L'EFFET DE L'ENTRAÎNEMENT PHYSIQUE PROPRIOCEPTIF SUR LA PROPRIOCEPTION CERVICALE, LA KINÉSIOPHOBIE ET LES NIVEAUX DE CATASTROPHISME FACE À LA DOULEUR CHEZ LES PERSONNES GÉRIATRIQUES SARCOSIQUES RÉSIDANT EN ÉTABLISSEMENTS D'HÉBERGEMENT POUR PERSONNES ÂGÉES
L'EFFET DE L'ENTRAÎNEMENT À L'EXERCICE PROPRIOCEPTIF SUR LA PROPRIOCEPTION CERVICALE, LA KINÉSIOPHOBIE ET LES NIVEAUX DE CATASTROPHISME DE LA DOULEUR CHEZ LES PERSONNES GÉRIATRIQUES SARCOSPHIQUES RÉSIDANT EN MAISONS DE RETRAITE
Cette étude a examiné les effets de l'entraînement aux exercices proprioceptifs cervicaux sur la proprioception cervicale, la kinésiophobie et la catastrophisation de la douleur chez les résidents de maisons de retraite sarcopéniques.
Dans cette étude randomisée contrôlée, 63 résidents de maisons de retraite ont été dépistés pour la sarcopénie sur la base des critères EWGSOP2, qui incluent des évaluations de la force de préhension (dynamomètre Jamar), de la performance physique (test de marche de 4 mètres) et de la masse musculaire évaluée par analyse d'impédance bioélectrique. Trente et un résidents diagnostiqués avec une sarcopénie ont été assignés au hasard soit à un groupe témoin (n = 17), qui a terminé 12 séances supervisées d'exercices conventionnels (posturaux, amplitude de mouvement cervical et exercices isométriques) sur six semaines, soit à un groupe expérimental (n = 14), qui a reçu 12 séances supplémentaires d'exercices proprioceptifs cervicaux. Les deux groupes ont été évalués avant et après l'intervention. Le sens de la position articulaire cervicale, la kinésiophobie, la catastrophisation de la douleur ont été évalués à l'aide du test d'erreur de position articulaire ; de l'échelle de kinésiophobie de Tampa ; de l'échelle de catastrophisation de la douleur.

- Adulte plus âgé
Accepte les volontaires sains
La description
Critères d'inclusion ;
- Être âgé de 65 ans ou plus,
- Être diagnostiqué avec une sarcopénie,
- Avoir au moins 91 points ou plus selon l'indice d'activités de la vie quotidienne de Barthel (être légèrement dépendant ou complètement indépendant),
- Avoir 24 points ou plus selon l'évaluation cognitive minimale, ce qui est considéré comme mentalement sain.
Critères d'exclusion ;
- Avoir un handicap (tel qu'un membre, une perte de vision, une perte auditive),
- Avoir un trouble neurologique, orthopédique ou rhumatologique grave pouvant affecter la proprioception,
- Avoir une hypertension/diabète non contrôlé(e),
- Avoir des douleurs dans la colonne vertébrale et/ou les extrémités,
- Avoir un diagnostic de maladie psychiatrique.

Évaluation de la proprioception cervicale

La proprioception cervicale a été évaluée à l'aide du test d'erreur de position articulaire.
Assis, le participant faisait face à un tableau positionné à 90 cm de distance au niveau de la tête, composé de cercles concentriques (40 cm de diamètre, incréments de 1 cm), et portait une casquette avec un pointeur laser monté. Le participant a d'abord aligné le laser avec le point central pour établir la position neutre, puis - les yeux ouverts - a été guidé à travers la flexion, l'extension, la rotation droite/gauche et la flexion latérale droite/gauche, revenant à la position neutre après chaque mouvement.
Les mêmes six mouvements ont ensuite été effectués activement les yeux fermés.
Six répétitions ont été effectuées pour chaque direction, et le point où le laser s'arrêtait était marqué sur le tableau après chaque essai.
La moyenne de ces points a été calculée, et la déviation angulaire par rapport à la cible a été déterminée en utilisant la méthode de l'arctan basée sur la distance fixe au tableau.
Une déviation de plus de 4 degrés était considérée comme indiquant une proprioception altérée.

Évaluation de la kinésiophobie

La kinésiophobie a été évaluée à l'aide de l'Échelle des Causes de la Kinésiophobie (KCS).
Développée par Kocjan et Knapik (2014), l'échelle identifie les sources biologiques et psychologiques de la peur du mouvement et contribue à comprendre les causes sous-jacentes de la réduction de l'activité motrice.
Elle comprend deux domaines : biologique et psychologique.
Le domaine biologique inclut les facteurs morphologiques, les exigences de mouvement, les ressources énergétiques, les pulsions biologiques, les expériences négatives, la compétence motrice et les croyances liées aux soins du corps, tandis que le domaine psychologique évalue des facteurs tels que la confiance, l'auto-efficacité et l'interaction sociale qui contribuent au comportement d'évitement.
L'étude de validité et de fiabilité turque a été réalisée par Çayır et al. (2020).
L'échelle comprend 20 items évalués sur une échelle de Likert en 5 points (1 = pas du tout d'accord, 5 = tout à fait d'accord).
Des scores plus élevés indiquent une plus grande peur du mouvement.

Évaluation de la dramatisation de la douleur

La catastrophisation de la douleur a été évaluée à l'aide de l'Échelle de Catastrophisation de la Douleur (PCS), une mesure en 13 items évaluant les pensées et sentiments inadaptés liés à la douleur.
L'échelle comprend trois sous-dimensions : impuissance, amplification et rumination.
Chaque item est noté sur une échelle de 0 à 4, et les scores des sous-échelles sont obtenus en additionnant les items correspondants ; le score total de la PCS est calculé comme la somme de tous les items.
L'étude de validité et de fiabilité turque a été menée par Uğurlu et al. (2017).
Les scores totaux varient de 0 à 52, des scores plus élevés indiquant une plus grande catastrophisation de la douleur. Les scores supérieurs à 30 reflètent des niveaux cliniquement significatifs de catastrophisation.
