Yksityiskohtainen kuvaus
Tutkijat suorittavat pilotti-, yksihaaraisen, avoimen lääkekokeilun interventiotutkimuksen 30 nuorelle ja vanhemmalle, jotka kokevat epävarmuutta lääkkeiden käytön jatkamisesta. Koska lääkekokeen interventio ei ole kliinisesti merkityksellinen nuorille, jotka käyttävät ei-stimuloivaa ADHD-lääkitystä, heidät suljetaan pois. Ilmoittautumisen yhteydessä perheet suorittavat perusmittaukset sähköisesti ja varaavat henkilökohtaisen vierailun. Tutkijat tarkistavat annostustietojen perusteella määrätyt lääkkeet ja lääkkeillä kuluneiden päivien lukumäärän kuluneen vuoden aikana. Vierailun aikana vanhemmat ja nuoret valitsevat yhden viidestä lääkityskokeesta, jotka suoritetaan yhdessä ja seurataan vaikutuksia halutun ajan.
- Älä tee muutoksia. Seuraa nykyistä tilaa: jatka lääkkeiden ottamista senhetkisen määräyksen mukaan.
- Ota lääkkeitä koulupäivinä. Seuraa vaikutuksia: Jos et käytä lääkkeitä koulun ulkopuolisina päivinä, aloita lääkkeiden käyttö kyseisinä päivinä ja seuraa vaikutuksia.
- Lopeta lääkkeiden käyttö koulun ulkopuolisina päivinä. Seuraa tehosteita. Jos käytät parhaillaan lääkkeitä koulun ulkopuolisina päivinä, lopeta lääkkeiden käyttö näinä päivinä ja seuraa vaikutuksia.
- Tee virallinen lääkekoe. Ota nykyinen lääkitys tällä hetkellä määrätyllä tavalla 2 viikon ajan seurannan aikana, lopeta sitten lääkitys 2-4 viikoksi ja jatka seurantaa.
- Muuta annosta tai vaihda lääkitystä. Harkitse toista lääkitystä tai nykyisen lääkityksen eri annosta yhdessä tutkimuslääkärin kanssa. Seuraa tehosteita.
Kun dyadit ovat saaneet lääkityskokeen päätökseen, he suorittavat seurantakäynnin tarkastelemaan kokeen tuloksia, keskustelemaan ohjelmiston toteutettavuudesta ja suorittamaan tutkimuksen lopetustoimenpiteet.

Kuvaus
Osallistumiskriteerit: Tutkimukseen osallistujien on täytettävä kaikki seuraavat kriteerit:
- Suostumus: Vanhemman tai laillisen huoltajan on annettava kirjallinen tietoinen suostumus
- Suostumus: Nuorten on annettava kirjallinen suostumus osallistuakseen tutkimukseen
- Ikäraja 11-15
- Lastenlääkärin hoitama ADHD
- Ensimmäinen ADHD-lääkemääräys yli vuosi ennen ilmoittautumista
- Täytti vähintään yhden piristyslääkkeen reseptin viimeisen vuoden aikana
- Epävarmuus jatkuvasta ADHD-lääkkeiden käytöstä
- Vain yksi lapsi per kotitalous voi osallistua tutkimukseen. Perheet, joissa on useampi kuin yksi mahdollisesti tukikelpoinen lapsi, voivat päättää, mikä heidän lapsistaan sopisi parhaiten tutkimukseen.

Decisional Conflict - Parent Report
Aikaikkuna: The DCS was collected at the first study visit, and then again after the medication experiment was completed (between 4 and 12 weeks later)

The Decisional Conflict Scale (DCS) has an initial question to ascertain the option that has been selected.
Options presented will be: continue [medication regimen from experiment], resume previous medication/dosage, conduct another experiment, return to pediatrician to adjust dosage or change medication, other [write in].
The DCS then has 16 questions that are divided into five subscales: informed subscale, values clarity subscale, support subscale, uncertainty subscale, and an effective decision subscale.
The total DCS scores are calculated by summing item scores and then converting the scores to a 0-100 scale, where 0 implies no decisional conflict and 100 implies extremely high decisional conflict.

System Usability Scale Total Score

Parents and children completed the System Usability Scale (SUS) to assess the usability of the mehealth platform for completing a personalized medication experiment.
The SUS is a 10-item questionnaire with items rated on a 5-point agreement scale.
Item scores are converted and summed using standard scoring procedures to produce a total score ranging from 0 to 100, with higher scores indicating better usability/better outcome.

Implementation of Decision

We obtained pharmacy dispensing records to confirm eligibility and to assess decision implementation within 90 days post-study, identifying cases where pharmacy records contradicted the dyad's decision about their future ADHD treatment.
