Kuvaus
Sisällyttämiskriteerit:
- 18-70-vuotiaat miehet ja naiset
- Asuu Missourin tai Illinoisin osavaltioissa
- Valitukset tiettyjen asioiden tai kaiken hajuista ovat vääristyneitä
- Hajuhäiriö, joka on jatkunut yli 2 kuukautta epäillyn COVID-19-infektion jälkeen
- Kyky lukea, kirjoittaa ja ymmärtää englantia
Poissulkemiskriteerit:
- Aiempi hajuhäiriö ennen COVID-19-infektiota
- Kaikki samanaikaisten hoitojen käyttö erityisesti hajuaistin häiriön hoitoon
- Yli 12 kuukautta kestänyt hajuaistin toimintahäiriö historiassa
- Kaksisuuntainen mielialahäiriö historiassa, koska SSRI-lääkkeet voivat teoriassa horjuttaa kaksisuuntaista mielialahäiriötä
- Osallistujat, joilla on masennuksen oireita, joista on osoituksena vähintään 10 pistettä Potilaan terveyskyselyssä 9 (PHQ-9).95 PHQ-9 on yhdeksän kohdan kyselylomake, joka on suunniteltu arvioimaan ja auttamaan potilaiden diagnosoinnissa kliinisissä ja yhteisöllisissä olosuhteissa.
- Aiemmin hermostoa rappeuttava sairaus (eli Alzheimerin dementia, Parkinsonin tauti, Lewyn kehon dementia, frontotemporaalinen dementia)
- Aiempi krooninen rinosinusiitti tai poskionteloleikkaus
- Raskaana olevat tai imettävät äidit.
- Olet jo ilmoittautunut toiseen COVID 19 -lääketutkimukseen tai saanut monoklonaalisen vasta-aineen infuusion.
- Donepetsiilin tai fluoksetiinin (perustelua: nämä lääkkeet ovat S1R-agonisteja) tai sertraliinin (S1R-antagonisti) ottaminen.
- Osallistujat, jotka käyttävät teofylliiniä, titsanidiinia, klotsapiinia tai olantsapiinia (lääkkeitä, joilla on kapea terapeuttinen indeksi ja jotka metaboloituvat ensisijaisesti CYP 1A2:n vaikutuksesta, jota fluvoksamiini estää).
- Toisen SSRI:n ottaminen, paitsi jos se on pieni annos (esim. escitalopraami 5 mg), jotta fluvoksamiinin lisääminen ei vaaranna potilasta serotoniinioireyhtymästä.
- Kumadiinin ottaminen perustuu teoreettiseen riskiin lisätä verenvuotoa fluvoksamiinin kanssa.
- Tietoon perustuvaa suostumusta ei voida antaa.

Kokeellinen: Fluvoksamiini
Tälle käsille annetaan aktiivista hoitoa, 25 mg:n oraalisia fluvoksamiinikapseleita. Ensimmäiset kuusi viikkoa on asteittainen titraus (viikot 1 ja 2 25 mg BID, viikot 3 ja 4 75 mg BID, viikot 5 ja 6 100 mg BID). Seuraavien kuuden viikon kiinteä annos on 100 mg kolme kertaa päivässä. Viimeiset kaksi viikkoa ovat vähentyneet (ensimmäinen viikko 50 mg BID ja toinen viikko 25 mg BID) Aktiivihoitoa on 14 viikkoa ja arvioinnit suoritetaan viikon 12 jälkeen, ennen kevennysjakson aloittamista. |
Fluvoksamiini on SSRI-lääke, jota käytetään masennukseen ja ahdistuneisuushäiriöihin.
Tässä tutkimuksessa tutkitaan fluvoksamiinin tehoa hajuhäiriöiden parantamisessa potilailla, joilla on post-Covid-19 -parosmia.

Placebo Comparator: Plasebo
Plasebokapseleita, jotka näyttävät, tuoksuvat ja maistuvat fluvoksamiinikapseleilta, annetaan lumeryhmälle. Tutkimuksen kaksoissokkoutumisen säilyttämiseksi koehenkilöt saavat yhden kapselin kahdesti vuorokaudessa titrausaikataulua seuraavien kuuden ensimmäisen viikon aikana ja yhden kapselin TID seuraavien kuuden viikon aikana kiinteän annoksen jakson aikana. Koehenkilöt pienentävät lumelääkettä jäljittelemään fluvoksamiinivartta kahden viikon ajan. Kliininen globaali impressioasteikko (CGI)

CGI:ssä on kaksi osaa - CGI-vakavuus ja CGI-parannus. CGI-vakavuusasteikon pisteet vaihtelevat 1–7 (1 on normaali, 7 on täydellinen hajun menetys) ja ne antavat tietoa potilaan havaitseman toimintahäiriön vakavuudesta lähtötasolla. CGI-parannusasteikon pisteet vaihtelevat välillä 1-7 (1 on erittäin paljon parantunut, 7 on erittäin huonontunut). Jokainen luokitus on tarkasti määritelty tarkkuuden maksimoimiseksi. UPSIT koostuu 40 liuskasta mikrokapseloituja hajusteita, jotka ovat jokaisen sivun alaosassa, juuri neljän vaihtoehdon monivalintakysymyksen alapuolella.
Tietystä esineestä potilas vapauttaa hajun raaputtamalla mikrokapseloitua tyynyä kynän kärjellä, haistaa tyynyn ja ilmoittaa hajun laadun neljästä vaihtoehdosta.
Vaikka hajua ei havaitakaan, vastaus vaaditaan (eli testi on pakkovalinta).
Kohteen oikea kokonaispistemäärä 40 pisteestä määritetään ja tarjoaa objektiivisen mittarin hajutoiminnasta.

DASS-21 on kolmen itseraportointiasteikon sarja, jotka on suunniteltu mittaamaan masennuksen, ahdistuksen ja stressin emotionaalista tilaa.
Kukin kolmesta DASS-21-asteikosta sisältää 7 kohdetta, jotka on jaettu samansisältöisiin aliasteikoihin.
Masennusasteikko arvioi dysforiaa, toivottomuutta, elämän devalvoitumista, itsensä halveksuntaa, kiinnostuksen/osallistumisen puutetta, anhedoniaa ja inertiaa.
Ahdistuneisuusasteikko arvioi autonomista kiihottumista, luustolihasvaikutuksia, tilanneahdistusta ja subjektiivista kokemusta ahdistuneisuudesta.
Stressiasteikko on herkkä kroonisen epäspesifisen kiihottumisen tasoille ja arvioi rentoutumisvaikeutta, hermostunutta kiihottumista ja hermostuneisuutta/levottomuutta, ärtyisyyttä/ylireaktiivisuutta ja kärsimättömyyttä.
Masennuksen, ahdistuneisuuden ja stressin pisteet lasketaan summaamalla asiaankuuluvien kohteiden pisteet.

SF-36 on 36 kohdan kyselylomake, jossa arvioidaan fyysistä toimintaa, kehon kipua, fyysisistä terveysongelmista johtuvia roolirajoituksia, henkilökohtaisista tai henkisistä ongelmista johtuvia roolirajoituksia, emotionaalista hyvinvointia, sosiaalista toimintaa, energiaa/väsymystä ja yleisiä terveyskäsityksiä. .
Tämän testin avulla voimme tutkia COVID-19:ään liittyvästä OD:sta kärsivien yleisten terveydentilan ja elämänlaadun muutoksia.
