osallistujille voidaan antaa verensiirto tai he voivat saada erytropoieettista hoitoa tämän kriteerin täyttämiseksi.
- Kansainvälinen normalisoitu suhde (INR) tai aktivoitu osittainen tromboplastiiniaika (aPTT) ≤ 1,5 × normaalin yläraja (ULN). - Seerumin bilirubiini ≤ 1,5 × ULN. Osallistujat, joilla on tunnettu Gilbertin oireyhtymä ja joiden seerumin bilirubiinitaso on ≤ 3 × ULN, voidaan ottaa mukaan.

Poissulkemiskriteerit:
- Mikä tahansa aikaisempi hoito monoklonaalisilla vasta-aineilla, jotka kohdistuvat ohjelmoidun kuoleman 1/ligandin (PD-1/PD-L1) -akseliin, mukaan lukien vasta-aine-lääkekonjugaatit ja muut kokeelliset aineet.
- Hoito millä tahansa hyväksytyllä tai tutkittavalla aineella tai osallistuminen toiseen kliiniseen tutkimukseen terapeuttisessa tarkoituksessa 14 päivän tai viiden lääkkeen puoliintumisajan kuluessa, riippuen siitä kumpi on pidempi, ennen ilmoittautumista.
Oireettomat osallistujat, joilla on hoidettuja tai hoitamattomia keskushermostovaurioita, ovat kelpoisia, jos kaikki seuraavat kriteerit täyttyvät:
- Mitattavissa olevan taudin RECIST v1.1:n mukaan on oltava keskushermoston ulkopuolella.
- Osallistujalla ei ole aiemmin ollut kallonsisäistä verenvuotoa tai selkäytimen verenvuotoa.
- Osallistuja ei ole saanut stereotaktista sädehoitoa 7 päivän aikana ennen tutkimushoidon aloittamista, kokoaivojen sädehoitoa 14 päivän kuluessa ennen tutkimushoidon aloittamista tai neurokirurgista resektiota 28 päivän kuluessa ennen tutkimushoidon aloittamista.
- Osallistujilla ei ole jatkuvaa tarvetta käyttää kortikosteroideja keskushermoston sairauden hoitona. Antikonvulsanttihoito vakaalla annoksella on sallittu.
- Raskaus tai imetys.
- Aiemmat vakavat allergiset, anafylaktiset tai muut yliherkkyysreaktiot kimeerisille tai humanisoiduille vasta-aineille tai fuusioproteiineille.
- Tunnettu yliherkkyys kiinanhamsterin munasarjasoluissa tuotetuille biofarmaseuttisille aineille tai jollekin atetsolitsumabivalmisteen aineosalle.
Aktiivinen autoimmuunisairaus missä tahansa vaiheessa viimeisten 2 vuoden aikana ennen ilmoittautumista, mukaan lukien, mutta ei rajoittuen:
- Myasthenia gravis, myosiitti, autoimmuunihepatiitti, systeeminen lupus erythematosus, nivelreuma, tulehduksellinen suolistosairaus, antifosfolipidioireyhtymään liittyvä verisuonitukos, Wegenerin granulomatoosi, Sjögrenin oireyhtymä, Guillain-Barrén oireyhtymä, multippeliskleroosi, vaskuliitti tai glomerulontulehdus.
- Osallistujat, joilla on ollut autoimmuuniperäinen kilpirauhasen vajaatoiminta ja jotka ovat saaneet vakaan annoksen kilpirauhasen korvaushormonia, voivat olla kelvollisia tähän tutkimukseen.
- Osallistujat, joilla on hallinnassa oleva tyypin I diabetes, jotka saavat vakaata insuliiniannosta, ovat kelvollisia tähän tutkimukseen.
Osallistujat, joilla on ekseema, psoriaasi, lichen simplex chronicus tai vitiligo, joilla on vain dermatologisia oireita (esim. osallistujat, joilla on psoriaattinen niveltulehdus, suljetaan pois), ovat sallittuja, jos he täyttävät seuraavat ehdot:
- Ihottuma tulee peittää alle 10 % (kymmenen prosenttia) kehon pinta-alasta (BSA).
- Sairaus on lähtötilanteessa hyvin hallinnassa ja vaatii vain vähätehoisia paikallisia steroideja.
- Ei akuutteja perussairauden pahenemisvaiheita viimeisten 12 kuukauden aikana, jotka vaatisivat hoitoa joko psoraleenilla ja ultraviolettisäteilyllä (PUVA), metotreksaatilla, retinoideilla, biologisilla aineilla, suun kautta otetuilla kalsineuriinin estäjillä tai voimakkailla tai oraalisilla steroideilla.
- Aiempi idiopaattinen keuhkofibroosi, järjestäytynyt keuhkokuume (esim. obliterans bronchiolitis), lääkkeiden aiheuttama keuhkotulehdus, idiopaattinen keuhkotulehdus tai näyttö aktiivisesta keuhkotulehduksesta seulonnassa rintakehän CT-skannauksessa. Säteilykentällä esiintynyt keuhkotulehdus (fibroosi) on sallittu.
- Aiempi positiivinen HIV-testi (osallistujat, joilla on aiemmin ollut HIV-oireita, ovat kelpoisia vain, jos serologiset testit ovat negatiivisia).
- Osallistujat, joilla tiedetään olevan aktiivinen hepatiitti B -virus (HBV; krooninen tai akuutti; määritelty hepatiitti B:n pinta-antigeenin [HBsAg]-testin positiiviseksi seulonnassa) tai hepatiitti C:ksi.
- Aktiivinen tuberkuloosi.
- Vakavat infektiot 2 viikon sisällä ennen sykliä 1, päivää 1, mukaan lukien mutta ei rajoittuen sairaalahoitoon infektion, bakteremian tai vaikean keuhkokuumeen vuoksi.
- Merkittävät sydän- ja verisuonisairaudet, kuten New York Heart Associationin sydänsairaus (luokka II tai suurempi), sydäninfarkti viimeisen 3 kuukauden aikana, epästabiilit rytmihäiriöt tai epästabiili angina pectoris.
- Suuri kirurginen toimenpide 21 päivän sisällä ennen sykliä 1, päivää 1 tai suuren kirurgisen toimenpiteen tarve tutkimuksen aikana.
- Aikaisempi allogeeninen kantasolu- tai kiinteä elinsiirto.
- Hoito systeemisillä immunostimuloivilla aineilla (mukaan lukien, mutta niihin rajoittumatta interferonit, interleukiini-2 (IL-2)) 6 viikon tai viiden lääkkeen puoliintumisajan kuluessa, sen mukaan, kumpi on lyhyempi, ennen sykliä 1, päivää 1.
- Hoito systeemisillä kortikosteroideilla tai muilla systeemisillä immunosuppressiivisilla lääkkeillä (mukaan lukien, mutta niihin rajoittumatta prednisoni, deksametasoni, syklofosfamidi, atsatiopriini, metotreksaatti, talidomidi ja kasvainnekroositekijä [anti-TNF] -aineet) 2 viikon sisällä ennen sykliä 1, päivää tai odotettu tarve saada systeemisiä immunosuppressiivisia lääkkeitä tutkimuksen aikana.
- Hoito elävällä, heikennetyllä rokotteella 4 viikon sisällä ennen tutkimushoidon aloittamista tai tällaisen rokotteen tarpeen ennakoiminen atetsolitsumabihoidon aikana tai 5 kuukauden sisällä viimeisen atetsolitsumabiannoksen jälkeen.
- Osallistujat, jotka hoitava kliinikko muuten katsovat olevansa sopimattomia jatkamaan tätä protokollaa.
