Celem tego badania jest przetestowanie skuteczności doustnej interwencji odżywczej zawierającej bioaktywne składniki oleju z ryb w celu przyspieszenia gojenia przewlekłych żylnych owrzodzeń podudzi (CVLU) poprzez zmniejszenie przewlekłego stanu zapalnego w miejscach rany, który zapobiega postępowi gojenia. Badanie planuje objąć 248 kolejno kwalifikujących się dorosłych w wieku ≥ 55 lat z CVLU, którzy nadal otrzymują standardową opiekę w dwóch uniwersyteckich poradniach leczenia ran. Uczestnicy zostaną losowo przydzieleni do 2 grup: 12-tygodniowa terapia doustna EPA+DHA (1,87 g/d EPA + 1,0 g/d DHA) lub codzienna terapia doustna placebo. Zadanie podrzędne 1a. Porównaj ekspresję genów cytokin zapalnych przez PMN we krwi (neutrofile i monocyty).

Cel 2. Porównanie poziomu aktywacji PMN (krew, płyn CVLU) i proteaz pochodzących z PMN (płyn CVLU).

Cel 3. Porównanie redukcji obszaru rany, kontrolując kluczowe czynniki, o których wiadomo, że wpływają na gojenie, oraz określenie zależności z mediatorami lipidowymi, cytokinami i aktywacją PMN.

Podzadanie 3a. Porównaj częstość nawrotów CVLU i poziomy badanych zmiennych we krwi między 2 podgrupami w grupie EPA+DHA z wyleczonymi CVLU (po 3 dodatkowych miesiącach terapii EPA+DHA w porównaniu z terapią placebo po upływie 12. tygodnia).

The VEINES-QOL/Sym questionnaire consists of 26 items designed to assess both disease-specific quality of life (VEINES-QOL) and symptom severity (VEINES-Sym) in individuals with venous disorders. The instrument evaluates two separate metrics, both of which are calculated using the mean of standardized z-scores. The typical theoretical range for both is roughly 0 to 100, with a mean population standard of 50. The final score was calculated after calculating the mean of all z scores, which were multiplied by 10, and then added to 50. Higher scores indicate better quality of life and fewer symptoms (i.e., a more favorable outcome). VEINES-QOL/Sym scores are expressed as Z-scores standardized to a reference population, with a mean of 0 and standard deviation of 1. A Z-score of 0 represents the population mean. Higher Z-scores indicate better quality of life and fewer venous insufficiency symptoms, while lower Z-scores indicate worse outcomes.
