Een fase 3-onderzoek naar 131I-metaiodobenzylguanidine (131I-MIBG) of ALK-remmertherapie toegevoegd aan intensieve therapie voor kinderen met nieuw gediagnosticeerd hoogrisiconeuroblastoom (NBL)

Deze fase III-studie bestudeert iobenguaan I-131 of lorlatinib en standaardtherapie bij de behandeling van jongere patiënten met nieuw gediagnosticeerd hoog-risico neuroblastoom of ganglioneuroblastoom. Radioactieve geneesmiddelen, zoals iobenguaan I-131, kunnen straling rechtstreeks naar tumorcellen overbrengen en normale cellen niet beschadigen. Lorlatinib kan de groei van tumorcellen stoppen door enkele enzymen te blokkeren die nodig zijn voor celgroei. PRIMAIRE DOELEN:

I. Om in de context van een gerandomiseerde studie te bepalen of de gebeurtenisvrije overleving (EFS) van patiënten met nieuw gediagnosticeerd hoog-risico neuroblastoom (NBL) wordt verbeterd door toevoeging van iobenguaan I-131 (131I-MIBG) tijdens inductie, voorafgaand aan tandem autologe stamceltransplantatie (ASCT).

II. Om te bepalen of de toevoeging van lorlatinib aan intensieve multimodaliteitstherapie voor patiënten met NBL met een hoog risico, bij wie de tumoren activerende puntmutaties in of amplificatie van het ALK-gen met een variante allelfrequentie (VAF) >= 5%, resulteert in superieure EFS in vergelijking met een gelijktijdig behandeld cohort van patiënten met tumoren zonder gedocumenteerde ALK-activerende mutaties.

SECUNDAIRE DOELSTELLINGEN:

I. Om de toxiciteit te beschrijven die gepaard gaat met de behandeling van NBL met een hoog risico met en zonder toevoeging van 131I-MIBG- of ALK-remmertherapie.

II. EFS schatten en toxiciteit beschrijven bij patiënten met nieuw gediagnosticeerde NBL met een hoog risico, gerandomiseerd naar behandeling met een 131I-MIBG-bevattende inductie voorafgaand aan busulfan/melphalan (BuMel) ASCT.

III. Om de algehele overleving (OS) en responspercentages te beschrijven (geëvalueerd volgens International Neuroblastoma Response Criteria [INRC]-criteria voorafgaand aan ASCT en voorafgaand aan postconsolidatietherapie) voor patiënten met hoog-risico neuroblastoom behandeld met of zonder 131I-MIBG of ALK-remmer behandeling.

IV. Om prospectief de relatie tussen het responspercentage volgens de herziene International Neuroblastoma Response Criteria (INRC) en EFS en OS te evalueren bij patiënten met NBL met een hoog risico die werden behandeld met en zonder de toevoeging van 131I-MIBG- of ALK-remmertherapie.

VERKENNENDE DOELSTELLINGEN:

I. Om de stralingsdosis van het hele lichaam, tumorfactoren en gastheerfactoren te evalueren als potentiële voorspellers van werkzaamheid en/of toxiciteit geassocieerd met 131I-MIBG-therapie en transplantatieconditionering.

II. Om eind-inductierespons, EFS en OS te beschrijven volgens specifieke ALK-mutaties, VAF, ALK-amplificatie, de aanwezigheid van aanvullende genomische bevindingen of de toegediende ALK-remmer.

III. Karakteriseren van veranderingen in tumormarkers (circulerend tumordesoxyribonucleïnezuur [DNA], inclusief ALK en andere tumorspecifieke genetische afwijkingen, en circulerend GD2) in de loop van de tijd als reactie op protocoltherapie.

IV. Resultaten van tumor- en gastheerprofilering correleren met eindinductierespons en EFS.

V. Om EFS prospectief te evalueren voor patiënten met MIBG die niet-gretig hoog-risico NBL zijn in vergelijking met patiënten met MIBG-enthousiast hoog-risico NBL die gerandomiseerd zijn naar behandeling zonder 131I-MIBG.

VI. Correleren van Curie-scores berekend op basis van 131I-MIBG-scans na behandeling met eindinductierespons, EFS en OS.

VII. Om veranderingen in beeldgedefinieerde risicofactoren (IDRF's) in de loop van inductietherapie te beschrijven, met correlatie met chirurgische resultaten en lokale faalpercentages na primaire tumorresectie.

VIII. Het definiëren van faalpatronen op het moment van de eerste terugval of progressie bij patiënten met NBL met een hoog risico.

IX. Om de haalbaarheid te bepalen van prospectieve monitoring van ongewenste voorvallen met behulp van elektronische medische dossiers.

X. Ter vergelijking van lokale, centrale en computerondersteunde Curie-scoretoekenning bij baseline en tijdens therapie bij patiënten met MIBG-enthousiaste hoog-risico NBL.

XI. Het vergelijken van late toxiciteiten (waaronder verminderde orgaanfunctie en het optreden van secundaire tumoren) bij patiënten die werden behandeld met 131I-MIBG- of ALK-remmertherapie met late toxiciteiten bij patiënten die deze therapieën niet hebben gekregen.

XII. Om het verband te bepalen tussen huishoudelijke materiële problemen (HMH) en klinische resultaten, inclusief gebeurtenisvrije en algehele overleving, en 131I-MIBG-ontvangst.

XIII. Om de resultaten (EFS, OS en toxiciteit) van patiënten die werden behandeld met postconsolidatietherapie die geen aldesleukine bevat, te vergelijken met historische uitkomstgegevens voor patiënten die werden behandeld met vergelijkbare inductie- en consolidatieregimes gevolgd door postconsolidatietherapie die aldesleukine bevatte.

XIV. Karakteriseren en beschrijven van longitudinale neuropsychologische en gedragseffecten van hoog-risico neuroblastoomtherapie.

XV. Om de verandering in neurologische gedragsresultaten in de loop van de tijd te evalueren bij patiënten met neuroblastoom die werden behandeld met hoog-risico neuroblastoom-therapie plus lorlatinib in vergelijking met alleen hoog-risico-therapie met behulp van ouder- of zelfrapportagemetingen van adaptief, uitvoerend en psychosociaal functioneren.

OVERZICHT: Patiënten worden gerandomiseerd of toegewezen aan 1 van de 5 armen.

Alle patiënten krijgen cyclofosfamide intraveneus (IV) gedurende 15-30 minuten en topotecanhydrochloride IV gedurende 30 minuten op dag 1-5 tijdens cyclus 1 van inductietherapie bij afwezigheid van ziekteprogressie of onaanvaardbare toxiciteit. Patiënten die aan het einde van cyclus 1 nog niet aan een arm zijn toegewezen, kunnen een aanvullende cyclus van cyclofosfamide en topotecan krijgen.

ARM A:

INDUCTIETHERAPIE: Patiënten krijgen cyclofosfamide IV gedurende 15-30 minuten en topotecan hydrochloride IV gedurende 30 minuten op dag 1-5 van cyclus 2 en cisplatine IV gedurende 4 uur en etoposidefosfaat IV gedurende 2 uur op dag 1-3 van cyclus 3 en 5 Patiënten krijgen ook vincristinesulfaat IV gedurende 1 minuut op dag 1 en dexrazoxaanhydrochloride IV gedurende 5-15 minuten, doxorubicinehydrochloride IV gedurende 1-15 minuten en cyclofosfamide IV gedurende 1-6 uur op dag 1-2 van cyclus 4 in de afwezigheid van ziekteprogressie of onaanvaardbare toxiciteit.

CONSOLIDATIE THERAPIE:

HSCT#1: Patiënten krijgen thiotepa IV gedurende 2 uur op dag -7 tot -5 en cyclofosfamide IV gedurende 1 uur op dag -5 tot -2 bij afwezigheid van ziekteprogressie of onaanvaardbare toxiciteit.

HSCT#2: Patiënten krijgen melfalanhydrochloride IV gedurende 30 minuten op dag -7 tot -5, en etoposidefosfaat IV gedurende 24 uur en carboplatine IV gedurende 24 uur op dag -7 tot -4 bij afwezigheid van ziekteprogressie of onaanvaardbare toxiciteit.

POSTCONSOLIDATIETHERAPIE: Patiënten krijgen sargramostim subcutaan (SC) op dagen 1-14, dinutuximab IV gedurende 10 uur op dagen 4-7 van cycli 1-5, en isotretinoïne oraal (PO) tweemaal daags (BID) op dagen 11-24 van cycli 1-5 en dagen 15-28 tijdens cyclus 6 bij afwezigheid van ziekteprogressie of onaanvaardbare toxiciteit.

ARM B:

INDUCTIETHERAPIE: Patiënten krijgen cyclofosfamide, topotecanhydrochloride, cisplatine en etoposidefosfaat zoals in arm A, iobenguaan I-131 IV gedurende 1,5-2 uur op dag 1 vanaf 3 weken na het begin van cyclus 3, en vincristinesulfaat, dexrazoxaanhydrochloride, doxorubicinehydrochloride en cyclofosfamide zoals in arm A beginnen niet eerder dan 35 dagen na de infusie van iobenguaan I-131.

CONSOLIDATIE THERAPIE:

HSCT#1: Patiënten krijgen thiotepa en cyclofosfamide zoals in arm A.

HSCT#2: Patiënten krijgen melfalan, etoposidefosfaat en carboplatine zoals in arm A.

POST-CONSLIDATIETHERAPIE: Patiënten krijgen sargramostim, dinutuximab en isotretinoïne zoals in arm A-D.

ARM C (GESLOTEN VOOR ACCRUAL PER 17 DECEMBER 2020):

INDUCTIETHERAPIE: Patiënten krijgen cyclofosfamide, topotecanhydrochloride, cisplatine, etoposidefosfaat, iobenguaan I-131, vincristinesulfaat, dexrazoxaanhydrochloride, doxorubicinehydrochloride en cyclofosfamide zoals in arm B.

CONSOLIDATIETHERAPIE: Patiënten krijgen busulfan IV gedurende 3 uur op dag -6 tot -3 en melfalanhydrochloride IV gedurende 30 minuten op dag -1 bij afwezigheid van ziekteprogressie of onaanvaardbare toxiciteit.

POST-CONSOLIDATIETHERAPIE: Patiënten krijgen sargramostim, dinutuximab en isotretinoïne zoals in arm A.

ARM D: Patiënten krijgen dezelfde behandeling als arm A.

ARM E:

INDUCTIETHERAPIE: Patiënten krijgen cyclofosfamide, topotecanhydrochloride, cisplatine, etoposidefosfaat, vincristinesulfaat, dexrazoxaanhydrochloride, doxorubicinehydrochloride en cyclofosfamide zoals in arm A. Patiënten krijgen ook lorlatinib PO eenmaal daags (QD) vanaf cyclus 2 voorafgaand aan HSCT #1 in de afwezigheid van ziekteprogressie of onaanvaardbare toxiciteit.

CONSOLIDATIE THERAPIE:

HSCT#1: Patiënten krijgen thiotepa en cyclofosfamide zoals in arm A. Patiënten krijgen ook lorlatinib PO QD tot dag -8 van HSCT#2 bij afwezigheid van ziekteprogressie of onaanvaardbare toxiciteit.

HSCT#2: Patiënten krijgen melfalanhydrochloride, etoposidefosfaat, carboplatine zoals in arm A. Lorlatinib wordt opnieuw gestart wanneer de patiënt ten minste dag +14 post-HSCT#2 heeft bereikt en enterale medicatie kan verdragen, op voorwaarde dat er geen bewijs is van ziekte progressie of onaanvaardbare toxiciteit.

STRALINGSTHERAPIE: Patiënten krijgen lorlatinib PO QD gelijktijdig met radiotherapie bij afwezigheid van ziekteprogressie of onaanvaardbare toxiciteit.

POSTCONSOLIDATIETHERAPIE: Patiënten krijgen sargramostim en dinutuximab zoals in arm A-D. Patiënten krijgen ook isotretinoïne PO tweemaal daags op dag 11-24 van cyclus 1-5 en dag 15-28 van cyclus 6, en lorlatinib PO eenmaal daags op dag 1-28 van cyclus 1-6 bij afwezigheid van ziekteprogressie of onaanvaardbare toxiciteit.

VERVOLG THERAPIE: Patiënten krijgen lorlatininb PO QD op dag 1-28. Cycli herhalen zich elke 28 dagen gedurende 18 maanden bij afwezigheid van ziekteprogressie of onaanvaardbare toxiciteit.

Na voltooiing van de onderzoekstherapie worden patiënten in Arms A-D elke 3 maanden gedurende 18 maanden gevolgd en vervolgens elke 6 maanden gedurende 42 maanden; patiënten in arm E worden elke 3 maanden gedurende 6 maanden gevolgd en vervolgens elke 6 maanden gedurende 42 maanden.

Deelname Criteria

Onderzoekers zoeken naar mensen die aan een bepaalde beschrijving voldoen, de zogenaamde geschiktheidscriteria. Enkele voorbeelden van deze criteria zijn iemands algemene gezondheidstoestand of eerdere behandelingen.

Inclusiecriteria:

Patiënten moeten zijn ingeschreven op ANBL00B1 (NCT00904241) of APEC14B1 (NCT02402244) voorafgaand aan inschrijving op ANBL1531 (NCT03126916)
Patiënt moet >= 365 dagen en =< 30 jaar oud zijn bij diagnose
Patiënten moeten een diagnose van neuroblastoom of ganglioneuroblastoom (nodulair) hebben, geverifieerd door tumorpathologieanalyse of demonstratie van klonten tumorcellen in het beenmerg met verhoogde urinaire catecholaminemetabolieten; de volgende ziektegroepen komen in aanmerking:
- Patiënten met de International Neuroblastoma Risk Group (INRG) stadium M-ziekte komen in aanmerking als ze een van de volgende kenmerken hebben:
  - MYCN-amplificatie (> 4-voudige toename in MYCN-signalen in vergelijking met referentiesignalen), ongeacht aanvullende biologische kenmerken; OF
  - Leeftijd > 547 dagen ongeacht biologische kenmerken
- Patiënten met MS-ziekte in het INRG-stadium met MYCN-amplificatie
- Patiënten met INRG stadium L2-ziekte met MYCN-amplificatie
- Patiënten ouder dan 547 dagen met aanvankelijk de diagnose INRG-stadium L1, L2 of MS en progressie naar stadium M zonder voorafgaande chemotherapie, kunnen zich binnen 4 weken na progressie naar stadium M inschrijven
- Patiënten >= 365 dagen oud bij wie aanvankelijk de diagnose MYCN-versterkte INRG stadium L1-ziekte werd gesteld en die zonder systemische therapie doorgroeien naar stadium M, kunnen zich inschrijven binnen 4 weken na progressie naar stadium M
Patiënten waarvan aanvankelijk werd vastgesteld dat ze een ziekte met een hoog risico hebben, mogen geen eerdere systemische therapie hebben gehad (anders dan topotecan/cyclofosfamide gestart op een opkomende basis en binnen de toegestane timing); patiënten die zijn geobserveerd of behandeld met een enkele chemokuur volgens een neuroblastoomregime met een laag of gemiddeld risico (bijv. volgens ANBL0531, ANBL1232 of vergelijkbaar) voor wat aanvankelijk leek op een niet-hoogrisicoziekte, maar later bleek te voldoen aan de criteria, zullen ook worden behandeld verkiesbaar; patiënten die gelokaliseerde noodstraling krijgen op plaatsen met een levensbedreigende of functiebedreigende ziekte voorafgaand aan of onmiddellijk na het stellen van de definitieve diagnose, komen in aanmerking
Creatinineklaring of radio-isotoop glomerulaire filtratiesnelheid (GFR) >= 70 ml/min/1,73 m^2 of een serumcreatinine op basis van leeftijd/geslacht als volgt:
- 1 tot < 2 jaar: man = 0,6; vrouw = 0,6
- 2 tot < 6 jaar: man = 0,8; vrouw = 0,8
- 6 tot < 10 jaar: man = 1; vrouw = 1
- 10 tot < 13 jaar: man = 1,2; vrouw = 1,2
- 13 tot < 16 jaar: man = 1,5; vrouw = 1,4
- >= 16 jaar: man = 1,7; vrouw = 1,4
Totaal bilirubine =< 1,5 x bovengrens van normaal (ULN) voor leeftijd, en
Serumglutamaatpyruvaattransaminase (SGPT) (alanineaminotransferase [ALT]) < 10 x ULN; voor de doeleinden van deze studie is ULN voor SGPT (ALT) 45
Verkortingsfractie van >= 27% op echocardiogram, of ejectiefractie van > 50% op echocardiogram of radionuclide-angiogram
Geen bekende contra-indicatie voor verzameling van perifere bloedstamcellen (PBSC); voorbeelden van contra-indicaties kunnen een gewicht of maat zijn die kleiner is dan de opvanginstelling haalbaar acht, of een fysieke conditie die het kind beperkt om de plaatsing van een aferesekatheter te ondergaan (indien nodig) en/of de afereseprocedure

Uitsluitingscriteria:

Patiënten met INRG-stadium L2-tumoren zonder MYCN-amplificatie ongeacht tumorhistologie (voldoen mogelijk aan de criteria voor classificatie met hoog risico, maar komen niet in aanmerking voor deze studie)
Patiënten met beenmergfalensyndromen
Patiënten voor wie gerichte radiofarmaceutische therapie gecontra-indiceerd zou zijn vanwege onderliggende medische aandoeningen
Vrouwelijke patiënten die zwanger zijn sinds foetale toxiciteit en teratogene effecten zijn opgemerkt voor verschillende van de onderzoeksgeneesmiddelen; een zwangerschapstest is vereist voor vrouwelijke patiënten die zwanger kunnen worden
Zogende vrouwen die van plan zijn hun baby's borstvoeding te geven
Seksueel actieve patiënten die zich kunnen voortplanten en niet hebben ingestemd met het gebruik van een effectieve anticonceptiemethode voor de duur van hun studiedeelname
