Pre-adolescent stress- en gezondheidsonderzoek
Doel 1: karakteriseren van allostatic load (AL) biomarkers in een steekproef van vroege adolescenten met een laag inkomen (N = 225). Hoeveel kinderen in gezinnen met lage inkomens hebben atypische of afwijkende biomarkerniveaus, op welke biomarkers en in welke combinaties (doel 1a)? Voorspellen AL-biomarkerverhogingen fysieke en mentale gezondheidsproblemen in de vroege adolescentie (doel 1b)? Hoeveel en welk type verandering in AL-biomarkers vindt plaats tussen 11 en 14 jaar (doel 1c)?
Doel 2: Onderzoeken in dezelfde steekproef van vroege adolescenten die zijn blootgesteld aan verschillende niveaus van stress in het vroege leven (ELS), de relatieve bijdragen van ELS, recente (afgelopen jaar) en cumulatieve (sinds 5 jaar) stressblootstellingen aan initiële AL-niveaus op T1 (Doelstelling 2a), en op veranderingen in AL gedurende de twee jaar van de studie (Doelstelling 2b).
Doel 3: Onderzoeken in welke mate coping-middelen, waaronder de coping-vaardigheden van kinderen, de fysiologische zelfregulatie van kinderen en ouderlijke coping-socialisatie, een unieke en synergetische invloed hebben op AL-niveaus en accumulaties bij deze vroege adolescenten.

Gedetailleerde beschrijving
Tegenspoed bij kinderen wordt in verband gebracht met vroegtijdige verouderingsziekten, de meeste vormen van psychopathologie en vroege sterfte, en chronische stress is een cruciaal mechanisme van dit fenomeen. Biologisch geïnformeerde interventies zijn hard nodig om deze verderfelijke cyclus te doorbreken, maar zeer weinig onderzoek heeft potentieel kneedbare psychobiologische risico's en beschermende processen tijdens de kindertijd onderzocht die in interventies kunnen worden gebruikt. Allostatische belasting (AL) - de opeenhoping van biologische beledigingen die worden veroorzaakt door de pogingen van het lichaam om de homeostase te behouden in het licht van chronische stress - wordt erkend als zo'n risicoproces. Het meeste AL-onderzoek is tot nu toe uitgevoerd bij volwassenen, maar het opkomende onderzoek bij adolescenten suggereert dat diepgaand onderzoek van AL-accumulatieprocessen tijdens de vroege adolescentie kritische inzichten kan opleveren die nodig zijn om psychobiologisch krachtige interventies te ontwikkelen. Coping en zelfregulering zijn potentiële kneedbare beschermende processen, maar ze hebben tot nu toe niet veel aandacht gekregen in AL-onderzoek. Dit R01-project zal daarom de accumulatie van AL-biomarkers in de vroege adolescentie onderzoeken, en zal de bijdragen testen van stress (vroege leven, cumulatief, recent) en van coping-middelen (copingvaardigheden, coping-socialisatie van ouders, fysiologische regulatie) aan AL-accumulatie. Daarnaast zullen we associaties onderzoeken tussen AL en het ontstaan van vroegtijdige verouderingsziekten (bijvoorbeeld diabetes type II, metabool syndroom) halverwege de adolescentie.
Verhoogde AL-biomarkers zijn gedetecteerd in monsters van kansarme preadolescenten (bijv. Evans, 2003; Keller et al., 2012; Rogosch et al. 2011), en zijn gecorreleerd met gezondheidsproblemen in deze populatie. Desalniettemin blijven er tal van belangrijke hiaten in ons begrip van jeugd-AL-processen. Ten eerste is AL niet goed gekarakteriseerd in steekproeven van kinderen of adolescenten. Het meeste bestaande onderzoek waarbij kinderen betrokken zijn, richt zich alleen op geselecteerde aspecten van de AL-index (zoals cortisol), en een uitgebreide beoordeling van de belangrijkste klassen van biomarkers die worden aangegeven in AL (metabool, cardiovasculair, immunologisch, neuro-endocrien) bij risicokinderen ontbreekt. Ten tweede is de afwezigheid van klinische benchmarks die AL koppelen aan ziekte bij kinderen een belangrijke beperkende factor, die in combinatie met lagere basispercentages van AL-gerelateerde ziekten bij kinderen vraagtekens plaatst bij de typische praktijk van het berekenen van een AL-index met drempels voor volwassenen (d.w.z. bovenste kwartiel van bereik). Ten derde is het onduidelijk in welke mate recente en cumulatieve stressoren ervaren na de vroege levensfase (geboorte tot 5 jaar) bijdragen aan de ontwikkeling van AL bovenop stress in het vroege leven (ELS) in welke leeftijdsgroep dan ook, en veel AL-onderzoeken doen dat niet. omvatten diepgaande beoordelingen van levensstress. Ten vierde is er weinig bekend over de mate waarin coping-middelen (copingvaardigheden van jongeren, coping-socialisatie) kinderen kunnen beschermen tegen de ontwikkeling en/of verslechtering van AL in de loop van de tijd. Dit is een cruciale vraag, aangezien het onderzoek van de leidende PI heeft aangetoond dat coping-vaardigheden en -middelen risicokinderen (bijv. in armoede) kunnen beschermen tegen veelvoorkomende stressfactoren en dat coping effecten heeft op primaire neuro-endocriene mediatoren van AL, zoals de HPA (Wadsworth et al. al., 2016; Bendezu & Wadsworth, 2017).
Het huidige project bouwt voort op de unieke sterke punten en capaciteiten van de twee PI's en hun mede-onderzoeker en omvat state-of-the-art beoordelingen van stress, middelen om ermee om te gaan en AL om deze processen volledig vast te leggen op meerdere analyseniveaus. Tweehonderd 11-12-jarige Medicaid-geregistreerde patiënten zullen worden aangeworven door ervaren pediatrische onderzoeksverpleegkundigen bij Penn State Hershey Hope Drive Pediatrics. Vijfentwintig extra 11-12-jarige patiënten van wie de familie niet in aanmerking komt voor Medicaid, zullen dienen als vergelijkingsgroep uit de middenklasse voor benchmarking van biomarkers. Deze aan onderzoek gelieerde kindergeneeskundepraktijk voert 40.000 patiëntbezoeken per jaar uit en schrijft patiënten in van zowel landelijke als stedelijke gemeenschappen in centraal PA. Jongeren worden gedurende twee jaar gevolgd en nemen deel aan drie jaarlijkse beoordelingen. Biologische monsters omvatten bloedafnames, nachtelijke urine en speekselmonsters die op elk tijdstip worden genomen. De beoordelingen omvatten verder (a) ouder- en kind-jeugdlevensstress-interviews om indices te verkrijgen van vroege, recente en cumulatieve levensstress, en (b) de Trier Social Stress Test om SAM- en HPA-activeringspatronen van adolescenten en de coping-socialisatie van ouders te meten. Ouder-kind parasympathische co-regulatie en rapporten van psychologische symptomen, ouderschap, ouder-kind gehechtheid en copingvaardigheden zullen ook worden verkregen. We zullen niveaus en correlaten van AL-biomarkers op T1 onderzoeken, evenals voorspellers van de veranderingen die optreden tussen T1, T2 en T3 (AL-accumulatie). Studiedoelen zijn als volgt:
