Celem tego badania jest zbadanie zastosowania technologii ultrasonograficznej wspomaganej sztuczną inteligencją w przedoperacyjnej ocenie raka tarczycy. Tradycyjne dane z badania ultrasonograficznego pacjentów z rakiem tarczycy, w tym dwuwymiarowe obrazy ultrasonograficzne, obrazy przepływu w kolorze Dopplera oraz szczegółowa charakterystyka guzków tarczycy i węzłów chłonnych, taka jak liczba, rozmiar, morfologia, echogeniczność, marginesy, zwapnienia i współczynnik kształtu, pozwolą zostać zebrane. Przed operacją przeprowadzona zostanie ponowna ocena z wykorzystaniem technologii ultrasonograficznej wspomaganej AI, a wyniki wykrycia i diagnostyki guzków tarczycy i węzłów chłonnych będą rejestrowane przez system AI. Pooperacyjne wyniki patologiczne każdego guzka będą stanowić „złoty standard” w analizie porównawczej systemu AI z tradycyjnymi dwuwymiarowymi badaniami ultrasonograficznymi. Przeanalizowana zostanie dokładność systemu AI w wykrywaniu i lokalizacji guzków, a także obliczona zostanie jego czułość, swoistość i dokładność w celu oceny jego skuteczności diagnostycznej i wiarygodności w przedoperacyjnej ocenie raka tarczycy. Celem tych badań jest opracowanie dokładniejszej i bardziej niezawodnej, wspomagającej metody diagnostycznej do przedoperacyjnej oceny raka tarczycy, która pomoże w podejmowaniu decyzji klinicznych. Eksperymentalny: AI-Assisted Preoperative Review
Participants undergo standard preoperative thyroid ultrasonography followed by a standardized AI-assisted repeat examination and surgeon-led review incorporating the locked AI system's thyroid-nodule output. The AI system does not assess cervical lymph nodes. All supplementary examinations and final diagnostic and surgical decisions are made by clinicians.
After standard preoperative ultrasonography, the same participant underwent a standardized AI-assisted repeat examination. The locked system identified and classified thyroid nodules and provided malignancy-score information for surgeon-led review. It did not assess cervical lymph nodes and was not retrained or recalibrated during the study. Additional fine-needle aspiration, intraoperative pathologic examination, and surgical management were determined by clinicians using the complete clinical assessment.
