상세 설명
우리는 X-CGD 환자에서 충분한 수의 HSPC를 수집하기 위해 동원 시험을 설계했습니다. 이 절차는 잠재적으로 만성 염증 상태에 의해 유발된 기능적 결함으로 인해 이러한 환자들에게 도전적이라는 것은 잘 알려져 있습니다.
G-CSF와 Plerixafor의 조합은 유전자 치료 시험에서 HSPC 수확을 위한 최신 기술로 간주됩니다. 우리는 HSPC 동원을 증가시키고 HSPC 함량을 변경하는 역할을 할 수 있는 염증을 줄이기 위해 비스테로이드성 염증 약물을 추가하는 것을 고려했습니다.
이번 임상에서 조사 중인 3개 약물의 시너지 효과가 확인되면 향후 X-CGD 환자를 대상으로 한 유전자 치료 임상시험에서 HSPC 동원에 대해 이러한 요법이 고려될 예정이다.

- X-CGD의 유전적 진단
- 18-45세
- 카르노프스키 지수 > 80%
- 적절한 심장, 신장, 간 및 폐 기능.
- 음성 혈전성 선별 검사 및 이전 혈전 사건에 대한 음성 병력
- 서면 동의서
제외 기준:
- 이전 골수 이식 또는 이전 유전자 치료.
- 연구 등록 전 4주 이내(지속형 제제를 사용하는 경우 6주 이내)에 다른 연구용 제제 사용.
- 진행 중인 IFN-γ 치료(4주 이내).
- 증상이 있는 염증성 장 질환.
- 자격 부여 후 6주 이내에 증상이 있는 바이러스, 세균 또는 진균 감염
- 신생물(국소 피부암 제외) 또는 "가족" 암 병력
- 골수이형성증 또는 기타 심각한 혈액학적 장애
- 조절되지 않는 발작 및 심부 정맥 혈전증의 병력
- 기타 시술이 부적합하다고 판단되는 전신질환
- HIV 및/또는 HCV RNA 및/또는 HbsAg 및/또는 HBV DNA에 대한 양성
- 연구 6개월 이내에 활성 알코올 또는 약물 남용.
- IBU, G-CSF, Plerixafor 또는 Pantoprazole 투여에 대한 금기

실험적: XCGD 동원
Ibuprofen, Myelostim 및 Mozobil의 조합으로 치료
|
이부프로펜: 3mg/kg tid(총 일일 투여량: 9mg/kg); 1일부터 5일까지 그리고 14일부터 마지막 LP 전날까지 경구 투여.
Myelostim(G-CSF): 5μg/kg bid(총 일일 투여량 10μg/kg); 19일째부터 마지막 LP의 날까지 피하 투여한다.
모조빌(Plerixafor): 매일 0.24mg/kg.
CD34+가 ≥ 10/μL인 경우 이 약은 다음날(또는 CD34+가 < 10/μL인 경우 24일부터)부터 마지막 LP일까지 피하 투여됩니다.
