撲滅の努力にもかかわらず、2016 年には中枢神経系マラリアの結果、アフリカの 40 万人に上る子供たちが脳損傷を被りました。
急性マラリア感染中の最高体温 (Tmax) の上昇は、神経学的後遺症の確立された危険因子であり、マラウイとザンビアで実施されたアセトアミノフェンとイブプロフェンによる積極的な解熱療法のランダム化比較試験 (RCT) が 2019 年に登録を開始しました (R01NS102176)。
その臨床試験では、主要な結果は急性感染中の Tmax でした。
ただし、この RCT で使用される解熱療法は、Tmax に影響を与えずに神経保護効果を提供する可能性があります。たとえば、抗炎症メカニズムによる神経保護です。
An MRI Ancillary Study of Malaria FEVER RCT では、RCT のコンテキストでニューロイメージングを使用して、CNS マラリアに対する積極的な解熱療法の潜在的な神経保護効果をさらに評価し、これらの効果の可能なメカニズムを調査することを提案します。
中枢神経系マラリアからの回復後の構造的な脳異常の有病率について、積極的な解熱療法と通常のケアに割り当てられた子供を比較すると、神経保護のための非発熱経路の評価が容易になります。
脳MRIは、回復後1か月および12か月でRCTに登録された子供で取得されます。
分析は、RCT治療の割り当てに基づいて、および治療の割り当てによって層別化された(Tmax)に基づいて、構造的損傷を有する確率を比較して完了し、発熱の観点から治療への反応に特に関連する変化を評価します。
攻撃的な解熱剤関連の傷害の潜在的なメカニズムは、治療関連のCNS出血の評価を含めて評価されます。
ニューロ イメージングは、小児中枢神経系マラリアを含む脳損傷後の神経学的転帰の十分に確立された有効なプロキシです。
この MRI 補助研究を私たちの発熱 RCT に追加することで、治療に関連する損傷のメカニズムが解明され、そうでなければ認知および行動評価のための長期的な追跡調査が必要となる積極的な解熱剤の使用による神経保護の早期発見が可能になる可能性があります。
この研究は、研究デザインを最適化するためにイメージングを組み込む可能性のあるマラリアの将来の神経保護研究に役立つ重要な洞察を提供します。

包含基準
- 親マラリアRCT研究に登録されているすべての子供が対象です
- 署名された保護者の同意が必要です
- マラリアに伴う中枢神経症状。
除外基準
- 循環不全(四肢の冷え、3秒を超える毛細血管の再充満、くぼんだ目、皮膚の張りの↓)
- 過去2時間の嘔吐
- 血清Cr > 1.2mg/dL
- 肝疾患の病歴
- 黄疸または総ビリルビンが3.0mg/dLを超える
- 胃潰瘍または消化管出血の病歴
- -血小板減少症またはその他の原発性血液疾患の病歴
- 点状出血またはその他の出血異常の臨床徴候
- -イブプロフェン、アセトアミノフェン、アスピリンまたは非ステロイド薬に対する既知のアレルギー
- -経鼻胃管(NGT)の配置および/または経腸薬の送達に対する禁忌
