이 연구는 모의 급속 연속 유도(RSI)에서 베드 업 헤드 엘리베이션 BUHE 위치 대 스니핑 위치에서 기관내삽관(ETI)을 비교하는 무작위 통제 시험을 수행하는 것을 목표로 합니다.
목표는 머리를 위로 올린 자세에서 삽관에 걸리는 시간이 스니핑 자세에서 삽관에 걸리는 시간보다 열등하지 않은지 확인하는 것입니다.
가설:
- 전신 마취하에 모의 응급 수술에서 빠른 시퀀스 유도를 받는 환자에서 직접 후두경 검사(DL) 및 BUHE 위치의 ETI는 스니핑 위치에서 DL 및 성공적인 ETI에 필요한 시간보다 열등하지 않습니다.
- 전신 마취하에 모의 응급 수술에서 빠른 시퀀스 유도를 받는 환자에서 직접 후두경 검사(DL) 및 BUHE 위치의 ETI는 POGO 점수를 향상시킵니다.
- 전신 마취 하에 모의 응급 수술에서 빠른 시퀀스 유도를 받는 환자에서 직접 후두경 검사(DL) 및 BUHE 위치의 ETI는 기도 관련 합병증을 감소시킵니다.
술어:
직접 후두경 검사(DL) 및 기관내 삽관법(ETI): 환자가 전신 마취를 받은 후 기관(기관)에 호흡관을 삽입하는 방법입니다.
베드 업 헤드 엘리베이션(BUHE): 흉골 절흔이 있는 외이도 사이 정렬을 목표로 20-30도 베드 업 헤드 엘리베이션.
스니핑 자세: 머리 받침이 있는 머리를 들어 앙와위 자세를 유지합니다.
신속 순차 유도(RSI): 위 내용물이 폐로 흡인될 위험이 있는 환자에게 마취를 유도하는 확립된 방법입니다. 그것은 안면 마스크 환기 없이 삽관이 뒤따르는 윤상골 압박 동안 의식 상실을 포함합니다. 목적은 가능한 한 빠르고 안전하게 기관을 삽관하는 것입니다.

상세 설명
2018년 4월부터 2019년 6월까지 15개월 동안 University Malaya Medical Center의 수술실에서 전신 마취 하에 선택적 수술을 받는 18세부터 75세까지의 모든 환자를 포함 및 제외 기준에 따라 모집합니다.
기준을 충족하고 연구에 동의한 환자는 2그룹으로 무작위 배정됩니다.
i) BUHE 그룹: 외이도와 흉골 절흔 사이의 정렬을 목표로 20-30도 위로 눕습니다.
ii) 스니핑 그룹: 헤드 레스트(폼 도넛)를 사용하여 머리를 올리고 앙와위 자세를 유지합니다.
마취 유도는 다음과 같이 시작됩니다.
- 100% 산소로 3-5분 동안 전산소화를 수행하여 호기말 산소가 85%에 도달할 때까지 수행합니다.
- 미리 계산된 용량의 유도제를 투여한 후 즉시 신경근 차단제를 투여합니다.
(IV 펜타닐 2mcg/kg, IV 프로포폴 2-3mg/kg, IV 로쿠로늄 1mg/kg).
- 의식을 잃으면 10뉴턴의 윤상연골 압력이 적용되어 30뉴턴까지 증가합니다.
- 적절한 신경근 차단 후, 두 그룹 환자 모두 Macintosh 블레이드 크기 3 또는 4를 사용하여 직접 후두경 검사를 통해 한 명의 조사자가 삽관합니다.
- 호기말 CO2 모니터에서 CO2 감지를 통해 기관내관 배치가 확인될 때까지 Macintosh 블레이드를 구강에 삽입한 시간이 기록됩니다.

제외 기준:
- 기도 폐쇄 환자
- 목 확장이 금기인 환자
- BMI> 35kg/m2
- 이전 삽관 이력으로 인해 기도가 어려운 병력이 있는 환자
- 환자가 입원 기간 동안 여러 번의 수술을 받은 경우 단일 삽관만 포함됩니다.
