HET EFFECT VAN PROPRIOCEPTIEVE OEFENTRAINING OP CERVICALE PROPRIOCEPTIE, KINESIOFOBIE EN PIJNCATASTROFERINGSLEVELS BIJ SARCOPENISCHE GERIATRISCHE INDIVIDUEN DIE IN VERPLEEGTEHUIZEN WONEN
Deze studie onderzocht de effecten van cervicale proprioceptieve oefentraining op cervicale proprioceptie, kinesiofobie en pijncatastroferen bij sarcopene verpleeghuisbewoners.
In deze gerandomiseerde gecontroleerde studie werden 63 verpleeghuisbewoners gescreend op sarcopenie op basis van de EWGSOP2-criteria, waaronder beoordelingen van handknijpkracht (Jamar-dynamometer), fysieke prestaties (4-meter looptest) en spiermassa geëvalueerd door bio-elektrische impedantieanalyse. Eenendertig bewoners bij wie sarcopenie was vastgesteld, werden willekeurig toegewezen aan een controlegroep (n = 17), die zes weken lang 12 begeleide sessies van conventionele oefeningen (houdings-, cervicale bewegingsbereik- en isometrische oefeningen) voltooide, of een experimentele groep (n = 14), die 12 extra sessies van cervicale proprioceptieve oefeningen kreeg. Beide groepen werden voor en na de interventie geëvalueerd. Cervicale gewrichtspositiezin, kinesiofobie en pijncatastroferen werden beoordeeld met behulp van de Joint Position Error Test; de Tampa Scale of Kinesiophobia; de Pain Catastrophizing Scale.

Beschrijving
Inclusiecriteria;
- 65 jaar of ouder zijn,
- Gediagnosticeerd zijn met sarcopenie,
- Minimaal 91 punten of meer hebben volgens de Barthel-index voor activiteiten in het dagelijks leven (licht afhankelijk of volledig onafhankelijk),
- 24 punten of meer hebben volgens de mini-mental-statusonderzoek, wat als mentaal gezond wordt beschouwd.
Exclusiecriteria;
- Een handicap hebben (zoals ledemaat, gezichts-, gehoorverlies),
- Een ernstige neurologische, orthopedische of reumatologische aandoening hebben die de proprioceptie kan beïnvloeden,
- Ongecontroleerde hypertensie/diabetes hebben,
- Pijn in de wervelkolom en/of ledematen hebben,
- Een psychiatrische ziekte diagnose hebben.

Beoordeling van cervicale proprioceptie

Cervicale proprioceptie werd beoordeeld met behulp van de joint position error test.
Terwijl de deelnemer zat, keek deze naar een bord op 90 cm afstand op hoofdhoogte, bestaande uit concentrische cirkels (40 cm diameter, 1-cm tussenruimtes), en droeg een pet met een gemonteerde laserpointer. De deelnemer richtte eerst de laser op het centrale punt om de neutrale positie vast te stellen, en werd vervolgens - met open ogen - begeleid door flexie, extensie, rechts/links rotatie en rechts/links laterale flexie, waarbij na elke beweging teruggekeerd werd naar neutraal.
Dezelfde zes bewegingen werden daarna actief uitgevoerd met gesloten ogen.
Er werden zes herhalingen voltooid voor elke richting, en het punt waar de laser stopte werd na elke proef op het bord gemarkeerd.
Het gemiddelde van deze punten werd berekend, en de hoekafwijking van het doel werd bepaald met behulp van de arctan-methode op basis van de vaste afstand tot het bord.
Een afwijking van meer dan 4 graden werd beschouwd als een indicatie van verminderde proprioceptie.

Beoordeling van kinesiofobie

Kinesiofobie werd beoordeeld met behulp van de Kinesiofobie Oorzaken Schaal (KOS).
Ontwikkeld door Kocjan en Knapik (2014), identificeert de schaal de biologische en psychologische bronnen van bewegingsangst en draagt bij aan het begrijpen van de onderliggende oorzaken van verminderde motorische activiteit.
De schaal bestaat uit twee domeinen: biologisch en psychologisch.
Het biologische domein omvat morfologische factoren, bewegingsvereisten, energiebronnen, biologische drijfveren, negatieve ervaringen, motorische competentie en overtuigingen met betrekking tot lichaamszorg, terwijl het psychologische domein factoren beoordeelt zoals zelfvertrouwen, zelfeffectiviteit en sociale interactie die bijdragen aan vermijdingsgedrag.
De Turkse validiteits- en betrouwbaarheidsstudie werd uitgevoerd door Çayır et al. (2020).
De schaal bestaat uit 20 items beoordeeld op een 5-punts Likertschaal (1 = sterk oneens, 5 = sterk eens).
Hogere scores duiden op een grotere bewegingsangst.

Pijncatastroferen werd beoordeeld met behulp van de Pijn Catastroferen Schaal (PCS), een 13-items meting die maladaptieve gedachten en gevoelens met betrekking tot pijn evalueert.
De schaal omvat drie subdimensies: hulpeloosheid, vergroting en ruminatie.
Elk item wordt gescoord op een schaal van 0-4, en subscores worden verkregen door de corresponderende items op te tellen; de totale PCS-score wordt berekend als de som van alle items.
De Turkse validiteits- en betrouwbaarheidsstudie werd uitgevoerd door Uğurlu et al. (2017).
Totale scores variëren van 0 tot 52, waarbij hogere scores wijzen op meer pijncatastroferen. Scores boven de 30 weerspiegelen klinisch significante niveaus van catastroferen.
