The screener consists of 16 items assessing frequency or quantity of consumption of key Mediterranean diet components including fruits, vegetables, whole grains, legumes, nuts, fish, olive oil, and red or processed meats.

Reported height will be taken from an average of the patient's three most recent electronic medical record heights.
