중국의 고혈압 관리에서 원격의료 접근법과 일반적인 치료의 효과 비교

상세 설명
배경: 고혈압은 심혈관계 질환의 주요 위험인자이며 조절률은 전 세계적으로 낮은 수준을 유지하고 있다. 연구에 따르면 원격 모니터링 혈압(BP)은 무작위 대조 시험에서 고혈압을 조절하는 데 도움이 되었습니다. 그러나 일차 진료 의사(PCP)가 전자 의료 데이터 공유 및 의료 개입에서 심장 전문의와 협력하고 있는 구조화된 일차 진료 모델에서의 효과에 대해서는 알려진 바가 거의 없습니다. 이 연구는 중국에서 지역사회 고혈압 관리를 용이하게 하기 위해 원격 진료 도구를 적용하는 조정된 PCP-심장병 전문의 모델의 효과를 확인하는 것을 목표로 합니다.
방법/설계: Shanghai JiaoTong University, Shanghai Chest Hospital과 학문적으로 제휴된 4개의 커뮤니티 의료 센터(CHC)에서 치료를 받는 고혈압 환자는 이전 3개월 동안 혈압이 조절되지 않고 모바일 인터넷에 액세스할 수 있는 경우 자격이 있습니다. 연구 대상자는 3개의 중재적 그룹에 무작위로 할당됩니다. 1) 일반적인 치료; 2) 내장된 GSM 모듈과 무제한 데이터 플랜이 있는 가정 기반 BP 원격 모니터, 개인 의료 기록에 액세스하고 개인화된 라이프스타일 코칭 콘텐츠를 받을 수 있는 앱과 사용 숙련도 교육; 또는 3) PCP와 심장 전문의가 안전한 CareLinker 웹사이트를 통해 데이터를 공유하여 중재적 접근 방식을 결정하는 조정된 PCP-심장 전문의 진료. 1차 결과는 12개월 동안 수축기 혈압(SBP)의 평균 변화입니다. 2차 결과는 확장기 혈압(DBP), HbA1C, 혈중 지질 및 8개 항목의 Morisky Medication Adherence Scale MMAS로 측정한 약물 순응도의 변화입니다.

- 지난 3개월 동안 조절되지 않는 혈압을 동반한 고혈압 임상 진단, 현재 항고혈압제를 복용 중이거나 복용 예정
- 고등학교 이상의 교육을 받은
- 스마트폰(Android 또는 Apple) 및 모바일 앱의 활성 사용자
- CHC에서 스크리닝 방문 중 3회 혈압 측정의 평균은 ≥ 140/90 mm Hg이거나 환자가 당뇨병 또는 신장 질환이 있는 경우 ≥ 130/80 mm Hg입니다.

- 지난 3개월 이내의 뇌졸중
- 신부전
- 암
- 치매, 중증 또는 급성 정신 질환, 임신 또는 향후 18개월 이내 임신 예정, 3개월 이내 입원.
- 추가 제외 기준에는 다른 임상 시험, 팔에 대한 참여가 포함됩니다.
- 원격 모니터의 커프 크기 제한으로 인해 BP 측정의 정확도에 영향을 미칠 수 있는 둘레 >32cm 및 12개월 개입 기간을 준수하지 않으려는 경우.

이 팔의 고혈압 관리는 국가 가이드라인을 준수하며 1기(≥ 140/90mmHg) 고혈압 환자의 경우 1~3개월마다 1회, 2기(≥ 160/100 mm Hg) 및 3기(≥ 180/110 mm Hg) 환자의 경우 2주에 한 번.
