- 18세에서 70세 사이의 남녀
- 미주리 또는 일리노이 주에 거주
- 특정 사물 또는 모든 것의 냄새에 대한 불만이 왜곡됩니다.
- COVID-19 감염 의심 후 2개월 이상 지속된 후각 장애
- 영어를 읽고 쓰고 이해하는 능력
제외 기준:
- COVID-19 감염 전 후각 기능 장애의 병력
- 후각 기능 장애의 치료를 위한 병용 요법의 모든 사용
- 12개월 이상의 후각 기능 장애의 병력
- SSRI가 양극성 장애를 이론적으로 불안정하게 만들 수 있기 때문에 양극성 장애의 역사
- Patient Health Questionnaire-9(PHQ-9)에서 10점 이상의 점수로 입증된 우울증 증상이 있는 참여자.95 PHQ-9는 임상 및 지역 사회 환경에서 우울증 환자를 평가하고 진단하는 데 도움이 되도록 설계된 9개 항목으로 구성된 설문지입니다.
- 신경퇴행성 질환(즉, 알츠하이머 치매, 파킨슨병, 레비 소체 치매, 전측두엽 치매)의 병력
- 만성 비부비동염 또는 부비동 수술의 병력
- 임신 또는 수유 중인 산모.
- 이미 다른 COVID 19 약물 시험에 등록했거나 단클론 항체 주입을 받았습니다.
- 도네페질 또는 플루옥세틴(근거: 이 약물은 S1R 작용제임) 또는 세르트랄린(S1R 길항제) 복용.
- 테오필린, 티자니딘, 클로자핀 또는 올란자핀(플루복사민에 의해 억제되는 CYP 1A2에 의해 주로 대사되는 좁은 치료 지수를 가진 약물)를 복용하는 참가자.
- 플루복사민을 추가해도 환자가 세로토닌 증후군의 위험에 처하지 않도록 저용량(예: 에스시탈로프람 5mg)을 사용하지 않는 한 다른 SSRI를 복용합니다.
- 플루복사민으로 출혈이 증가할 이론적 위험에 근거하여 쿠마딘을 복용합니다.

실험적: 플루복사민
이 팔에는 각각 25mg의 경구용 플루복사민 캡슐을 적극적으로 투여할 것입니다. 처음 6주는 점진적으로 적정합니다(1주 및 2주 25mg BID, 3주 및 4주 75mg BID, 5주 및 6주 100mg BID). 다음 6주 동안 100mg TID의 고정 용량을 투여합니다. 마지막 2주는 테이퍼링 다운될 것입니다(첫 주 50mg BID 및 두 번째 주 25mg BID). 14주간의 적극적인 치료가 있을 것이며, 12주가 완료된 후 테이퍼 다운 기간을 시작하기 전에 평가가 수행될 것입니다. |
Fluvoxamine은 우울증과 불안 장애에 사용되는 SSRI입니다.

플루복사민 캡슐처럼 보이고, 냄새가 나고, 맛이 나는 플라시보 캡슐이 플라시보 팔에 주어질 것입니다. 연구의 이중 눈가림을 유지하기 위해 대상자는 적정 일정 후 처음 6주 동안 캡슐 BID 1개를, 고정 용량 기간 동안 다음 6주 동안 캡슐 TID 1개를 받게 됩니다. 그런 다음 피험자는 2주 동안 플루복사민 암을 모방하기 위해 위약을 줄입니다. 테이퍼 다운 기간을 시작하기 전에 고정 용량 기간 완료 후 12주에 평가를 수행할 것입니다. |

CGI(Clinical Global Impression Scale)

CGI에는 CGI-Severity와 CGI-Improvement라는 두 가지 구성 요소가 있습니다. CGI-심각도 척도의 점수 범위는 1에서 7까지이며(1은 정상, 7은 완전한 후각 상실) 기준선에서 환자가 인식하는 기능 장애의 심각도에 대한 정보를 제공합니다. CGI-개선 척도의 점수 범위는 1에서 7까지입니다(1은 매우 많이 개선됨, 7은 매우 많이 악화됨). 각 등급은 정확도를 최대화하기 위해 잘 정의되어 있습니다. CGI-I에서 3개는 최소한으로 개선됨, 2개는 많이 개선됨 또는 1개는 매우 많이 개선됨으로 보고하는 참가자는 치료에 대한 반응자로 간주되고 비반응자에 대한 반응자의 수는 두 부문 사이에서 비교됩니다. CGI-P(Clinical Global Impression-Severity Scale for Parosmia)도 사용됩니다. UPSIT는 각 페이지 하단의 4개 선택형 객관식 질문 바로 아래에 있는 40개의 마이크로캡슐화된 취기제 스트립으로 구성됩니다.
주어진 항목에 대해 환자는 연필 끝으로 마이크로캡슐화 패드를 긁어서 냄새를 방출하고 패드 냄새를 맡고 네 가지 대안에서 냄새 품질을 나타냅니다.
냄새가 나지 않더라도 응답이 필요합니다(즉, 테스트가 강제 선택됨).
40개 항목 중 피험자의 총 정답 점수가 결정되고 후각 기능의 객관적인 척도를 제공합니다.

DASS-21은 우울증, 불안 및 스트레스의 감정 상태를 측정하도록 설계된 세 가지 자기 보고식 척도입니다.
3개의 DASS-21 척도 각각에는 7개의 항목이 포함되어 있으며 유사한 내용을 가진 하위 척도로 나뉩니다.
우울증 척도는 불쾌감, 절망감, 삶의 평가 절하, 자기 비하, 관심 / 참여 부족, 무감동 및 무기력을 평가합니다.
불안 척도는 자율신경 각성, 골격근 효과, 상황 불안, 불안 정서의 주관적 경험을 평가합니다.
스트레스 척도는 만성적인 비특이적 각성 수준에 민감하며 이완의 어려움, 신경 각성, 쉽게 화를 내거나 초조해하는 것, 짜증을 잘 내는 것/과민 반응 및 참을성이 없는 것을 평가합니다.
우울증, 불안, 스트레스에 대한 점수는 해당 항목의 점수를 합산하여 계산합니다.
