DIE WIRKUNG VON PROPRIOZEPTIVEM ÜBUNGSTRAINING AUF DIE ZERVICALE PROPRIOZEPTION, KINESIOPHOBIE UND SCHMERZKATASTROPHISIERUNGSLEVEL BEI SARKOPENISCHEN GERIATRISCHEN PERSONEN IN PFLEGEHEIMEN
DIE AUSWIRKUNG VON PROPRIOZEPTIVEM BEWEGUNGSTRAINING AUF DIE ZERVICALE PROPRIOZEPTION, KINESIOPHOBIE UND SCHMERZKATASTROPHISIERUNG BEI SARKOPENISCHEN GERIATRISCHEN PERSONEN IN PFLEGEHEIMEN
Diese Studie untersuchte die Auswirkungen von zervikalem propriozeptivem Bewegungstraining auf die zervikale Propriozeption, Kinesiophobie und Schmerzkatastrophisierung bei sarkopenischen Bewohnern von Pflegeheimen.
In dieser randomisierten kontrollierten Studie wurden 63 Bewohner von Pflegeheimen auf Sarkopenie nach den EWGSOP2-Kriterien gescreent, welche Bewertungen der Handgriffstärke (Jamar-Dynamometer), der körperlichen Leistungsfähigkeit (4-Meter-Gehtest) und der Muskelmasse mittels Bioelektrischer Impedanzanalyse umfassen. Einunddreißig mit Sarkopenie diagnostizierte Bewohner wurden zufällig entweder einer Kontrollgruppe (n = 17) zugeordnet, die über sechs Wochen hinweg 12 betreute Sitzungen mit konventionellen Übungen (Haltung, zervikaler Bewegungsumfang und isometrische Übungen) absolvierte, oder einer experimentellen Gruppe (n = 14), die zusätzlich 12 Sitzungen mit zervikalen propriozeptiven Übungen erhielt. Beide Gruppen wurden vor und nach der Intervention bewertet. Der zervikale Gelenkpositionssinn, Kinesiophobie und Schmerzkatastrophisierung wurden mittels des Joint Position Error Tests, der Tampa Scale of Kinesiophobia und der Pain Catastrophizing Scale beurteilt.

Beschreibung
Einschlusskriterien;
- 65 Jahre oder älter sein,
- Diagnose von Sarkopenie,
- Mindestens 91 Punkte oder mehr nach dem Barthel-Index für Aktivitäten des täglichen Lebens (leicht abhängig oder völlig unabhängig),
- 24 Punkte oder mehr nach der Mini-Mental-Status-Untersuchung, was als geistig gesund gilt.
Ausschlusskriterien;
- Eine Behinderung haben (wie Gliedmaßen-, Seh-, Hörverlust),
- Eine schwere neurologische, orthopädische oder rheumatologische Erkrankung haben, die die Propriozeption beeinträchtigen könnte,
- Unkontrollierte Hypertonie/Diabetes haben,
- Schmerzen in der Wirbelsäule und/oder den Extremitäten haben,
- Eine psychiatrische Erkrankungsdiagnose haben.

Experimental: Versuchsgruppe
Zervikale propriozeptive Übungen und klassische Übungen wurden angewendet.

Die Teilnehmer in der Kontrollgruppe erhielten über 6 Wochen klassisches Training.
Die Teilnehmer in der experimentellen Gruppe erhielten zusätzlich zum klassischen Bewegungstraining 6 Wochen lang zervikale Propriozeptionsübungen.

Klassische Übungen (Bewegungsübungen der Halswirbelsäule + Haltungstraining + isometrische Übungen) wurden ausschließlich angewendet.

Beurteilung der zervikalen Propriozeption
Zeitfenster: 10 Monate

Die zervikale Propriozeption wurde mittels des Joint-Position-Error-Tests bewertet.
Im Sitzen blickte die Teilnehmerin auf eine 90 cm entfernt auf Kopfhöhe positionierte Tafel mit konzentrischen Kreisen (40 cm Durchmesser, 1-cm-Abstände) und trug eine Kappe mit montiertem Laserpointer. Die Teilnehmerin richtete zunächst den Laser auf den Mittelpunkt aus, um die Neutralposition festzulegen, dann – bei geöffneten Augen – wurde sie durch Flexion, Extension, Rechts-/Links-Rotation und Rechts-/Links-Lateralflexion geführt und kehrte nach jeder Bewegung zur Neutralposition zurück.
Die gleichen sechs Bewegungen wurden dann aktiv mit geschlossenen Augen ausgeführt.
Für jede Richtung wurden sechs Wiederholungen durchgeführt, und der Punkt, an dem der Laser stoppte, wurde nach jedem Versuch auf der Tafel markiert.
Der Mittelwert dieser Punkte wurde berechnet, und die Winkelabweichung vom Ziel wurde anhand der festen Entfernung zur Tafel mit der Arkustangens-Methode bestimmt.
Eine Abweichung von mehr als 4 Grad wurde als Hinweis auf eine beeinträchtigte Propriozeption gewertet.

Beurteilung der Kinesiophobie

Kinesiophobie wurde mit der Kinesiophobie-Ursachen-Skala (KUS) bewertet.
Entwickelt von Kocjan und Knapik (2014), identifiziert die Skala die biologischen und psychologischen Quellen der Bewegungsangst und trägt zum Verständnis der zugrunde liegenden Ursachen verminderter motorischer Aktivität bei.
Sie umfasst zwei Bereiche: biologisch und psychologisch.
Der biologische Bereich umfasst morphologische Faktoren, Bewegungsanforderungen, Energieressourcen, biologische Antriebe, negative Erfahrungen, motorische Kompetenz und Überzeugungen im Zusammenhang mit Körperpflege, während der psychologische Bereich Faktoren wie Vertrauen, Selbstwirksamkeit und soziale Interaktion bewertet, die zu Vermeidungsverhalten beitragen.
Die türkische Validitäts- und Reliabilitätsstudie wurde von Çayır et al. (2020) durchgeführt.
Die Skala besteht aus 20 Items, die auf einer 5-Punkte-Likert-Skala bewertet werden (1 = stimme überhaupt nicht zu, 5 = stimme voll und ganz zu).
Höhere Werte zeigen eine größere Bewegungsangst an.

Bewertung der Schmerzkatastrophisierung

Die Schmerzkatastrophisierung wurde mithilfe der Pain Catastrophizing Scale (PCS) bewertet, einem 13-Item-Instrument zur Erfassung maladaptiver Gedanken und Gefühle in Bezug auf Schmerzen.
Die Skala umfasst drei Subdimensionen: Hilflosigkeit, Dramatisierung und Grübeln.
Jedes Item wird auf einer Skala von 0-4 bewertet, und die Subskalenwerte werden durch Summierung der entsprechenden Items ermittelt; der Gesamt-PCS-Wert wird als Summe aller Items berechnet.
Die türkische Validitäts- und Reliabilitätsstudie wurde von Uğurlu et al. (2017) durchgeführt.
Die Gesamtwerte reichen von 0 bis 52, wobei höhere Werte auf eine stärkere Schmerzkatastrophisierung hinweisen. Werte über 30 deuten auf klinisch signifikante Katastrophisierungsniveaus hin.
