Inklusjonskriterier:
- Menn og kvinner mellom 18 og 70 år
- Bosatt i delstatene Missouri eller Illinois
- Klager på lukt av visse ting eller alt er forvrengt
- Olfaktorisk dysfunksjon som har vedvart i >2 måneder etter mistanke om COVID-19-infeksjon
- Evne til å lese, skrive og forstå engelsk
Ekskluderingskriterier:
- Historie med luktdysfunksjon før COVID-19-infeksjon
- Enhver bruk av samtidig terapi spesielt for behandling av luktdysfunksjon
- Historie med luktdysfunksjon på mer enn 12 måneder
- Anamnese med bipolar lidelse da SSRI-er teoretisk kan destabilisere bipolar lidelse
- Deltakere med symptomer på depresjon som dokumentert av en score på 10 eller høyere på pasienthelsespørreskjemaet-9 (PHQ-9).95 PHQ-9 er et ni-elements spørreskjema designet for å vurdere og hjelpe til med å diagnostisere pasienter med depresjon i kliniske og samfunnsmessige omgivelser.
- Historie med nevrodegenerativ sykdom (dvs. Alzheimers demens, Parkinsons sykdom, Lewy body demens, frontotemporal demens)
- Historie med kronisk rhinosinusitt eller bihulekirurgi
- Gravide eller ammende mødre.
- Allerede registrert i en annen COVID 19-medisinstudie eller mottak av infusjon av monoklonalt antistoff.
- Tar donepezil eller fluoksetin (begrunnelse: disse legemidlene er S1R-agonister) eller sertralin (en S1R-antagonist).
- Deltakere som tar teofyllin, tizanidin, klozapin eller olanzapin (legemidler med en smal terapeutisk indeks som primært metaboliseres av CYP 1A2, som hemmes av fluvoksamin.
- Å ta en annen SSRI, med mindre i en lav dose (f.eks. escitalopram 5 mg) slik at tilsetning av fluvoksamin ikke vil sette pasienten i fare for serotonergt syndrom.
- Tar coumadin basert på teoretisk risiko for økt blødning med fluvoksamin.
- Kan ikke gi informert samtykke.
- Kan ikke utføre studieprosedyrene.

Eksperimentell: Fluvoxamin
Denne armen vil få den aktive behandlingen, orale fluvoksaminkapsler på 25 mg hver. De første seks ukene vil være gradvis titrering (uke 1 og 2 25 mg BID, uke 3 og 4 75 mg BID, uke 5 og 6 100 mg BID). De følgende seks ukene vil være fast dose på 100 mg tre ganger daglig. De siste to ukene vil være en nedtrapping (første uke 50mg BID og andre uke 25mg BID) Det vil være 14 uker med aktiv behandling og vurderinger vil bli utført etter fullføring av uke 12, før nedtrappingsperioden starter. |
Fluvoxamine er et SSRI som brukes mot depresjon og angstlidelser.
Denne studien vil undersøke effekten av fluvoksamin for forbedring av luktdysfunksjon hos personer med post-Covid-19 parosmi.

Placebo komparator: Placebo
Placebokapsler som ser ut, lukter og smaker som fluvoksaminkapsler vil bli gitt til placeboarmen. For å bevare dobbeltblindingen av studien, vil forsøkspersonene motta én kapsel BID i løpet av de første seks ukene etter titreringsplanen og en kapsel TID i løpet av de neste seks ukene for den faste doseperioden. Forsøkspersonene vil deretter trappe ned placebo for å imitere fluvoksaminarmen i to uker. Vurderinger vil bli utført 12 uker etter fullføring av fastdoseperioden, før nedtrappingsperioden starter. |

Clinical Global Impression Scale (CGI)

CGI har to komponenter - CGI-alvorlighet og CGI-forbedring. Poeng på CGI-alvorlighetsskalaen varierer fra 1 til 7 (1 er normal, 7 er fullstendig lukttap) og gir informasjon om pasientens opplevde alvorlighetsgrad av funksjonssvikten ved baseline. Poengsummen på CGI-forbedringsskalaen varierer fra 1 til 7 (1 er veldig mye forbedret, 7 er veldig mye forverret). Hver vurdering er godt definert for å maksimere nøyaktigheten. Deltakere som rapporterer 3 som minimalt forbedret, 2 som mye forbedret eller 1 som svært mye forbedret i CGI-I vil bli ansett som respondere på behandling, og antall respondere på ikke-respondere vil bli sammenlignet mellom de to armene. Clinical Global Impression-Severity Scale for Parosmia (CGI-P) vil også bli brukt. University of Pennsylvania Smell Identification Test (UPSIT)

UPSIT består av 40 strimler med mikroinnkapslede luktstoffer, som er tilstede nederst på hver side, rett under et fire-alternativ flervalgsspørsmål.
For en gitt gjenstand frigjør pasienten en lukt ved å skrape den mikroinnkapslede puten med en blyantspiss, lukter på puten og indikerer luktkvaliteten fra fire alternativer.

Depresjon, angst og stress-skalaen - 21 elementer (DASS-21)

DASS-21 er et sett med tre selvrapporteringsskalaer designet for å måle den emosjonelle tilstanden depresjon, angst og stress.
Hver av de tre DASS-21-skalaene inneholder 7 elementer, delt inn i underskalaer med lignende innhold.
Depresjonsskalaen vurderer dysfori, håpløshet, devaluering av livet, selvironering, manglende interesse/engasjement, anhedoni og treghet.
Angstskalaen vurderer autonom opphisselse, skjelettmuskeleffekter, situasjonsangst og subjektiv opplevelse av engstelig affekt.
Stressskalaen er følsom for nivåer av kronisk ikke-spesifikk opphisselse og vurderer problemer med å slappe av, nervøs opphisselse og å være lett opprørt/opprørt, irritabel/overreaktiv og utålmodig.

SF-36 er et spørreskjema med 36 elementer som evaluerer fysisk funksjon, kroppslig smerte, rollebegrensninger på grunn av fysiske helseproblemer, rollebegrensninger på grunn av personlige eller emosjonelle problemer, emosjonelt velvære, sosial funksjon, energi/tretthet og generelle helseoppfatninger .
Denne testen lar oss studere den generelle helsemessige generelle livskvalitetsendringene for de som lider av COVID-19-relatert OD.
