本研究は、サルコペニアを有する介護施設入居者における頸部固有受容性感覚運動トレーニングが、頸部固有受容性感覚、運動恐怖症、および疼痛破局的思考に及ぼす影響を調査しました。
この無作為化比較試験では、EWGSOP2基準に基づき63名の介護施設入居者をサルコペニアのスクリーニング評価しました。評価項目は、握力(ジャマー握力計)、身体機能(4メートル歩行テスト)、および生体電気インピーダンス法による筋肉量評価を含みます。 サルコペニアと診断された31名の入居者は、対照群(n = 17)と実験群(n = 14)に無作為に割り付けられました。対照群は6週間にわたり12回の監督付き従来型運動(姿勢、頸部可動域、等尺性運動)を実施し、実験群はこれに加えて12回の頸部固有受容性感覚運動を追加で受けました。 両群とも介入前後に評価が行われました。 頸部関節位置覚、運動恐怖症、疼痛破局的思考は、それぞれ関節位置誤差テスト、タンパ運動恐怖症尺度、疼痛破局的思考尺度を用いて評価されました。

- サルコペニアと診断されていること、
- バーセル日常生活動作指数で91点以上(軽度依存または完全自立)であること、
- ミニメンタルステート検査で24点以上(精神的に健康と判断される)であること。
除外基準;
- 障害(四肢、視覚、聴覚障害など)があること、
- 固有感覚に影響を与える可能性のある重篤な神経学的、整形外科的、またはリウマチ学的疾患があること、
- 管理不良の高血圧/糖尿病があること、
- 脊椎および/または四肢に痛みがあること、
- 精神疾患の診断があること。

頸部固有感覚の評価

|
頸部固有受容感覚は関節位置誤差テストを用いて評価されました。
座位で、参加者は頭部の高さに90 cm離れた位置に設置されたボード(直径40 cmの同心円、1 cm間隔)に向かい、レーザーポインターが取り付けられたキャップを着用しました。参加者はまずレーザーを中心点に合わせて中立位置を確立し、その後、目を開けた状態で屈曲、伸展、右/左回旋、右/左側屈をガイドされ、各運動後に中立位置に戻りました。
同じ6つの運動をその後、目を閉じて能動的に実行しました。
各方向について6回の反復が完了し、毎試行後にレーザーが停止した点がボード上に記録されました。
これらの点の平均が計算され、ボードまでの固定距離に基づくアークタン法を用いて目標からの角度偏差が決定されました。
4度を超える偏差は固有受容感覚の障害を示すと見なされました。

運動恐怖症は、Kinesiophobia Causes Scale(KCS)を用いて評価されました。
KocjanとKnapik(2014)によって開発されたこの尺度は、運動への恐怖の生物学的および心理学的要因を特定し、運動活動の低下の根本的な原因を理解するのに貢献します。
この尺度は、生物学的領域と心理学的領域の2つの領域で構成されています。
生物学的領域には、形態的要因、運動要件、エネルギー資源、生物学的欲求、否定的な経験、運動能力、および身体ケアに関する信念が含まれ、一方、心理学的領域は、回避行動に寄与する自信、自己効力感、社会的相互作用などの要因を評価します。
トルコ語での妥当性と信頼性の研究は、Çayırら(2020)によって実施されました。
この尺度は、5段階のリッカート尺度(1=強く不同意、5=強く同意)で評価される20項目で構成されています。
高いスコアは、より強い運動への恐怖を示します。

疼痛破局的思考は、Pain Catastrophizing Scale (PCS) を使用して評価されました。これは、疼痛に関連する不適応な思考や感情を評価する13項目の尺度です。
この尺度には、無力感、誇大化、反芻の3つの下位次元が含まれています。
各項目は0-4の尺度で採点され、対応する項目を合計して下位尺度スコアを算出します。総合PCSスコアはすべての項目の合計として計算されます。
トルコ語での妥当性と信頼性の研究はUğurluら(2017)によって実施されました。
総合スコアは0から52の範囲で、より高いスコアはより大きな疼痛破局的思考を示します。30を超えるスコアは臨床的に有意なレベルの破局的思考を反映します。
