Inclusiecriteria:

- Deelnemer moet ≥12 jaar oud zijn op het moment dat de proefdeelnemer of wettelijke voogd het formulier voor geïnformeerde toestemming ondertekent.

Alleen voor deelnemers aan het escalatiegedeelte:

- Bevestigde diagnose van primaire of secundaire AML [elk subtype behalve acute promyelocytische leukemie (APL)] volgens de classificatie van de Wereldgezondheidsorganisatie (WHO). Patiënten met AML moeten voldoen aan een van de volgende criteria, a), b) of c) en zijn beperkt tot patiënten zonder beschikbare (of komen niet in aanmerking) therapie met bekend klinisch voordeel.

a) Primary Induction Failure (PIF) AML, gedefinieerd als een ziekte die ongevoelig is voor een van de volgende, i of ii.

i) Een intensieve introductiepoging, per instelling. Inductiepogingen omvatten hoge dosis en/of standaarddosis cytarabine ± een anthracyclines/antraceendion ± een antimetaboliet, met of zonder groeifactor of gerichte therapie bevattende regimes.

Voorbeelden omvatten, maar zijn niet beperkt tot:

      • Bevestigde diagnose van cluster van differentiatie 123 (CD123) + HR-MDS, met een Revised International Prognostic Scoring System (IPSS-R) risicocategorie van intermediair of hoger en zijn beperkt tot degenen zonder beschikbare (of komen niet in aanmerking) therapie met bekende klinische voordeel.
  • Komt niet in aanmerking voor inductietherapie en heeft ≥2 cycli van een van de volgende voltooid: hypomethylerende middelen (bijv. 5 azacitidine of decitabine) en/of venetoclax, chemotherapie of gerichte middelen.
  • Komt niet in aanmerking voor autologe stamceltransplantatie (ASCT) en heeft ≥1 kuur inductietherapie voltooid.

    • Bevestigde diagnose van CD123 + B-ALL zonder extramedullaire laesies waarvoor geen beschikbare (of niet in aanmerking komende) therapie met bekend klinisch voordeel is.

Alleen voor deelnemers aan het uitbreidingsgedeelte:

  • Voor deelnemers aan cohort A: deelnemers die voldoen aan de inclusiecriteria voor AML-patiënten die primair refractair (PIF) waren voor eerdere inductiebehandeling of bij wie ER 6 maanden of minder na een initiële remissie bij eerdere inductiebehandeling optrad.
  • Voor deelnemers aan cohort B: deelnemers die voldoen aan de inclusiecriteria voor AML-patiënten die een late terugval (LR) hebben gehad, meer dan 6 maanden na een initiële remissie bij eerdere inductiebehandeling.

Uitsluitingscriteria:

  • Eastern Cooperative Oncology Group (ECOG) prestatiestatus >2 (≥18 jaar oud). Karnovsky-schaal (16-17 jaar oud)
  • Geschiedenis van een actieve of chronische auto-immuunziekte die therapie nodig heeft of nodig heeft.
  • Tweede primaire maligniteit die actieve therapie vereist. Adjuvante hormonale therapie is toegestaan.
  • Bewijs van actieve leukemie van het centrale zenuwstelsel op het moment van inschrijving, zoals blijkt uit cytologie of pathologie.

  • Eerdere HSCT met recidief langer dan 3 maanden mag alleen worden opgenomen als er gedurende minimaal 4 weken geen immunosuppressie is geweest en er geen bewijs is van graft-versus-hostziekte (GVHD).
  • Ontvangen op het moment van de eerste toediening van een onderzoeksgeneesmiddel (IMP) corticosteroïd als gelijktijdige medicatie met corticosteroïddosis> 10 mg / dag orale prednison of het equivalent,
  • Voorafgaande behandeling met cellulaire therapie, bijv. chimere antigeenreceptor-T-cel (CAR-T) of chimere antigeenreceptor-NK-cel (CAR-NK).
  • Gelijktijdige behandeling met andere geneesmiddelen in onderzoek.
  • Radiotherapie, zelfs als deze palliatief bedoeld is, mag tijdens het onderzoek niet worden gegeven.
  • Profylactisch gebruik van hematopoëtische groeifactoren (bijv. granulocyt-koloniestimulerende factor (G-CSF), granulocyt-macrofaag-koloniestimulerende factor (GM-CSF), erytropoëtine) tijdens de DLT-observatieperiode alleen in het deel Dosisescalatie. - Individuen die op grond van een regelgevend of wettelijk bevel in een instelling zijn ondergebracht; gedetineerden of deelnemers die wettelijk zijn geïnstitutionaliseerd.
  • Zwangere vrouwen en vrouwen die borstvoeding geven.
  • Geschiedenis van solide orgaantransplantatie, inclusief hoornvliestransplantatie.

For subgroup of participants with TD at baseline, the TI rate was defined as the percentage of participants who convert from baseline TD to TI during on treatment period; For subgroup of participants with TI at baseline, the TI rate was defined as the percentage of participants who remain TI during 56-day post baseline period during treatment.
