담석 및 담낭 절제술 환자의 심혈관 질환(CVD) 및 비알코올성 지방간 질환(NAFLD)과 새로운 식단 및 근육 훈련 프로그램의 영향

상세 설명
배경. 담석 및 담낭 절제술을 포함한 담석 질환(GSD)은 전 세계적으로 흔한 소화기 질환이며 칠레는 보고된 담석 유병률이 가장 높은 국가 중 하나입니다. GSD, 특히 담낭절제술은 비알코올성 지방간 질환(NAFLD)과 관련이 있습니다. 이전 연구에서 연구자들은 담낭절제술과 남성에서만 NAFLD 위험이 더 높은 연관성을 확인했습니다. 또한 연구원들은 남성 담석 보균자의 경우 3년 내에 심혈관 사건 위험이 더 높다는 사실을 발견했습니다. 조사자들은 우리 인구에 맞춤화된 위험 감소 전략의 효과를 평가함으로써 중개 연구를 향해 나아가는 것을 목표로 합니다.
연구 목표. 다음을 기반으로 대사 위험 감소 전략을 평가하기 위해: 1. 근육량과 근력을 증가시키기 위한 신체 운동 시도, 2. 항염증 식단, 수면 위생, 장기 단식을 기반으로 한 영양 중재.
행동 양식. 집중적인 라이프스타일 개입, 무작위 대조 시험: 300명의 참가자가 대조군(n=150) 또는 실험군(n=150)에 무작위로 할당됩니다. 통제 그룹은 표준 예방 권장 사항, 맞춤형 조언, 생활 습관 및 대사 질환에 관한 교육 자료, 신체 활동, 신체 상태, 인체 측정, 식이 및 수면 습관에 대한 후속 평가를 받게 됩니다. 중재 그룹은 각 참가자에 맞게 조정됩니다. 3.1 근육 형성을 강조하는 직접 감독 및 가정 기반 원격 건강 세션으로 구성된 운동 프로그램 및 3.2. 항염증 성분과 콩류가 풍부하고 지방이 적고 정제된 탄수화물이 풍부한 식단, BMI에 따른 에너지 조절, 야간 단식 기간 연장, 건강한 수면 습관을 기반으로 한 영양 플러스 프로그램입니다. 이 프로그램은 지자체 체육관에서 직접 감독하는 격주 운동 세션과 주간 원격 건강 세션의 6개월 집중 단계를 거친 후 6개월 전화 지원 및 격월 직접 감독을 통해 순응도를 강화하고 위험 요인을 조기에 식별합니다. 포기를 위해. 기준선과 1년 및 2년차에 측정된 개입 결과는 NAFLD의 비침습적 혈청 및 초음파 기반 바이오마커의 변화입니다. 근감소증, 호기성 능력, 체성분, 지질 프로파일, 인슐린 저항성 및 심혈관 위험의 개선; 과체중 참가자 중 최소 5% 체중의 영구적인 손실; 우울증 증상 감소, 삶의 질 점수 향상, 건강한 생활 습관 유지.
예상 결과: 대사 보호 식단과 함께 개인화된 신체 활동 프로그램의 12개월 중재는 대사 증후군 및 NAFLD의 지표를 개선하고 두 중재의 50% 이상을 준수하는 모든 참가자의 CVD 위험 점수를 감소시킬 것으로 예상됩니다. 명확한 용량 반응 효과가 있습니다.

간섭 없음: 치료의 표준
컨트롤 암의 참가자는 먼저 인체 측정 평가, 생체 임피던스 분석, 간담도 초음파, 섬유 스캔, 혈중 지질, 대사 지표 배터리 및 식단 조사, 자동 자가 관리 24시간 식단 평가를 통해 영양 상태에 대한 기준선 평가를 받습니다. 도구(ASA24).
평가 기간 후 참가자들은 서면 자료를 포함하여 1회의 상담 세션을 받았습니다. 여기에는 과일과 채소가 풍부한 건강한 식단, 통곡물 식품, 저염분 및 설탕 섭취, 최소 30분 운동 계획에 대한 권장 사항이 포함되어 있습니다. 일주일에 세 번 유산소 운동을 한다.

운동 프로그램은 근력 및 유산소 운동으로 구성됩니다. 또한 각 세션은 저강도 워밍업 및 쿨다운 기간으로 시작하고 끝납니다.
이 프로그램은 몇 개월로 나누어집니다.
1-2개월, 일주일에 두 번 직접 감독하는 운동 그룹 세션 및 원격 의료 세션 1회.
3-6개월, 일주일에 한 번 직접 감독하는 그룹 세션 및 일주일에 두 번 가정 기반 원격 의료.
7-12개월, 월간 전화 지원.
12, 24, 36개월에 모든 참가자(통제 및 개입 부문)는 완전한 평가를 받게 됩니다.
식이 개입에서 연구자들은 항염증 성분, 콩류 및 식이 섬유가 풍부한 식단을 제안했습니다.
영양 상태의 기본 평가에는 인체측정법, 생체 임피던스 분석, 간담도 초음파, Fibroscan, 혈중 지질, 대사 지표 배터리 및 식이 조사, 자동 자가 관리 24시간 식이 평가 도구(ASA24)가 포함됩니다.
두 중재는 병렬로 수행됩니다.
