EL EFECTO DEL ENTRENAMIENTO CON EJERCICIOS PROPIOCEPTIVOS SOBRE LA PROPIOCEPCIÓN CERVICAL, LA CINESIOFOBIA Y LOS NIVELES DE CATASTROFISMO DEL DOLOR EN INDIVIDUOS GERIÁTRICOS SARCOPÉNICOS QUE RESIDEN EN RESIDENCIAS DE ANCIANOS
EL EFECTO DEL ENTRENAMIENTO CON EJERCICIOS PROPIOCEPTIVOS EN LA PROPRIOCEPCIÓN CERVICAL, LA CINESIOFOBIA Y LOS NIVELES DE CATASTROFISMO DEL DOLOR EN INDIVIDUOS GERIÁTRICOS SARCOPÉNICOS QUE RESIDEN EN RESIDENCIAS DE ANCIANOS
Este estudio investigó los efectos del entrenamiento con ejercicios propioceptivos cervicales sobre la propiocepción cervical, la kinesiofobia y la catastrofización del dolor en residentes de hogares de ancianos con sarcopenia.
En este estudio controlado aleatorizado, se evaluó a 63 residentes de hogares de ancianos para detectar sarcopenia según los criterios EWGSOP2, que incluyen evaluaciones de la fuerza de prensión manual (dinamómetro Jamar), el rendimiento físico (prueba de marcha de 4 metros) y la masa muscular evaluada mediante Análisis de Impedancia Bioeléctrica. Treinta y un residentes diagnosticados con sarcopenia fueron asignados aleatoriamente a un grupo de control (n = 17), que completó 12 sesiones supervisadas de ejercicios convencionales (posturales, de rango de movimiento cervical y ejercicios isométricos) durante seis semanas, o a un grupo experimental (n = 14), que recibió 12 sesiones adicionales de ejercicios propioceptivos cervicales. Ambos grupos fueron evaluados antes y después de la intervención. El sentido de posición articular cervical, la kinesiofobia y la catastrofización del dolor se evaluaron mediante la Prueba de Error de Posición Articular; la Escala de Kinesiofobia de Tampa; la Escala de Catastrofización del Dolor.

Descripción
Criterios de inclusión;
- Tener 65 años de edad o más,
- Estar diagnosticado de sarcopenia,
- Tener al menos 91 puntos o más según el índice de actividades de la vida diaria de Barthel (ser ligeramente dependiente o completamente independiente),
- Tener 24 puntos o más según el mini examen del estado mental, lo que se considera mentalmente sano.
Criterios de exclusión;
- Tener una discapacidad (como de extremidades, visión, pérdida auditiva),
- Tener un trastorno neurológico, ortopédico o reumatológico grave que pueda afectar la propiocepción,
- Tener hipertensión/diabetes no controlada,
- Tener dolor en la columna vertebral y/o extremidades,
- Tener un diagnóstico de enfermedad psiquiátrica.

Evaluación de la propiocepción cervical

|
La propiocepción cervical se evaluó mediante la prueba de error de posición articular.
Mientras estaba sentado, el participante miraba hacia un tablero colocado a 90 cm de distancia a la altura de la cabeza, que consistía en círculos concéntricos (40 cm de diámetro, incrementos de 1 cm), y llevaba una gorra con un puntero láser montado. El participante primero alineó el láser con el punto central para establecer la posición neutral, luego, con los ojos abiertos, fue guiado a través de flexión, extensión, rotación derecha/izquierda y flexión lateral derecha/izquierda, volviendo a la posición neutral después de cada movimiento.
Los mismos seis movimientos se realizaron luego activamente con los ojos cerrados.
Se completaron seis repeticiones para cada dirección, y el punto en el que se detuvo el láser se marcó en el tablero después de cada prueba.
Se calculó la media de estos puntos, y la desviación angular del objetivo se determinó utilizando el método arctan basado en la distancia fija al tablero.
Se consideró que una desviación de más de 4 grados indicaba una propiocepción alterada.

Evaluación de la kinesiofobia

La kinesiofobia se evaluó mediante la Escala de Causas de Kinesiofobia (KCS).
Desarrollada por Kocjan y Knapik (2014), la escala identifica las fuentes biológicas y psicológicas del miedo al movimiento y contribuye a comprender las causas subyacentes de la reducción de la actividad motora.
Comprende dos dominios: biológico y psicológico.
El dominio biológico incluye factores morfológicos, requisitos de movimiento, recursos energéticos, impulsos biológicos, experiencias negativas, competencia motora y creencias relacionadas con el cuidado del cuerpo, mientras que el dominio psicológico evalúa factores como la confianza, la autoeficacia y la interacción social que contribuyen al comportamiento de evitación.
El estudio de validez y fiabilidad turco fue realizado por Çayır et al. (2020).
La escala consta de 20 ítems calificados en una escala Likert de 5 puntos (1 = totalmente en desacuerdo, 5 = totalmente de acuerdo).
Las puntuaciones más altas indican un mayor miedo al movimiento.

Evaluación de la catastrofización del dolor

La catastrofización del dolor se evaluó mediante la Escala de Catastrofización del Dolor (PCS), una medida de 13 ítems que evalúa pensamientos y sentimientos desadaptativos relacionados con el dolor.
La escala incluye tres subdimensiones: impotencia, magnificación y rumiación.
Cada ítem se puntúa en una escala de 0 a 4, y las puntuaciones de las subescalas se obtienen sumando los ítems correspondientes; la puntuación total de la PCS se calcula como la suma de todos los ítems.
El estudio de validez y fiabilidad turco fue realizado por Uğurlu et al. (2017).
Las puntuaciones totales oscilan entre 0 y 52, y las puntuaciones más altas indican una mayor catastrofización del dolor. Las puntuaciones superiores a 30 reflejan niveles clínicamente significativos de catastrofización.
