Achtergrond: Hypertensie is een belangrijke risicofactor voor hart- en vaatziekten en het controlepercentage is wereldwijd laag gebleven. Studies hebben aangetoond dat telemonitoring van de bloeddruk (BP) hielp bij het beheersen van hypertensie in gerandomiseerde gecontroleerde onderzoeken. Er is echter weinig bekend over het effect ervan in een gestructureerd eerstelijnszorgmodel waarin huisartsen (PCP) samenwerken met cardiologiespecialisten bij het elektronisch delen van zorggegevens en medische interventies. Deze studie heeft tot doel de effecten te identificeren van een gecoördineerd PCP-cardioloogmodel dat telegeneeskundetools toepast om hypertensiecontrole in de gemeenschap in China te vergemakkelijken.
Methoden/ontwerp: Hypertensieve patiënten die zorg ontvangen in vier gemeenschapsgezondheidscentra (CHC's) die academisch verbonden zijn aan het Shanghai Chest Hospital, Shanghai JiaoTong University, komen in aanmerking als ze in de afgelopen drie maanden een ongecontroleerde bloeddruk hebben gehad en toegang hebben tot mobiel internet. Studieonderwerpen worden willekeurig toegewezen aan drie interventiegroepen: 1) gebruikelijke zorg; 2) thuisgebaseerde BP-telemonitor met ingebouwde GSM-module en onbeperkt data-abonnement, een app om toegang te krijgen tot persoonlijke medische dossiers en gepersonaliseerde inhoud voor levensstijlcoaching te ontvangen, en vaardigheidstraining voor het gebruik ervan; of 3) deze plus gecoördineerde PCP-Cardioloog-zorg waarin huisartsen en cardiologen gegevens delen via een beveiligde CareLinker-website om interventionele benaderingen te bepalen. De primaire uitkomstmaat is de gemiddelde verandering in de systolische bloeddruk (SBD) over een periode van 12 maanden. Secundaire uitkomstmaten zijn veranderingen van de diastolische bloeddruk (DBP), HbA1C, bloedlipiden en therapietrouw gemeten door de Morisky Medication Adherence Scale MMAS met acht items.
Discussie: Deze studie zal bepalen of een gecoördineerd PCP-Cardioloog Telemedicine Model (PCTM) dat de nieuwste telemedicine-technologieën bevat, de hypertensiezorg zal verbeteren. Het succes van het model zou helpen om de huidige zorgprocessen in de gemeenschap te stroomlijnen en een groter aantal ongecontroleerde hypertensieve patiënten te beïnvloeden.

Beschrijving
Inclusiecriteria:
- leeftijd 21 jaar of ouder
- een klinische diagnose van hypertensie met ongecontroleerde bloeddruk in de afgelopen drie maanden, die momenteel antihypertensiva gebruikt of gaat gebruiken
- middelbare school of hoger onderwijs hebben genoten
- actieve gebruiker van smartphone (Android of Apple) en mobiele apps
- het gemiddelde van drie bloeddrukmetingen tijdens het screeningsbezoek aan het CHC is ≥ 140/90 mm Hg, of ≥ 130/80 mm Hg als de patiënt diabetes of nieraandoeningen heeft;
- geïnformeerde toestemming kunnen geven.
Uitsluitingscriteria:
- Acute kransslagader syndroom
- hartfalen
- hartritmestoornissen
- beroerte in de afgelopen drie maanden
- nierfalen
- kanker
- dementie, ernstige of acute psychiatrische ziekte, zwangerschap of intentie om zwanger te worden in de komende 18 maanden en ziekenhuisopname binnen 3 maanden.
- aanvullende uitsluitingscriteria zijn deelname aan een andere klinische proef, arm
- omtrek >32 centimeter die de nauwkeurigheid van de bloeddrukmeting kan beïnvloeden vanwege de limiet van de manchetmaat van de telemonitor en de onwil om te voldoen aan de interventieduur van 12 maanden.

Patiënten worden zoals gebruikelijk behandeld door hun huisarts op de geregistreerde CHC's.

De hypertensiezorg van deze arm voldoet aan de nationale richtlijn en omvat aangewezen follow-ups door huisartsen eens per 1-3 maanden voor stadium 1 (≥ 140/90 mm Hg) hypertensiepatiënten, eenmaal per 1 maand voor stadium 2 (≥ 140/90 mm Hg) 160/100 mm Hg) en eenmaal per 2 weken voor patiënten in stadium 3 (≥ 180/110 mm Hg).

Het primaire eindpunt van de studie zijn veranderingen in de gemiddelde SBP vanaf baseline (T1) tot 12 maanden (T3), gemeten met behulp van de BP-telemonitor (Bliss BL928).
De bloeddrukmetingen over 12 maanden worden bepaald door het gemiddelde te nemen van drie bloeddrukmetingen tijdens het vervolgbezoek aan het CHC.
Alle BP-gegevens worden gelijktijdig verzameld en geüpload naar de proefdatabase.
