研究の目的は、脳卒中後の膝が固い歩行をする患者に対するBoNT-Aと地上外骨格ロボットの組み合わせの効果を評価することである。
膝硬直歩行(SKG)は、脳卒中後の患者によく見られる歩行パターンで、歩行の遊脚期での膝屈曲(KF)の制限を特徴とします。 大腿直筋へのボツリヌス毒素 A (BoNT-A) 注射は、SKG を治療するためのゴールドスタンダード手順と考えられています。
この歩行パターンを持つ患者は、歩行速度が低下し、足の指を引きずり、歩行の安定性を損ない、転倒の危険性が高まり、日常生活に支障をきたします。 このランダム化比較試験は、脳卒中後の痙性硬直膝歩行患者に対する効率的かつ効果的な治療プログラムの改良と開発の加速に貢献します。 ロボット支援歩行訓練 (RAGT) は、BoNT-A 治療後の最適な補助療法となる可能性があります。 BoNT-A とリハビリテーション トレーニングの組み合わせは、痙縮関連障害の治療効果を最適化するために提案されています。

- 虚血性脳卒中または出血性脳卒中 3 か月以上
- 年齢 20 歳以上
- 機能的歩行カテゴリー ≥4
- 影響を受けた大腿直筋の痙縮(MAS 1+ と 2 の間)
- BoNT-A治療歴がない、または患者登録前に罹患脚にBoNT-A治療を4か月以上行っている
- 経口筋弛緩剤または痙縮に対するその他の薬剤の投与を 2 か月以上安定した用量で受けている
- 簡単な命令に従うことができる
除外基準:
- 妊娠中
- BoNT-A に対する感受性
- 注射部位の皮膚、軟組織の感染症
- 他の治験への参加
- 影響を受けた脚の拘縮または骨変形を修正
- -神経溶解療法または外科的処置による患肢の以前の治療(すなわち、フェノールブロック、腱延長の移動、腱切り術、筋肉の解放、関節固定術)

ボトックスブランドのBoNT-A精製神経毒複合体(アラガン・ファーマシューティカルズ社、カリフォルニア州アーバイン)は、凍結乾燥毒素を0.9%生理食塩水で33〜100U/mlの濃度に希釈することによって調製されます。
対象となる筋肉のサイズによって異なります。
エコーガイドを用いて対象となる筋肉の位置を確認します。
総用量範囲は 100 単位です。
各対象筋肉の線量範囲は以下の通りです:大腿直筋で100単位。
本研究では、ウェアラブル地上歩行用外骨格下肢ロボットシステムを使用します。
ロボット下肢システムは、左右の膝を補助するための両側モーター、デバイスを吊り下げるための骨盤ベルトとキャリッジシステム、および外骨格「リンク」をユーザーに取り付けるための大腿部と下腿部カフで構成されています。
患者は、影響を受けた大腿直筋にBoNT-Aを注射した後、外骨格下肢ロボットを装着し、次の機能歩行訓練を開始します:1) 地上歩行、2) 旋回歩行、3) 椅子の出入り、4) しゃがみ込みと立ち上がり、5) 階段の上り下り。
