이 연구는 종양 DNA 및 RNA 분석 결과 그들이 아테졸리주맙에 민감할 수 있음을 나타내는 사람들을 대상으로 특정 암 유형에 대한 아테졸리주맙의 효과를 조사할 것입니다. 이 연구는 암 게놈 분석의 정보가 개인 및 그들의 암에 대한 아테졸리주맙의 효과와 일치하는지 확인하는 것을 목표로 합니다.
이는 약물에 대한 예비 안전성 시험이 완료된 후 진행되는 2상 연구로 약 200명의 참가자가 참여하게 된다. 참가자는 유방암, 폐암, 위장관(GI), 미지의 원발암, 비뇨생식기(GU), 육종, 부인과 및 '기타' 암 유형과 같은 기본 종양 유형에 따라 8개의 코호트 중 하나에 할당됩니다. 모든 코호트의 참가자는 동일한 용량의 아테졸리주맙(3주마다 1200mg)을 받게 됩니다. 각 코호트의 첫 번째 단계에서는 8명의 참가자가 등록되며 참가자가 치료에 반응하지 않으면 해당 코호트에 대한 등록이 종료됩니다. 1명 이상의 참가자가 치료에 반응하는 경우 최대 16명의 추가 참가자가 해당 코호트에 등록됩니다. 참여자는 더 이상 치료의 혜택을 받지 못하거나 치료를 중단하기로 결정할 때까지 치료를 계속합니다.

POG(Personalized OncoGenomics) 또는 이에 상응하는 프로그램의 일환으로 전체 게놈 및 전사체 분석(WGTA)을 받은 치료 불가능한 고형 종양이 있는 참가자.
ㅏ. 참가자는 성공적인 종양 시퀀싱을 수행하고 POG 게놈 분석가에 의해 공식적으로 검토되었으며 CAPTIV-8 요인이 식별된 것으로 확인되었으며(Immune, Burden, Variant(IBV) 점수 ≥ 5 포함) Molecular Tumor Board에서 검토되었습니다. MTB), 문서화된 최종 의견과 함께 특정 종양 정의 코호트(등록 가능)에 할당됩니다.
- Eastern Cooperative Oncology Group(ECOG) 수행 상태 0-2.
- 참가자는 RECIST 1.1에 정의된 측정 가능한 질병이 있어야 합니다.

첫 번째 연구 치료 전 28일 이내에 얻은 다음 실험실 결과에 의해 정의된 적절한 혈액학적 및 말단 장기 기능:
- 과립구 콜로니 자극 인자 지원 없이 절대 호중구 수(ANC) ≥ 1500 cells/µL.
- 백혈구(WBC) 수 > 2500/µL.
- 림프구 수 ≥ 500/µL.
- 혈청 알부민 ≥ 2.5g/dL.

제외 기준:
- 항체-약물 접합체 및 기타 실험 제제를 포함하여 프로그램화된 사망 1/리간드(PD-1/PD-L1) 축을 표적으로 하는 단클론 항체를 사용한 모든 이전 치료.
- 등록 전 14일 또는 약물의 반감기 5일 중 더 긴 기간 내에 승인된 또는 시험용 제제로 치료하거나 치료 의도가 있는 다른 임상 시험에 참여.
치료 또는 미치료 중추신경계(CNS) 병변이 있는 무증상 참가자는 다음 기준을 모두 충족하는 경우 자격이 있습니다.
- RECIST v1.1에 따라 측정 가능한 질병은 CNS 외부에 존재해야 합니다.
- 참가자는 두개내 출혈 또는 척수 출혈의 병력이 없습니다.
- 참가자는 연구 치료 시작 전 7일 이내에 정위 방사선 요법, 연구 치료 시작 전 14일 이내에 전뇌 방사선 요법 또는 연구 치료 시작 전 28일 이내에 신경외과 절제술을 받지 않았습니다.
- 참가자는 CNS 질병에 대한 요법으로 코르티코스테로이드에 대한 지속적인 요구 사항이 없습니다. 안정적인 용량의 항경련제 치료가 허용됩니다.
- 임신 또는 모유 수유.
- 키메라 또는 인간화 항체 또는 융합 단백질에 대한 중증 알레르기, 아나필락시스 또는 기타 과민 반응의 병력.
- 차이니즈 햄스터 난소 세포 또는 아테졸리주맙 제제의 구성 성분에서 생산된 생물약제에 대해 알려진 과민증.
다음을 포함하되 이에 국한되지 않는 등록 전 지난 2년 이내의 임의 시점의 활동성 자가면역 질환:
- 중증 근무력증, 근염, 자가면역 간염, 전신성 홍반성 루푸스, 류마티스성 관절염, 염증성 장 질환, 항인지질 증후군과 관련된 혈관 혈전증, 베게너 육아종증, 쇼그렌 증후군, 길랭-바레 증후군, 다발성 경화증, 혈관염 또는 사구체신염.
- 안정적인 용량의 갑상선 대체 호르몬에 대한 자가면역 관련 갑상선 기능 저하증 병력이 있는 참가자는 이 연구에 참여할 수 있습니다.
- 안정적인 투여량의 인슐린 요법으로 통제된 제1형 진성 당뇨병을 가진 참가자가 이 연구에 적합합니다.
습진, 건선, 만성 단순 태선 또는 피부 증상만 있는 백반증이 있는 참가자(예: 건선성 관절염 참가자는 제외됨)는 다음 조건을 충족하는 경우 허용됩니다.
- 발진은 체표면적(BSA)의 10% 미만이어야 합니다.
- 질병은 기준선에서 잘 통제되고 낮은 효능의 국소 스테로이드만 필요로 합니다.
- 지난 12개월 동안 소랄렌 + 자외선(PUVA), 메토트렉세이트, 레티노이드, 생물학적 제제, 경구용 칼시뉴린 억제제 또는 고효능 또는 경구용 스테로이드로 치료가 필요한 근본적인 상태의 급성 악화가 없었습니다.

- New York Heart Association 심장병(Class II 이상), 이전 3개월 이내의 심근경색, 불안정 부정맥 또는 불안정 협심증과 같은 중대한 심혈관 질환.
- 주기 1, 1일 전 21일 이내의 주요 수술 또는 연구 과정 동안 주요 수술이 필요할 것으로 예상되는 경우.
- 이전 동종 줄기 세포 또는 고형 장기 이식.
- 주기 1, 제1일 전에 6주 또는 약물의 5반감기 중 더 짧은 기간 내에 전신 면역자극제(인터페론, 인터류킨-2(IL-2)를 포함하되 이에 국한되지 않음)로 치료.
- 주기 1, 1일 전 2주 이내에 전신 코르티코스테로이드 또는 기타 전신 면역억제제(프레드니손, 덱사메타손, 시클로포스파미드, 아자티오프린, 메토트렉세이트, 탈리도마이드 및 항종양 괴사 인자[항-TNF] 제제를 포함하되 이에 국한되지 않음)로 치료 , 또는 시험 기간 동안 전신성 면역억제제에 대한 예상 요구 사항.
- 연구 치료 시작 전 4주 이내 또는 아테졸리주맙 치료 중 또는 아테졸리주맙 최종 투여 후 5개월 이내에 그러한 백신이 필요할 것으로 예상되는 약독화 생백신으로 치료.
- 치료하는 임상의가 이 프로토콜을 진행하기에 부적합하다고 느끼는 참가자.
