過去 20 年間の収束データは、大量の歩行練習を提供することで脳卒中後の個人の歩行機能を改善できることを示唆しており、中程度から高い有酸素強度で歩行トレーニングを行うと、より大きな改善が見られます。 それにもかかわらず、入院患者のリハビリテーション中の現在の理学療法の実践は、限られた歩行練習が提供され、低心血管強度が達成されることを示唆しています。
この現在の品質改善プロジェクトの目的は、定期的な入院患者の理学療法中に集中的、集中的、反復的なステッピング トレーニング (FIRST) を実施することの実現可能性と、患者の転帰測定に対する潜在的な影響を評価することです。
プロジェクトの通常のケア段階の前に、アウトカム測定チームは、特定のアウトカム測定値 (10 メートルの歩行テスト、6 分間の歩行テスト、バーグ バランス スケール) の定期的な収集を定期的な間隔 (入院、毎週、退院) で標準化する作業を行いました。第三者の支払者が義務付けた措置 (例: Functional Independence Measure; FIM)。 その後、既存の理学療法の実践と結果を約 9 ~ 12 か月間監視します。 その後、これらの患者の管理に関連する理学療法士に教育トレーニングが提供され、継続的なサポート (例: 教訓、精神運動、技術) および監査/フィードバック (例: カルテ監査、歩数フィードバック) が提供され、忠実度指標が監視されます。チャートの監査 (ウォーキングの練習、ウォーキングの優先順位付け、記録された心血管強度、達成された目標強度) および歩数計の歩数から。

除外基準:
- 下肢の体重負荷の制限 (切断や下肢骨折など)
