Zanzalintininib on lääkkeiden luokassa, jota kutsutaan tyrosiinikinaasin estäjiksi.

Tutkimuksen yleiskatsaus

Yksityiskohtainen kuvaus

Ensisijainen tavoite:

I. Arvioida turvallisuuden, siedettävyyden ja annosta rajoittavan toksisuuden (DLT) arvioimiseksi potilailla, joilla on pitkälle edennyt hepatosellulaarinen karsinooma (AHCC, Child-Pugh-luokka A ja B-maksakirroosi) ja suositellun vaiheen II annoksen (RP2D) määrittämisen kanssa Zanzalininibillä. (Vaiheen IB annoksen lisääntyminen vain)

Toissijaiset tavoitteet:

Arvioi mediaani yleinen eloonjääminen (OS) AHCC -zanzalintinibi -hoidetuissa potilaissa. (Vaihe IB ja vaihe II) III. Arvioi AHCC -zantsalintinibi -hoidettujen potilaiden osuus, joilla on objektiivinen vaste hoitoon kuvantamisella.

Tutkittava (korrelatiivinen) tavoite:

I. Arvioi korrelaatioiden ja farmakokinetiikan (PK) profiili biomarkkereiden arvioinnilla zantsalintinibi -potilaiden AHCC -potilaiden hoidossa.

LUOTTAMINEN: Tämä on vaiheen IB-annos-eskalaatiotutkimus, jota seuraa vaiheen II annos-laajennustutkimus.

Ensisijaiset tulostoimenpiteet

Tulosmittaus
Toimenpiteen kuvaus
Aikaikkuna
Dose-limiting toxicity (DLT) and Recommended Phase 2 Dose (Phase Ib dose-escalation only)
Aikaikkuna: Through the duration of cycle 1 (cycle length = 28 days)
For the primary endpoint, the recommended phase II dose (RP2D) of zanzalintinib and the associated drug-related toxicity will be evaluated using descriptive statistics to summarize the frequency and severity of dose-limiting toxicities (DLTs) and adverse events (AEs). These events will be categorized according to the Common Terminology Criteria for Adverse Events (CTCAE) version 5.0. Patients who complete the DLT evaluation period will be included in the analysis, and any patient who does not experience a DLT but discontinues the study prematurely will be replaced and not included in the final analysis. The number and proportion of patients experiencing DLTs will be reported, with each patient counted only once, regardless of the number of DLTs experienced.
Through the duration of cycle 1 (cycle length = 28 days)

Mediaani etenemisvapaa eloonjääminen (PFS) (vaihe IB ja II)
Aikaikkuna: Kokeilun alusta alkaen (sykli 1 päivä 1 [syklin pituus = 28 päivää]), kunnes taudin eteneminen, myöhemmän syövän vastaisen hoidon aloittaminen, tutkimuksen valmistuminen tai minkä tahansa syyn kuolema, joka tapahtuu ensin, arvioidaan enintään yhden vuoden
Määritetään radiografisella kuvantamisella, joka arvioidaan vasteen arviointikriteereillä kiinteissä kasvaimissa (RECIST) V 1.1 -kriteerit. Analysoidaan Kaplan-Meier-eloonjäämisanalyysillä. Kaplan-Meier-käyrät syntyvät PFS-hintojen arvioimiseksi tietyissä ajankohtina, ja mediaani PF: t ilmoitetaan 95%: n luottamusväleissä. Lisäksi COX: n suhteellisten vaarojen regressiomalleja voidaan käyttää mahdollisten sekoittajien sopeutumiseen ja etenemisen tai kuoleman vaarasuhteiden arvioimiseen.

Objektiivinen vastausprosentti
Aikaikkuna: Enintään vuosi
Määritellään käsitellyiden kohteiden osuudeksi, jotka kokevat vahvistetun täydellisen vasteen tai vahvistetun osittaisen vasteen RECIST: tä kohti V 1.1.
Enintään vuosi
Overall survival (Phase Ib and II)
Aikaikkuna: From the start of trial therapy to the time of death from any cause, assessed up to 1 year
This endpoint will evaluate median overall survival (OS) rate. OS data will be collected from time of diagnosis until the patient initiates subsequent anti-cancer therapy, completes study participation, or experiences death from any cause (whichever is sooner). If death from any cause is not observed prior to initiating subsequent anti-cancer therapy or completing study participation, the OS will be censored as the last available follow-up.
