Esta investigación debería aumentar nuestra comprensión de la variación humana, la genética de la adaptación al entrenamiento físico y los cambios concomitantes en los factores de riesgo de enfermedades cardiovasculares y diabetes.
NARRATIVA DE DISEÑO:
Se reclutó un total de 742 sujetos sedentarios, inicialmente evaluados, entrenados en el laboratorio con el mismo programa durante 20 semanas y nuevamente evaluados. Los sujetos procedían de familias de ascendencia caucásica con ambos padres y tres hijos adultos biológicos y familias de ascendencia afroamericana. El consumo de oxígeno, el volumen espiratorio y el índice de intercambio respiratorio, la presión arterial, la frecuencia cardíaca, el lactato en sangre, la glucosa, el glicerol y los ácidos grasos libres, el volumen sistólico y el gasto cardíaco se midieron durante el ejercicio antes y después del entrenamiento y se determinó el consumo máximo de oxígeno. También se evaluaron los lípidos, las lipoproteínas y las apoproteínas del plasma, la tolerancia a la glucosa y la respuesta de la insulina a una carga de glucosa intravenosa, los esteroides sexuales y los glucocorticoides en el plasma, las presiones arteriales sistólica y diastólica en reposo, y la grasa corporal y la distribución regional de la grasa. Los hábitos dietéticos, el nivel de actividad física habitual y otros componentes del estilo de vida se evaluaron mediante cuestionarios. Los análisis genéticos incluyeron la determinación del nivel de heredabilidad para cada fenotipo y su respuesta al ejercicio regular, la prueba de la presencia de efectos paternos o maternos, efectos limitados por sexo, efectos genéticos principales y patrones de segregación. Se utilizaron análisis genéticos multivariados y análisis de segregación compleja para desarrollar hipótesis sobre la base genética de la respuesta al entrenamiento físico.
El estudio se renovó en septiembre de 1997 para realizar análisis de los datos recopilados en la Fase I. Se llevó a cabo una serie de estudios no genéticos sobre el conjunto de datos. Se investigaron los determinantes fisiológicos, conductuales y sociales de los indicadores máximos y submáximos de la resistencia cardiorrespiratoria en el estado sedentario y en la respuesta al entrenamiento teniendo en cuenta las contribuciones de la edad, el sexo y la raza. Se realizaron análisis similares sobre los factores de riesgo de enfermedad cardiovascular y diabetes mellitus no dependiente de insulina (NIDDM) monitoreados en el estudio. Los análisis genéticos determinaron los niveles de heredabilidad y evaluaron los efectos paternos o maternos, los principales efectos genéticos y los patrones de segregación que se utilizaron para desarrollar hipótesis sobre las bases genéticas de la respuesta al ejercicio de resistencia. Se tipificó un panel de genes candidatos y se usó para estudios de asociación y ligamiento. Se utilizó el análisis de visualización diferencial de las transcripciones del músculo esquelético para identificar nuevos genes candidatos para la respuesta al ejercicio de resistencia. Finalmente, se llevó a cabo una búsqueda en todo el genoma para aislar las regiones genómicas candidatas y los genes candidatos posicionales para la respuesta de la resistencia cardiorrespiratoria y los fenotipos de factores de riesgo cardiovasculares y NIDDM.
El estudio se renovó en 2001 por cuatro años para continuar con los análisis de los datos.
