결절성 경화증 복합체(TSC)는 결절성 경화증 복합체 1(TSC1) 또는 TSC2 종양 억제 유전자의 돌연변이로 인해 발생하는 상염색체 우성 장애입니다. TSC는 광범위한 조직의 종양, 발작, 정신 지체, 자폐증 및 장기 부전을 특징으로 합니다. 림프관평활근종증(LAM)은 TSC가 있는 여성의 주요 폐 증상으로 비정형 평활근 유사 세포(TSC-/- LAM 세포)의 침윤 및 실질 낭종 형성을 특징으로 하는 진행성 폐 질환입니다. 산발성 LAM은 TSC2 유전자의 체세포 돌연변이로 인해 TSC 진단 기준을 충족하지 못하는 여성에서 발생할 수 있습니다.
이 연구의 장기 목표는 LAM 환자를 위한 새로운 치료 전략을 고안하는 것입니다. 침윤성 성장 패턴, 전이 가능성 및 변경된 세포 분화와 관련하여 LAM 세포의 관찰된 행동은 상피-중간엽 전이(EMT)를 겪는 세포를 연상시킵니다. Src 키나아제는 세포 증식, 운동성, 침습성 및 EMT의 주요 조절자입니다. 최근 결과에 따르면 autophagy는 활성 Src의 분해를 촉진합니다. 따라서 LAM 세포에서 발생하는 것으로 알려진 mTOR 활성화로 인한 자가포식 감소는 이러한 세포에서 활성 Src의 축적에 중요한 역할을 할 수 있습니다. - 여성 환자. LAM은 거의 전적으로 여성에게서 발생한다는 점에 유의해야 합니다.
- 18~65세.
- 모든 환자는 흉부 컴퓨터 단층촬영(CT)과 다음 중 하나에서 일치하는 낭성 변화로 정의된 LAM 진단을 받아야 합니다.
- LAM과 일치하는 열린 폐, 기관지 또는 흉부 바늘 생검
- LAM과 일치하는 개방 또는 바늘 복부 생검 소견
- 결절성 경화종 복합체(TSC), 신혈관지방종, 낭포성 복부 림프관종 또는 흉부 또는 복부의 유미 삼출 병력의 임상 소견
- 혈청 혈관 내피 성장 인자 D(VEGF-D) > 800pg/ml
- 피험자는 등록 전 마지막 24개월 동안 최소 6개월 간격으로 측정한 최소 2회의 폐 기능 검사(PFT)에서 나타난 바와 같이 최근 1초 강제 호기량(FEV1) > 50ml/년의 감소가 있어야 합니다. 공부하다.
