Primární cíl:
I. Posoudit bezpečnost, snášenlivost a toxicitu omezující dávku (DLT) u pacientů s pokročilým hepatocelulárním karcinomem (AHCC, cirhóza třídy A a B) a stanovení doporučené dávky fáze II (RP2D) se zanzalintinibem. (Pouze eskalace dávky fáze IB)
Sekundární cíle:
I. Posoudit podíl AHCC Zanzalintinib léčených pacientů, kteří jsou naživu a medián přežití bez progrese (PFS) léčby. (Fáze IB a fáze II) II. Posoudit medián celkového přežití (OS) u pacientů léčených AHCC Zanzalintinib. (Fáze IB a fáze II) III.

- 3.2.23 Patients with psychiatric illness/social situations that would limit compliance with study requirements or give informed consent, per the opinion of the treating investigator.
- 3.2.24 Patients with other conditions which, in the opinion of the Investigator, would compromise the safety of the patient or the patient's ability to complete the study.
- 3.2.25 Patients with documented hepatic encephalopathy (HE) within 6 weeks before first dose of study treatment. Patients with clinically meaningful ascites (i.e., ascites requiring paracentesis or escalation in diuretics) within 2 weeks prior to registration, C1D1.

Dose-limiting toxicity (DLT) and Recommended Phase 2 Dose (Phase Ib dose-escalation only)
Časové okno: Through the duration of cycle 1 (cycle length = 28 days)
|
For the primary endpoint, the recommended phase II dose (RP2D) of zanzalintinib and the associated drug-related toxicity will be evaluated using descriptive statistics to summarize the frequency and severity of dose-limiting toxicities (DLTs) and adverse events (AEs).
These events will be categorized according to the Common Terminology Criteria for Adverse Events (CTCAE) version 5.0.
Patients who complete the DLT evaluation period will be included in the analysis, and any patient who does not experience a DLT but discontinues the study prematurely will be replaced and not included in the final analysis.
The number and proportion of patients experiencing DLTs will be reported, with each patient counted only once, regardless of the number of DLTs experienced.

Střední přežití bez progrese (PFS) (fáze IB a II)
Časové okno: Od začátku zkušební terapie (cyklus 1 den 1 [délka cyklu = 28 dní]) do progrese onemocnění, zahájení následné protirakovinové terapie, dokončení studie nebo smrt z jakékoli příčiny, podle toho, co nastane nejprve, bylo posouzeno až 1 rok
|

Overall survival (Phase Ib and II)
Časové okno: From the start of trial therapy to the time of death from any cause, assessed up to 1 year
|
This endpoint will evaluate median overall survival (OS) rate.
OS data will be collected from time of diagnosis until the patient initiates subsequent anti-cancer therapy, completes study participation, or experiences death from any cause (whichever is sooner).
If death from any cause is not observed prior to initiating subsequent anti-cancer therapy or completing study participation, the OS will be censored as the last available follow-up.
