Szczegółowy opis
Celem Teen-LABS jest ułatwienie skoordynowanych badań klinicznych, epidemiologicznych i behawioralnych w dziedzinie chirurgii bariatrycznej nastolatków, poprzez wspólne opracowanie wspólnych protokołów klinicznych i bazy danych chirurgii bariatrycznej, która będzie gromadzić informacje z uczestniczących ośrodków klinicznych przeprowadzających operacje bariatryczne na nastolatkach .
Teen-LABS pomoże połączyć niezbędną wiedzę kliniczną i zasoby administracyjne w celu ułatwienia terminowego i efektywnego przeprowadzenia wielu badań klinicznych. Ponadto stosowanie ustandaryzowanych definicji, wspólnych protokołów klinicznych i narzędzi do gromadzenia danych zwiększy zdolność badaczy do dostarczania sensownych, opartych na dowodach zaleceń dotyczących oceny pacjentów, selekcji i dalszej opieki.
Oprócz badania wyników chirurgicznych, kolejnym szerszym celem Teen-LABS jest lepsze zrozumienie etiologii, patofizjologii i aspektów behawioralnych ciężkiej otyłości u młodzieży oraz wpływu tego stanu na ludzi w czasie.
W trzecim pięcioletnim cyklu finansowania tego projektu dodano kilka dodatkowych długoterminowych celów badawczych, wszystkie ogólnie zgodne z pierwotnymi celami projektu, tj. ocena skuteczności i bezpieczeństwa operacji bariatrycznych wykonywanych w wieku młodzieńczym.

Wśród osób, u których rozpoznano cukrzycę (DM) na początku badania, remisja DM w każdym punkcie czasowym badania pooperacyjnego została zdefiniowana jako: niestosowanie leków na DM i HbA1c < 6,5% lub, jeśli HbA1c nie było dostępne, glikemia na czczo <126 mg/dL.
Wyjściową cukrzycę zdefiniowano na podstawie samoopisu dotyczącego wcześniejszej diagnozy, a także wcześniejszej dokumentacji medycznej od lekarza kierującego, stosowania leków na cukrzycę, wyjściowej wartości HbA1c ≥6,5%, glukozy na czczo ≥126 mg/dl lub wyników tolerancji glukozy doustnie w ciągu ostatnich 6 miesięcy.
Uczestnicy zgłaszający zespół policystycznych jajników, którzy nie spełniali laboratoryjnych kryteriów cukrzycy i nie przyjmowali leków na cukrzycę innych niż metformina, nie byli uważani za chorych na cukrzycę.
Uczestników, którzy przyjmowali metforminę na początku badania w celu kontroli masy ciała lub leczenia insulinooporności, bez udokumentowanych innych wskazań na wcześniejszą diagnozę DM i bez wyników badań laboratoryjnych zgodnych z rozpoznaniem DM, nie uznano za chorych na DM.

BP mierzono w czasie wizyty badawczej, a stosowanie leków kontrolujących BP odnotowywano w formularzu stosowania leków (MED). Nadciśnienie tętnicze definiowano w sposób zgodny z definicją kliniczną nadciśnienia tętniczego: stosowanie leków BP lub skurczowe BP >140 mmHg lub rozkurczowe BP >90 mmHg.
