목표 1: 환자-간병인 쌍은 기준선을 설정하기 위해 반구조화 인터뷰에 참여합니다. 부부는 CGM 교육 및 장치 사용 지침과 함께 표준 당뇨병 교육 자료를 받게 됩니다. 그런 다음 환자는 14일 동안 CGM 장치를 착용하게 됩니다. 14일의 스캐닝 기간 동안 세 번의 후속 인터뷰와 평가 및 기술 지원을 위한 스캐닝 기간 이후의 보고 인터뷰가 진행됩니다.

환자-간병인 Dyad 포함 기준(목표 1 + 2):
- 환자는 MCI 또는 ADRD 및 당뇨병(DM)의 이중 진단을 받아야 합니다.
- 환자는 DM에 대한 유효한 처방전이 있어야 합니다.
- 환자는 12개월 이내에 Eskenazi 또는 IU Health 1차 진료 클리닉을 최소 한 번 방문해야 합니다.
- 환자가 동의할 능력이 없는 경우 환자는 동의를 제공할 수 있어야 하며 법적 대리인(LAR)의 동의를 받아야 합니다.
- 환자는 매일 또는 거의 매일 환자와 상호 작용하는 18세 이상의 간병인이 있어야 합니다.
- 환자와 간병인은 모두 영어를 구사해야 합니다.
- 환자와 간병인은 모두 지역사회에 거주해야 합니다.

System Usability Scale

The System Usability Scale (SUS) is a 10-item validated questionnaire used to evaluate the usability of the CGM ASSIST interactive information display.
Participants rate each item on a 5-point Likert scale ranging from 1 (Strongly Disagree) to 5 (Strongly Agree).
To calculate the final SUS score, the response for each odd-numbered item is reduced by 1, and the response for each even-numbered item is subtracted from 5.
These adjusted scores are then summed and multiplied by 2.5.
The total score ranges from 0 to 100.
A higher score indicates better usability (e.g., a score above 68 is generally considered above average).

CollaboRATE Questionnaire

The CollaboRATE questionnaire is a 3-item validated patient-reported measure used to assess the level of shared decision-making during clinical encounters.
The three items focus on how much effort was made: 1) to help the participant understand their health issues, 2) to listen to what matters most to the participant, and 3) to include the participant's preferences in the choice of next steps.
Each item is scored on a 5-point Likert scale ranging from 1 (No effort was made) to 5 (Every effort was made).
For each participant, a total score is calculated by summing the scores of the three items.
The total score ranges from 3 to 15.
A higher score indicates a higher perceived level of shared decision-making.

This measure tracks the total number of management changes made to a participant's diabetes care plan based on medical record review.
A "change" is defined as any of the following: 1) Initiation of new medications or technology, 2) New referrals to specialists or community services, 3) Discontinuation of existing medications, and 4) Modification (increase or decrease) of current medication dosages.
