基本的
- 估计和比较接受保乳手术治疗的导管原位癌 (DCIS) 女性的局部无复发生存期,然后进行全乳照射并进行与不进行放射瘤床推量的比较。

- 比较两组之间的无复发生存期。
- 比较总生存期。
- 比较急性和晚期毒性。
- 比较美容效果和生活质量。
- 使用生物测试识别有晚期毒性风险的患者。

- 将血液样本存储在 CEPH(人类多态性中心的法国中心)进行多态性分析。
大纲:这是一项多中心研究。 所有患者均接受保乳手术(单次介入或二次切除后),切缘 ≥ 1 mm。 如果手术分别离开了皮肤组织或胸筋膜肌,则浅表或深部边缘是可以接受的。 十二周后,患者被随机分配到 2 个治疗组中的 1 个。
- 第 I 组:患者每天接受一次全乳照射 (WBI),每周 5 天,持续 5 周。
- 第 II 组:患者像第 I 组一样接受 WBI。患者还在 2 周内每天接受 8 次(仅限工作日)放射肿瘤床推量。

经组织学证实的乳腺导管原位癌
- 无侵入性成分
- 没有临床可触及的淋巴结或前哨淋巴结中存在肿瘤细胞(如果有)
- 原发性乳腺癌无局部复发
- 无不适合保守手术的多中心或多灶性肿瘤 R0 切缘 ≥ 1 mm
- 在随机分组前 6 个月内接受过双侧乳房 X 光检查
患者特征:
- ECOG 体能状态 0-2
- 预期寿命 > 5 年
- 未怀孕
- 可用于长期随访
- 对侧乳房无原位癌病史
- 除皮肤基底细胞癌或宫颈原位癌外,无其他癌症病史
- 无不受控制的心脏、肾脏或肺部疾病
- 无不受控制的全身性疾病(例如,红斑狼疮或硬皮病)
- 没有艾滋病毒阳性
- 隶属于社会卫生系统
