Klinische toepassing van AI-ondersteunde echografietechnologie bij de preoperatieve evaluatie van schildklierkanker

Deze studie heeft tot doel de toepassing van AI-ondersteunde echografietechnologie bij de preoperatieve beoordeling van schildklierkanker te onderzoeken.
Traditionele echografieonderzoeksgegevens van patiënten met schildklierkanker zullen worden verzameld en AI-systemen zullen worden gebruikt om schildklierknobbeltjes en lymfeklieren te detecteren en diagnosticeren.
In gevallen waarin er onenigheid bestaat tussen de resultaten van de tweedimensionale echografie en het AI-systeem, zal verdere bevestiging worden gezocht via een biopsie.
Vervolgens zullen pathologische resultaten dienen als de ‘gouden standaard’ voor de vergelijking tussen het AI-systeem en de resultaten van traditioneel echografisch onderzoek, waarbij de nauwkeurigheid en betrouwbaarheid ervan wordt beoordeeld.
Door deze onderzoeksinspanning wordt ernaar gestreefd een nauwkeurigere en betrouwbaardere methode voor preoperatieve beoordeling van schildklierkanker aan te bieden, waardoor de klinische besluitvorming wordt ondersteund en de weg wordt vrijgemaakt voor nieuwe toepassingen van AI op het gebied van medische beeldvormingsdiagnostiek.

Deze studie heeft tot doel de toepassing van AI-ondersteunde echografietechnologie bij de preoperatieve beoordeling van schildklierkanker te onderzoeken.
Traditionele echografieonderzoeksgegevens van patiënten met schildklierkanker, waaronder tweedimensionale echografiebeelden, Doppler-stroombeelden in kleur en gedetailleerde kenmerken van schildklierknobbeltjes en lymfeklieren, zoals aantal, grootte, morfologie, echogeniciteit, marges, calcificaties en aspectverhouding, zullen worden verzameld.
Voorafgaand aan de operatie zal een herbeoordeling worden uitgevoerd met behulp van AI-ondersteunde echografietechnologie, en de detectie- en diagnostische resultaten van schildklierknobbeltjes en lymfeklieren door het AI-systeem zullen worden vastgelegd.
In gevallen waarin er discrepantie bestaat tussen de resultaten van tweedimensionale echografie en het AI-systeem, zal een fijne naaldaspiratiebiopsie of een intra-operatieve biopsie worden uitgevoerd om de aard ervan verder te bevestigen.
Na de operatie zullen de pathologische resultaten van elke knobbel dienen als de ‘gouden standaard’ voor vergelijkende analyses tussen het AI-systeem en traditionele tweedimensionale echografische onderzoeken.
De nauwkeurigheid van het AI-systeem bij het detecteren en lokaliseren van knobbeltjes zal worden geanalyseerd, en de gevoeligheid, specificiteit en nauwkeurigheid ervan zullen worden berekend om de diagnostische werkzaamheid en betrouwbaarheid ervan bij de preoperatieve beoordeling van schildklierkanker te evalueren.
Met dit onderzoek wordt beoogd een nauwkeurigere en betrouwbaardere aanvullende diagnostische methode voor de preoperatieve beoordeling van schildklierkanker te verschaffen ter ondersteuning van de klinische besluitvorming.
Daarnaast zullen nieuwe wegen en richtingen voor de toepassing van AI op het gebied van medische beeldvormingsdiagnostiek worden verkend.

Inclusiecriteria:
- Patiënten met preoperatieve pathologische bevestiging van kwaadaardige schildkliertumoren die een chirurgische behandeling ondergaan.
- Patiënten met goedaardige schildkliertumoren, zoals schildklieradenomen die compressiesymptomen veroorzaken, die een chirurgische behandeling ondergaan.
- Patiënten met volledige en hoogwaardige traditionele tweedimensionale kleurenechografiebeelden.
- Volledige postoperatieve pathologierapporten.
- Bereidheid om deel te nemen aan deze klinische proef en ondertekening van geïnformeerde toestemming.
Uitsluitingscriteria:
- Patiënten met een voorgeschiedenis van nekchirurgie of radiotherapie.
- Patiënten met een voorgeschiedenis van kwaadaardige tumoren in andere delen van het lichaam.
- Patiënten met schildklierdisfunctie.
- Onvolledige of slechte kwaliteit traditionele tweedimensionale echografiebeelden in kleur.
- Onvolledige postoperatieve pathologierapporten.
- Weigering om deel te nemen aan deze klinische proef.

Experimenteel: AI-Assisted Preoperative Review
Participants undergo standard preoperative thyroid ultrasonography followed by a standardized AI-assisted repeat examination and surgeon-led review incorporating the locked AI system's thyroid-nodule output.
The AI system does not assess cervical lymph nodes.
All supplementary examinations and final diagnostic and surgical decisions are made by clinicians.

After standard preoperative ultrasonography, the same participant underwent a standardized AI-assisted repeat examination.
The locked system identified and classified thyroid nodules and provided malignancy-score information for surgeon-led review.
It did not assess cervical lymph nodes and was not retrained or recalibrated during the study.
Additional fine-needle aspiration, intraoperative pathologic examination, and surgical management were determined by clinicians using the complete clinical assessment.
