Veel epidemiologische studies hebben de blootstelling aan door het verkeer veroorzaakte luchtverontreiniging door motorvoertuigen, luchttoxines uit de industrie en andere milieutoxines door zwangere vrouwen in verband gebracht met metingen van slechte geboorte-uitkomsten, waaronder pre-eclampsie (PE), vroeggeboorte (PTB) en intra-uteriene groeivertraging. (IUGR). Pre-eclampsie, vroeggeboorte en IUGR, gezamenlijk bekend als ischemische placentaziekte, zijn sterk gecorreleerd met kindermorbiditeit en tal van volwassen ziekten, variërend van coronaire hartziekte tot kanker. Hoewel algemeen wordt aangenomen dat de pathofysiologische mechanismen die leiden tot complicaties van ischemische placentaziekte en placenta-insufficiëntie een vergelijkbare biologische oorsprong hebben, beginnend al bij een defecte placenta-implantatie, zijn er tot op heden geen voorspellende studies die prospectief de structuur en/of functie van de placenta onderzoeken. Dit kan te maken hebben met bestaande technologieën die routinematig worden gebruikt bij klinische besluitvorming, zoals echografie en bepaalde biomarkers, die niet nauwkeurig zijn in het eerste trimester en onvoldoende gevoeligheid en specificiteit in het tweede tot derde trimester. Beoordeling en voorspelling van normaliteit versus afwijking van de placentafunctie ontbreken dus. Daarom is de overkoepelende doelstelling van dit voorstel het ontwikkelen en evalueren van een reeks geavanceerde multi-parametrische magnetische resonantie beeldvormingstechnologieën (mp-MRI) (primaire voorspeller), en deze nieuwe placentale beeldvormingsmodaliteiten te vertalen naar het beoordelen van de impact van milieuvervuiling. blootstelling op voorspelling van de samenstelling van placentaafwijking/-insufficiëntie en gerelateerde uitkomsten (PE, PTB en IUGR) (primaire uitkomst).
Onze centrale hypothese is dat chronische blootstelling aan hoge mate van milieuvervuiling, onafhankelijk van sociaal-economische status (SES), het risico op placenta-insufficiëntie verhoogt als gevolg van vroege zwangerschapsontwikkeling van ongunstige placenta-structuur / -functie, zoals gedetecteerd door mp-MRI technologische vooruitgang.
Om deze hypothese te testen, zal ons interdisciplinaire onderzoeksteam aan de UCLA eerst non-contrast freebreathing placenta mp-MRI ontwikkelen, bestaande uit multi-delay pseudo-continuous arterial spin labeling (pCASL) perfusie en multi-contrast structurele MRI met hoge resolutie, die zorgen voor reproduceerbare metingen van placentaire perfusie en weefselmicrostructuur (AIM 1). De nieuwe mp-MRI-technieken zullen worden vergeleken met de baarmoederslagader Doppler-echografie tijdens de zwangerschap en uiteindelijk worden gevalideerd door de gouden standaard histologische pathologie en vasculariteit van de post-parturiënte placenta (AIM 2). Ten slotte zullen de onderzoekers diepgaande analyses uitvoeren van gerekruteerde proefpersonen op blootstelling aan milieuvervuiling en hun blootstellingspercentages correleren met uitgebreid verzamelde zwangerschapsuitkomsten, om een voorspellend model op te bouwen (AIM 3). Onze specifieke doelstellingen zijn:
DOEL 1: Ontwikkeling van non-contrast vrij-ademende placentale mp-MRI-technieken die op niet-invasieve wijze de in vivo placentale perfusie en microstructuur door alle trimesters kunnen karakteriseren.
A. Ontwikkel een vrij ademende pCASL-sequentie met meerdere vertragingen met behulp van 3D GRASE-beeldvorming met achtergrondonderdrukt gelokaliseerd volume voor kwantitatieve meting van placentaire perfusie binnen een scan van 5-10 minuten (1,5x1,5x2 mm3 resolutie).
B. Ontwikkel een vrij ademende 3D stapel-van-radiale reeks met gouden hoek met hoge resolutie met zelfnavigatie en retrospectieve bewegingscompensatie met behulp van gecomprimeerde detectie voor het in kaart brengen van de microstructuur van de placenta binnen een scan van 5-10 minuten (1x1x1 mm3 isotrope resolutie).
DOEL 2: Evaluatie van mp-MRI van de placenta door longitudinale vergelijking met echografie van de uteriene arterie Doppler tijdens de zwangerschap als een voorspellende marker van placenta-insufficiëntie.
A. Voer een 3-jarige prospectieve cohortstudie uit bij zwangere vrouwen, waarbij nieuw ontwikkelde mp-MRI (AIM1) in het 1e en 2e trimester wordt uitgevoerd, parallel aan ultrasone meting van de uteriene arterie Doppler, om het voorspellingsvermogen van mp-MRI versus echografie te beoordelen.
B. Verzamel gedetailleerde gegevens over zwangerschapsuitkomsten van een prospectief cohort, inclusief diagnose, neonatale uitkomst en placenta voor pathologische en histologische analyse om risicofactoren vast te stellen voor placenta-insufficiëntie als reactie op omgevingsblootstelling (AIM 3).
DOEL 3: Voer diepgaande analyses uit bij gerekruteerde proefpersonen om associaties van blootstelling aan milieuverontreiniging en zowel placentale mp-MRI-metingen als bijkomende zwangerschapsuitkomsten te bepalen.
A. Voer diepgaande analyses uit op metingen van gemodelleerde blootstelling aan milieuverontreiniging bij onze aangeworven proefpersonen om associaties met mp-MRI-metingen van placentastructuur en perfusie tijdens de zwangerschap te beoordelen, rekening houdend met meerdere verstorende factoren zoals de leeftijd van de moeder bij de bevalling, ras/etniciteit , en SES.
B. MP-MRI-metingen van placentastructuur en perfusie tijdens de zwangerschap correleren met zwangerschapsuitkomsten, postparturiële beoordeling van placentagewicht, pathologie en neonatale uitkomsten. Deze studies zullen de impact van milieuvervuiling op de structuur en functie van de placenta en de gerelateerde zwangerschapsuitkomsten vaststellen, waarbij verloskundige en neonatale praktijken surveillancetactieken omarmen op basis van de woonlocatie van zwangere vrouwen en hun nakomelingen. Bovendien zal het voorgestelde onderzoek het potentieel aantonen voor vroege herkenning van placenta-insufficiëntie, waardoor toekomstige strategieën van preventieve en interventionele therapieën worden geënsceneerd die gericht zijn op omkering voordat ze nadelige gevolgen ondervinden.
