Vliv veteránských hlasů a vize skupin kolegiální podpory na sociální integraci veteránů se SMI/psychózou

Veteráni s vážným duševním onemocněním (SMI) bojují se sociální integrací – účastí na práci, bydlení a občanství – kvůli symptomům, stigmatizaci a psychosociálním problémům. Navzdory značnému úsilí VA poskytovat péči o duševní zdraví veteránům se SMI programy, které podporují sociální integraci, chybí. Veteráni s SMI jsou během pandemie COVID-19 vystaveni obzvláště vysokému riziku špatné sociální integrace a sebevražedných myšlenek. Tento projekt řeší tuto potřebu pomocí skupinové psychosociální intervence pro veterány se SMI, která je spoluorganizována odborníky na vrstevníky (PS), nazvanou „Veteran Voices and Visions“ (VVV). VVV se zaměřuje na veterány s SMI, kteří trpí psychózou, což je skupina, která zvláště potřebuje podporu při sociální integraci. Virtuální VVV skupiny jsou společně vedeny VA kliniky duševního zdraví (MHC) a PS prostřednictvím online videokonference. Tento přístup usnadňuje skupinovou soudržnost a normalizaci běžných psychotických symptomů SMI: halucinace, bludy a sociální izolace. Tato intervence má potenciál vytvořit a podporovat podpůrnou komunitu, která zlepšuje sociální integraci účastníků snížením jejich úzkosti a sebestigmatizace a zvýšením vlastní účinnosti.

  • DSM 5 diagnózy schizofrenie, schizoafektivní porucha, bipolární s psychózou, deprese s psychózou, blíže neurčená psychóza

Kritéria vyloučení:

  • psychóza vyvolaná látkou,
  • klinicky významné neurologické onemocnění,
  • anamnéza vážného poranění hlavy se ztrátou vědomí > 1 hodina.

Experimentální: Veteran Voices & Visions (VVV)
Veteran Voices and Visions is the experimental condition. It is a group of Veterans who are experiencing chronic psychosis. The group meets for 60 minutes, weekly, for 12 weeks. The investigators adapted a manual and protocol for the VA and this study is testing this manual. This is a single-arm study.
Behaviorální: Veteran Voices and Visions
Clinician and peer co-led meaning-oriented support group based on the Hearing Voices approach.

The Psychotic Symptoms Rating Scale (PSYRATS-AH)
Časové okno: 12 weeks after the initiation of the intervention
PSYRATS-AH is an 11-item scale which assesses the frequency, duration, severity, loudness, location, negative content, and controllability of voices, intensity of distress, and beliefs about origin of voices and disruptiveness. Minimum score is 0, maximum 44. Any decrease in overall score is indicative of decreased distress.

Popis plánu IPD

PI vytvoří neidentifikované soubory dat specifické pro studii, které zahrnují všechny proměnné v publikaci. Vzhledem k nutnosti odstranit PHI může dojít k určité ztrátě informací. PI nahradí čísla sociálního zabezpečení a lékařských stanic čísly specifickými pro studii. PI vypustí datum narození a nahradí věk věkovými kategoriemi v souladu s požadavky PHI pro osoby ve věku 85 let a starší. Data služby budou nahrazena ročními figurínami.

Časový rámec sdílení IPD

Datové sady splňující standardy VA pro zveřejnění budou zpřístupněny do 1 roku od zveřejnění.

Kritéria přístupu pro sdílení IPD

Před distribucí místní úředník pro ochranu osobních údajů potvrdí, že všechny datové sady neobsahují žádné PHI. Finální datové sady budou uchovávány lokálně, dokud nebudou dostupné zdroje na podnikové úrovni pro dlouhodobé ukládání a přístup. Pokyny na vyžádání a distribuční procesy poskytne ORD.
