말라리아 발열 무작위 통제 시험(RCT)의 MRI 보조 연구
이 연구는 Malaria FEVER 연구에 등록된 아동의 부모로부터 동의를 얻어 자녀에 대한 신경 영상 및 12개월 신경정신과 결과 데이터 및 신장 기능을 얻을 것입니다.

박멸 노력에도 불구하고 2016년 약 400,000명의 아프리카 어린이들이 CNS 말라리아로 인해 뇌 손상을 입었습니다.
급성 말라리아 감염 동안 더 높은 최고 온도(Tmax)는 신경학적 후유증에 대한 확립된 위험 요소이며 말라위와 잠비아에서 실시된 아세트아미노펜과 이부프로펜을 사용한 공격적인 해열 요법의 무작위 통제 시험(RCT)이 2019년에 등록을 시작했습니다(R01NS102176).
해당 임상 시험에서 일차 결과는 급성 감염 동안의 Tmax였습니다.
그러나 이 RCT에 사용된 해열 요법은 Tmax에 영향을 미치지 않고 신경 보호 효과(예: 항염증 메커니즘을 통한 신경 보호)를 제공할 수 있습니다.
말라리아 발열 RCT의 MRI 보조 연구에서 우리는 CNS 말라리아에 대한 공격적인 해열 요법의 잠재적인 신경 보호 효과를 추가로 평가하고 이러한 효과에 대한 가능한 메커니즘을 탐색하기 위해 RCT의 맥락에서 신경 영상을 사용할 것을 제안합니다.
중추 신경계 말라리아에서 회복된 후 구조적 뇌 이상 유병률에 대한 공격적인 해열 요법과 일반 치료에 할당된 어린이를 비교하면 신경 보호를 위한 비열 경로의 평가가 용이해질 것입니다.
뇌 MRI는 회복 후 1개월 및 12개월에 RCT에 등록된 어린이에게서 얻을 것입니다.
열 감소 측면에서 치료에 대한 반응과 구체적으로 관련된 변화를 평가하기 위해 RCT 치료 할당 및 치료 할당에 의해 계층화된 (Tmax)를 기반으로 구조적 손상이 있을 확률을 비교하는 분석이 완료됩니다.
치료 관련 CNS 출혈에 대한 평가를 포함하여 공격적인 해열 관련 손상의 잠재적 메커니즘을 평가할 것입니다.
Neuroimaging은 소아 CNS 말라리아를 포함하여 뇌 손상 후 신경학적 결과에 대한 잘 확립되고 유효한 프록시입니다.

- 학부모 말라리아 RCT 연구에 등록된 모든 아동은 자격이 있습니다.
- 서명된 부모 동의가 필요합니다.
- 말라리아와 관련된 CNS 증상.
제외 기준
- 순환 장애(사지 냉증, 모세혈관 재충전 > 3초, 움푹 들어간 눈, ↓ 피부 팽만감)
- 지난 2시간 동안 구토
- 혈청 Cr > 1.2 mg/dL
- 간 질환의 역사
- 황달 또는 >3.0mg/dL의 총 빌리루빈
- 위궤양 또는 위장관 출혈의 병력
- 혈소판 감소증 또는 기타 원발성 혈액 장애의 병력
