背景:
哮喘是一种严重的慢性疾病,影响了大约 1400 万美国人。 哮喘患者每年有超过 1 亿天的活动受限,每年用于哮喘治疗的费用超过 100 亿美元。 哮喘在儿童中比在成人中更为普遍。
儿童因哮喘住院的人数一直在增加。 例如,从 1979 年到 1987 年,在 15 岁以下儿童中,以哮喘为首诊诊断的出院率上升了 43%,从每 10,000 人中有 19.8 人增加到 28.4 人。
黑人的哮喘死亡率高于白人,而且差异还在扩大。 1979 年,男女黑人死于哮喘的可能性大约是白人的两倍。 在过去的十年里,这个比例有所增加,到 1987 年,黑人的哮喘死亡率几乎是白人的三倍。 在儿童中,黑人和白人之间的这些死亡率差异更为显着。
目前关于儿童哮喘的流行病学和自然史的知识尚不完整,但生命早期哮喘与成年后慢性阻塞性肺病 (COPD) 发展之间的关系正变得越来越明显。 具有持续和严重哮喘症状的哮喘儿童在成年早期的肺功能水平低于疾病较轻的儿童。 最近的纵向研究证实,与无症状儿童相比,有症状(主要是喘息)儿童的 FEV1 肺功能增长率有所下降。 在发生 COPD 的人中,肺功能的初始水平是随后通气功能快速下降的最强预测因子。
因此,哮喘患儿的肺功能低于最大达到水平可能使他们在以后的生活中肺功能下降幅度大于正常水平。 尽管治疗对哮喘病程的长期影响尚不清楚,但降低支气管高反应性、最大限度地提高儿童期肺功能和生长的治疗目标可能对终生的肺部健康产生有益影响,并防止进展为不可逆的气流阻塞。
目前有两类药物可用于治疗炎症——皮质类固醇和色甘酸钠。 吸入皮质类固醇的副作用明显少于全身给药。 皮质类固醇不抑制早期哮喘反应,但能有效抑制晚期反应的炎症和支气管高反应性。 吸入皮质类固醇的长期研究表明,通过 FEV1 测量,它对肺功能有有益影响。 然而,人们一直担心长期使用吸入皮质类固醇可能产生的影响。 尽管使用吸入皮质类固醇的流行病学研究没有显示出明显的不良反应,但仍需要对其对儿童生长发育的影响进行大规模随机对照研究。
当 CAMP 在美国启动时,支气管扩张剂治疗是最常见的治疗方法。 两类支气管扩张剂,吸入型 2-肾上腺素能激动剂和口服茶碱,最常用于治疗哮喘。 迄今为止,尚无随机对照研究将这两类抗炎药相互比较,并与支气管扩张剂治疗哮喘病程进行比较。
该倡议由肺病咨询委员会工作组于 1987 年 10 月提出,并在 1988 年 2 月的会议上获得全体委员会的批准,并于 1990 年 5 月获得国家心肺血液咨询委员会的批准。 征求建议书于 1990 年 10 月发布。 奖项于 1991 年 9 月颁发。
设计叙述:
儿童被随机分配到三个治疗组之一,接受以下任一治疗:单独吸入沙丁胺醇、沙丁胺醇联合吸入布地奈德、沙丁胺醇联合奈多罗米。 随机化后,收集了以下方面的数据:人口统计学因素、身体和社会心理发育、临床因素(包括病史和过敏程度)以及生活质量因素(包括活动受限、旷课、急诊室就诊和住院)。 所有受试者都接受了共同的教育计划,不同之处仅在于所提供的有关受试者所用药物的信息。 每个受试者都获得了处理哮喘发作的标准方案。 所有受试者均接受治疗并随访五年,每年每季度访问一次。 招募从 1993 年 7 月开始,到 1995 年 6 月结束,招募了 1,041 名受试者。
该研究已通过三个资助阶段延长至 2011 年 6 月,以观察受试者但不提供哮喘治疗。 这将使 CAMP 能够 (1.) 确定 4 至 6 年的抗炎治疗对获得最大肺功能和最终身高的全部影响; (2.) 检查 26 岁之前的哮喘自然史; (3.) 定义年轻成人肺功能生长减慢和肺功能早期下降的模式。

- 筛选时年龄为 5 至 12 岁
- 在过去一年中至少有 6 个月的以下一项或多项历史发现证明是慢性哮喘:
- 每周至少出现 2 次哮喘症状
- 每周使用吸入支气管扩张剂 2 次或更多次
- 日常哮喘药物
- 在 prn 筛选期间的 8 天或更多天里,日记症状代码的当前哮喘症状为 1 或更高,或者上午或下午 PEFR 低于日记个人最佳支气管扩张后值的 80%
- 乙酰甲胆碱敏感性:估计 PC20 FEV1 小于或等于 12.5 mg/ml
- 监护人的同意和儿童的同意
- 能够遵守试验 5 - 6.5 年
排除标准:
- 存在以下一项或多项混淆或复杂问题:
- 任何其他活动性肺部疾病
- 任何被认为会干扰项目成功完成或混淆其解释的慢性病

- 严重的慢性鼻窦炎或鼻息肉病

- 目前使用甲氧氯普胺、雷尼替丁或西咪替丁

- 以下一项或多项表明严重哮喘的证据:
- 过去一年因哮喘住院两次或以上
- 在过去的一年中有六次或更多的类固醇爆发
- 证明需要连续使用口服或吸入糖皮质激素
- 当停止吸入 O2 激动剂超过 4 小时并停止吸入茶碱超过 24 小时时,FEV1 低于预计值的 65%
- 过去任何时候因哮喘插管
- 连续 3 天每天需要 9 次或更多喷沙丁胺醇(不包括运动前的预防性使用),或平均每周夜间哮喘觉醒超过 1.5 次,或平均日记卡症状代码大于 2,或要求在 prn 筛选期间使用其他药物控制哮喘
- 无法执行 3 次可接受的 FVC 操作,其中至少 2 次可重复的 FEV1 在最大 FEV1 的 10% 以内
- 无法完成乙酰甲胆碱挑战或乙酰甲胆碱 PC20 FEV1 大于 12.5 mg/ml
