O EFEITO DO TREINO DE EXERCÍCIOS PROPIOCEPTIVOS NA PROPRIOCEPÇÃO CERVICAL, CINESIOFOBIA E NÍVEIS DE CATASTROFIZAÇÃO DA DOR EM INDIVÍDUOS GERIÁTRICOS COM SARCOPENIA RESIDENTES EM LARES DE IDOSOS
O EFEITO DO TREINO DE EXERCÍCIO PROPIOCEPTIVO NA PROPRIOCEPÇÃO CERVICAL, CINESIOFOBIA E NÍVEIS DE CATÁSTROFIZAÇÃO DA DOR EM INDIVÍDUOS GERIÁTRICOS COM SARCÓPENIA RESIDENTES EM LA RES DE IDOSOS
Este estudo investigou os efeitos do treino de exercícios proprioceptivos cervicais na propriocepção cervical, cinesiofobia e catastrofização da dor em residentes sarcopénicos de lares de idosos.
Neste estudo randomizado controlado, 63 residentes de lares de idosos foram rastreados para sarcopenia com base nos critérios EWGSOP2, que incluem avaliações da força de preensão manual (dinamómetro Jamar), desempenho físico (teste de marcha de 4 metros) e massa muscular avaliada por Análise de Impedância Bioelétrica. Trinta e um residentes diagnosticados com sarcopenia foram aleatoriamente atribuídos a um grupo de controlo (n = 17), que completou 12 sessões supervisionadas de exercícios convencionais (posturais, amplitude de movimento cervical e exercícios isométricos) ao longo de seis semanas, ou a um grupo experimental (n = 14), que recebeu 12 sessões adicionais de exercícios proprioceptivos cervicais. Ambos os grupos foram avaliados antes e depois da intervenção. O sentido de posição articular cervical, a cinesiofobia e a catastrofização da dor foram avaliados usando o Teste de Erro de Posição Articular; a Escala de Cinesiofobia de Tampa; a Escala de Catastrofização da Dor.

Descrição
Critérios de inclusão;
- Ter 65 anos de idade ou mais,
- Ser diagnosticado com sarcopenia,
- Ter pelo menos 91 pontos ou mais de acordo com o índice de atividades da vida diária de Barthel (ser ligeiramente dependente ou completamente independente),
- Ter 24 pontos ou mais de acordo com a avaliação mental mínima, o que é considerado mentalmente saudável.
Critérios de exclusão;
- Ter uma deficiência (como membro, visão, perda auditiva),
- Ter um distúrbio neurológico, ortopédico ou reumatológico grave que possa afetar a propriocepção,
- Ter hipertensão/diabetes não controlada,
- Ter dor na coluna vertebral e/ou extremidades,
- Ter um diagnóstico de doença psiquiátrica.

Avaliação da propriocepção cervical

A proprioceção cervical foi avaliada através do teste de erro de posição articular.
Em posição sentada, o participante estava de frente para um painel posicionado a 90 cm de distância ao nível da cabeça, constituído por círculos concêntricos (40 cm de diâmetro, incrementos de 1 cm), e usava um boné com um ponteiro laser montado. O participante alinhou primeiro o laser com o ponto central para estabelecer a posição neutra, depois - com os olhos abertos - foi guiado através da flexão, extensão, rotação direita/esquerda e flexão lateral direita/esquerda, regressando à posição neutra após cada movimento.
Os mesmos seis movimentos foram depois realizados ativamente com os olhos fechados.
Foram completadas seis repetições para cada direção, e o ponto em que o laser parou foi marcado no painel após cada ensaio.
A média destes pontos foi calculada, e o desvio angular do alvo foi determinado usando o método arctan com base na distância fixa ao painel.
Um desvio superior a 4 graus foi considerado indicativo de proprioceção comprometida.

Avaliação da cinesiofobia

A cinesiofobia foi avaliada utilizando a Escala de Causas da Cinesiofobia (KCS).
Desenvolvida por Kocjan e Knapik (2014), a escala identifica as fontes biológicas e psicológicas do medo do movimento e contribui para a compreensão das causas subjacentes da redução da atividade motora.
Compreende dois domínios: biológico e psicológico.
O domínio biológico inclui fatores morfológicos, requisitos de movimento, recursos energéticos, impulsos biológicos, experiências negativas, competência motora e crenças relacionadas com o cuidado do corpo, enquanto o domínio psicológico avalia fatores como a confiança, a autoeficácia e a interação social que contribuem para o comportamento de evitação.
O estudo de validação e fiabilidade turco foi realizado por Çayır et al. (2020).
A escala consiste em 20 itens avaliados numa escala de Likert de 5 pontos (1 = discordo totalmente, 5 = concordo totalmente).
Pontuações mais elevadas indicam um maior medo do movimento.

Avaliação da catastrofização da dor

A catastrofização da dor foi avaliada utilizando a Escala de Catastrofização da Dor (PCS), uma medida de 13 itens que avalia pensamentos e sentimentos desadaptativos relacionados com a dor.
A escala inclui três subdimensões: sentimento de impotência, magnificação e ruminação.
Cada item é pontuado numa escala de 0 a 4, e as pontuações das subescalas são obtidas somando os itens correspondentes; a pontuação total da PCS é calculada como a soma de todos os itens.
O estudo de validade e fiabilidade turco foi conduzido por Uğurlu et al. (2017).
As pontuações totais variam de 0 a 52, com pontuações mais elevadas indicando maior catastrofização da dor. Pontuações superiores a 30 refletem níveis clinicamente significativos de catastrofização.
