Tato randomizovaná kontrolní studie (RCT) bude testovat hypotézu, že vzorce fyziologické odezvy kortizolu ve slinách, kardiovaskulární a elektrodermální aktivity odpovídají za naše dříve pozorované významné výsledky (tj. snížená podrážděnost a hyperaktivita a zlepšení sociální a komunikační schopnosti) a další výsledky ( regulace emocí pečovatel kvalita života a využití péče o duševní zdraví v krizi), u mládeže ve věku 6-16 let. s ASD a souběžnými psychiatrickými diagnózami randomizovanými do 10týdenní manuální intervence THR ve srovnání s kontrolou Barn Activity (BA) bez koní (cíl 1). Budeme hodnotit trvanlivost výsledků Cíle 1 ve skupině THR ve srovnání s kontrolní skupinou BA šest měsíců po období intervence (Cíl 2).   • dokumentovaná diagnóza ASD a souběžná psychiatrická porucha
  • Skóre subškály ABC dráždivost ≥8
  • Leiter-III Neverbální IQ ≥ 40
  • splnit Symptom Criterion score (minimální počet symptomů nezbytných pro DSM-V (nálada, úzkost nebo diagnóza ADHD) na CASI-5)
  • splňují mezní hodnoty ASD na SCQ (≥ 11) a na ADOS-2
  • Pouze jedno dítě s PAS na rodinu, aby byla zachována nezávislá pozorování
  • důsledný pečovatel (tj. rodič nebo zákonný zástupce), aby dokončil měření výsledků studie

Kritéria vyloučení:

  • zdravotní nebo behaviorální problémy, které brání účasti

Ti, kteří jsou zařazeni do skupiny čekatelů, nebudou mít během 10týdenní čekací doby žádný zásah související s koněm. Po této čekací době a dokončení následného hodnocení zahájí účastníci v tomto stavu hybridní skupinu (viz Hybridní rameno)
Experimentální: Terapeutická jízda na koni
This Arm je 10týdenní hodinová malá skupina (2-4 účastníci) vedená instruktorem THR. Skupina bude zahrnovat 45minutovou montovanou aktivitu, aby se naučili jezdecké dovednosti, jak je uvedeno v příručce THR studie, . Časy skupin budou mezi 13:00 a 17:00 pro sběr slinného kortizolu. Každý týden budou účastníci dodržovat důslednou rutinu nošení zařízení pro měření elektrodermální aktivity a srdečního tepu, před lekcí se posadí ke skupinovému uměleckému stolu a poté studijní personál dá účastníkům pokyn, aby si 10 cm dlouhou pěnovou tyčinku umístili pod jazyk. jednu minutu při sledování 1minutového časovače. Účastníci budou mít přiděleného jednoho dobrovolníka a nebudou mít žádný fyzický kontakt s koňmi v jezdeckém centru, pouze si koně prohlédnou z dálky. K dispozici bude plyšový kůň v životní velikosti pro praktické učení týkající se týdenního tématu podle studijní příručky BA. Časy skupin budou mezi 13:00 a 17:00 pro sběr slinného kortizolu. Každý týden budou účastníci dodržovat důslednou rutinu nošení zařízení pro měření elektrodermální aktivity a srdečního tepu, před lekcí se posadí ke skupinovému uměleckému stolu a poté studijní personál dá účastníkům pokyn, aby si 10 cm dlouhou pěnovou tyčinku umístili pod jazyk. jednu minutu při sledování 1minutového časovače. Účastníci budou mít přiděleného jednoho dobrovolníka a nebudou mít žádný fyzický kontakt s koňmi v jezdeckém centru, pouze si koně prohlédnou z dálky. K dispozici bude plyšový kůň v životní velikosti pro praktické učení týdenních témat podle studijní příručky BA. Skupina. Poté účastníci absolvují 5týdenní terapeutickou jízdu na koni v malé skupině (2-4 účastníci) pod vedením instruktora THR. Skupina bude zahrnovat 45minutovou jízdu na koni, aby se naučila jezdeckým dovednostem, po níž bude následovat 15minutová činnost na koni nasedání a připínání podle THR manuálu. Účastníci budou mít přiděleného koně a dobrovolníka (dobrovolníky).

It samples ECG and HRV up to 2,048 Hz, which is sufficient for research.125 It has a built-in accelerometer for quantifying periods of physical activity, necessary to identify signal artifacts in post-processing. Participants' Shimmer3 ECG assessments were measured continuously during the intervention lesson. Unit of measure is ms log-10 transformed.

Electrodermal Activity -- Baseline

The Shimmer3 EDA monitor was used to record EDA from participants' ventral area of the wrist using alternating current imperceptibly applied to the skin through two hypoallergenic, durable, and replaceable Ag electrodes. As dry non-adhesive electrodes tend to move on the skin surface during physical activity and destroy signal quality, disposable adhesive electrodes with an appropriate salinity (e.g., 0.5%; EL507) were used. EDA sampling frequency in the Shimmer3 is 4 Hz with a 0.01- 100uS range. Peripheral skin temperature was recorded by the Shimmer3 EDA monitor at 4 Hz in the -40 to 115-Celsius range using optical infrared thermopile. The Shimmer3 EDA recorded motion-based activity up to ±8g at 32 Hz using 3-axis accelerometry. Participants' Shimmer3 EDA assessments were measured during the intervention lesson. Unit of measure is microsiemens.
