Beschrijving
Inclusiecriteria:
- Mannen en vrouwen tussen de 18 en 70 jaar
- Woonachtig in de staten Missouri of Illinois
- Klachten over geuren van bepaalde dingen of van alles worden vervormd
- Olfactorische disfunctie die langer dan 2 maanden aanhoudt na een vermoedelijke COVID-19-infectie
- Mogelijkheid om Engels te lezen, schrijven en begrijpen
Uitsluitingscriteria:
- Geschiedenis van olfactorische disfunctie voorafgaand aan COVID-19-infectie
- Elk gebruik van gelijktijdige therapieën specifiek voor de behandeling van olfactorische disfunctie
- Geschiedenis van olfactorische disfunctie van langer dan 12 maanden
- Geschiedenis van een bipolaire stoornis, aangezien SSRI's in theorie een bipolaire stoornis kunnen destabiliseren
- Deelnemers met symptomen van depressie zoals blijkt uit een score van 10 of hoger op de Patient Health Questionnaire-9 (PHQ-9).95 De PHQ-9 is een vragenlijst met negen items die is ontworpen om patiënten met depressie in klinische en gemeenschapsomgevingen te beoordelen en te helpen diagnosticeren.
- Geschiedenis van neurodegeneratieve ziekte (d.w.z. de ziekte van Alzheimer, de ziekte van Parkinson, Lewy body dementie, frontotemporale dementie)
- Geschiedenis van chronische rhinosinusitis of sinuschirurgie
- Zwangere moeders of moeders die borstvoeding geven.
- Reeds ingeschreven in een andere COVID 19-medicatieproef of ontvangst van monoklonale antilichaaminfusie.
- Het gebruik van donepezil of fluoxetine (redenering: deze geneesmiddelen zijn S1R-agonisten) of sertraline (een S1R-antagonist).
- Deelnemers die theofylline, tizanidine, clozapine of olanzapine gebruiken (geneesmiddelen met een smalle therapeutische index die voornamelijk worden gemetaboliseerd door CYP 1A2, dat wordt geremd door fluvoxamine.
- Het nemen van een andere SSRI, tenzij bij een lage dosis (bijv. escitalopram 5 mg) zodanig dat toevoeging van fluvoxamine de patiënt geen risico op het serotoninesyndroom zou opleveren.
- Coumadin gebruiken op basis van het theoretische risico op verhoogde bloedingen met fluvoxamine.
- Kan geen geïnformeerde toestemming geven.
- Kan de studieprocedures niet uitvoeren.

Experimenteel: Fluvoxamine
Deze arm krijgt de actieve behandeling, orale fluvoxamine-capsules van elk 25 mg. De eerste zes weken zullen geleidelijk worden getitreerd (week 1 en 2 25 mg tweemaal daags, weken 3 en 4 75 mg tweemaal daags, weken 5 en 6 100 mg tweemaal daags). De volgende zes weken zal een vaste dosis van 100 mg TID zijn. De laatste twee weken wordt er afgebouwd (eerste week 50 mg tweemaal daags en tweede week 25 mg tweemaal daags) Er zal 14 weken actieve behandeling zijn en evaluaties zullen worden uitgevoerd na voltooiing van week 12, voorafgaand aan het begin van de afbouwperiode. |
Fluvoxamine is een SSRI die wordt gebruikt bij depressie en angststoornissen.
Deze studie zal de werkzaamheid van fluvoxamine onderzoeken voor het verbeteren van olfactorische disfunctie bij proefpersonen met post-Covid-19 parosmie.

Placebo-vergelijker: Placebo
Placebo-capsules die eruit zien, ruiken en smaken als fluvoxamine-capsules, worden aan de placebo-arm gegeven. Om de studie dubbelblind te houden, krijgen de proefpersonen gedurende de eerste zes weken na het titratieschema één capsule tweemaal daags en gedurende de volgende zes weken gedurende de vaste dosisperiode één capsule driemaal daags. Proefpersonen zullen vervolgens de placebo afbouwen om gedurende twee weken de fluvoxamine-arm te imiteren. Beoordeling vindt plaats 12 weken na voltooiing van de periode met een vaste dosis, voorafgaand aan de start van de afbouwperiode. |

Clinical Global Impression Scale (CGI)

De CGI heeft twee componenten: de CGI-Severity en de CGI-Improvement. Scores op de CGI-Severity Scale variëren van 1 tot 7 (1 is normaal, 7 is volledig reukverlies) en geven informatie over de door de patiënt waargenomen ernst van hun disfunctie bij baseline. De score op de CGI-verbeteringsschaal varieert van 1 tot 7 (1 is zeer veel verbeterd, 7 is zeer veel verslechterd). Elke beoordeling is goed gedefinieerd om de nauwkeurigheid te maximaliseren. Deelnemers die 3 als Minimaal verbeterd, 2 als Veel verbeterd of 1 als Zeer veel verbeterd rapporteren in de CGI-I, worden beschouwd als responders op de behandeling, en het aantal responders op non-responders zal tussen de twee armen worden vergeleken. De Clinical Global Impression-Severity Scale for Parosmia (CGI-P) zal ook worden gebruikt. De CGI-P-schaal loopt van 1 tot 5, waarbij 1 staat voor Geen vervorming, 2 voor Milde vervorming, 3 voor Matige vervorming, 4 voor Grotendeels vervorming en 5 voor Volledige vervorming. |

De UPSIT is samengesteld uit 40 stroken micro-ingekapselde geurstoffen, die onderaan elke pagina aanwezig zijn, net onder een vier-alternatieve meerkeuzevraag.
Voor een bepaald item geeft de patiënt een geur af door met een potloodpunt over het micro-ingekapselde kussentje te krabben, ruikt het kussentje en geeft de geurkwaliteit aan uit vier alternatieven.
Zelfs als er geen geur wordt waargenomen, is een reactie vereist (d.w.z. de test is een gedwongen keuze).
De totale correcte score van de proefpersoon van de 40 items wordt bepaald en biedt een objectieve maatstaf voor de reukfunctie.

De DASS-21 is een set van drie zelfrapportageschalen die zijn ontworpen om de emotionele toestand van depressie, angst en stress te meten.
Elk van de drie DASS-21-schalen bevat 7 items, verdeeld in subschalen met vergelijkbare inhoud.
De depressieschaal beoordeelt dysforie, hopeloosheid, devaluatie van het leven, zelfspot, gebrek aan interesse / betrokkenheid, anhedonie en inertie.
De angstschaal beoordeelt autonome opwinding, skeletspiereffecten, situationele angst en subjectieve ervaring van angstig affect.
De stressschaal is gevoelig voor niveaus van chronische niet-specifieke opwinding en beoordeelt de moeilijkheid om te ontspannen, nerveuze opwinding en gemakkelijk overstuur/geagiteerd, prikkelbaar/overreactief en ongeduldig zijn.
Scores voor depressie, angst en stress worden berekend door de scores op de relevante items op te tellen.

De SF-36 is een vragenlijst met 36 items die fysiek functioneren, lichamelijke pijn, rolbeperkingen als gevolg van fysieke gezondheidsproblemen, rolbeperkingen als gevolg van persoonlijke of emotionele problemen, emotioneel welzijn, sociaal functioneren, energie/vermoeidheid en algemene gezondheidspercepties evalueert. .
Met deze test kunnen we de algemene veranderingen in de algehele kwaliteit van leven in de gezondheid bestuderen voor mensen die lijden aan COVID-19-gerelateerde OD.
